OUR MISSION
Heartland Housing is a community development organization which facilitates the development of people through the preservation and creation of affordable and supportive housing with a priority in meeting the needs of homeless persons and families, very low income individuals, and persons with chronic disabilities.
OUR MODEL 1. We focus on the heartland, currently serving residents and communities in Chicagoland, Madison and Milwaukee, and evaluating new markets throughout the Midwest
2. Our unique approach involves a resident-driven design process, high-quality and environmentally sustainable construction, and responsive management and supportive services
3. Our business model includes three key components:
A real estate development company
A property management and maintenance company
Critical partnerships for the delivery of services

HHO delivers integrated medical and behavioral healthcare to people with complex health needs
OUR SERVICES
PRIMARY CARE: HHO is a specialty federally qualified health center focused on providing care to the homeless
COMMUNITY HOUSING: HHO provides permanent supportive housing through scattered site apartments leased across Chicagoland
BEHAVIORAL HEALTH: HHO provides outpatient mental health and substance abuse services to people experiencing homelessness
OUR SERVICES: PROJECT-
BASED HOUSING HHO provides permanent supportive
housing with intensive wrap around services at 3 developments including Rethke Terrace
Eligible population: people experiencing homelessness who are seriously mentally ill and/or have addictions disorders
31,200 nights of housing provided per year

HOUSING MODEL Permanent Supportive Housing is a national model to
help people move from homelessness to stability.
Rent is 30% of income
Housing is permanent
Professional, onsite staffing
Outcomes include: reduced homelessness; improved self-sufficiency, health and economic indicators; savings to public programs compared to previous usage of emergency support systems.
