Proposing a Solution Proposes a solution or approach to solving the problem or addressing the need identified
The proposed solution is a model or evidence-based program
The identified solution includes evidence of effectiveness or is proven to work in the target population
Identifying Indicators of Success
Outcome measures or indicators of success are included
Proposed outcome measures or indicators of success are logically linked or associated with the identified need or problem
The proposed outcome measures or indicators of success are based on past experience, industry standards or research that demonstrates a relationship between the solution and the identified problem, need or issue – the selected measures fully support checking the solution against identified issue
