Medical care for Los Angeles AFB members is provided by the 61st Medical Group (Building 210). A satellite clin-ic is located at the Fort MacArthur military housing area. Emergency medical care is not available at either clin-ic. For life threatening injuries or illnesses, call 911 or proceed to the nearest emergency room. After hours care: To obtain authorization for non-emergency care or for out of area care, call TRIWEST at 1-888-874-9378.

The LAAFB clinic is open on weekdays from 7 a.m. to 4 p.m. All patients are seen by appointment only. For appointments or information, call (310) 653-2873. The clinic is closed on the first Thursday of every month for training

Dental Clinic

The clinic is open Monday - Friday 0700 - 1600 by appointment only. Call (310) 653-2873. Dental sick call is Monday - Friday 0730 - 0820. Dental sick call can only see active duty members on orders, by appointment only.

BASE SERVICES On Call Service Provider

On Call Provider Service is available to patients when the clinic is closed. After business clinic hours are 4 pm to 7 am, Monday through Friday, and all day on weekends, holidays, training and family days. Patients can access this service by calling 310-653-2873, option number 7. A clerk will ask for the name of the caller, sponsor’s last four of social security number, patient’s name, telephone number and chief complaint. A physi-cian assistant or a doctor will return the call within 30 minutes and provide telephone consultation regarding the medical situation. Urgent care referrals may be authorized for patients assigned to the 61st Medical Group. Patients visiting the area and need an urgent care authorization will need to call their primary care medical unit at home base. No authorization is required for emergency care such as stroke, heart attack, major limb and eye injury, broken bones sticking out of skin, seizures or loss of consciousness. In such cases, call 911 or proceed to nearest emergency room immediately. 61st MSG Services exists to provide combat support to commanders directly in support of the Air Force mission. The Services Division at Los Angeles Air Force Base operates a consolidated club, a child development center, a youth center, a community center, two fitness centers, an Air Force Inn, a human resources office and an outdoor recreation operation that includes equipment rentals, a ticket and information office and an outdoor swimming pool.

The Los Angeles Air Force Base Honor Guard has two missions. The first mission is to provide military honors for deceased active duty, retired, and veteran members of the United States Air Force, Army Air Corps, Army Air Force, and Women’s Air Force. The second mission is to support base and community events through the execution of Color Guard, Saber Arch and military cordon ceremonies.

LAAFB BASE HISTORYIn 1961, the Air Force developed a plan to consolidate its Space Systems Division and the Research and Development (R&D) Center of The Aerospace Corporation, which had been created in 1960 to support Air Force space programs. The plan involved acquisition of two pieces of real estate adjoining the R&D Center. An aircraft plant owned by the Navy at the northwest corner of Aviation and El Segundo Boulevards, was transferred to the Air Force in October 1962. Another site at the southwest corner of the same intersec-tion, was owned by a mining company. The Aerospace Corporation acquired that site in November 1962 and built its new headquarters there between February 1963 and April 1964. By April 10, 1964, the Air Force property at the intersection of Aviation and El Segundo Boulevards was designated as Los Angeles Air Force Station, which was redesignated as Los Angeles Air Force Base in September 1987.

In 2006, Area A of LAAFB was permanently closed after a deal with a local builder to exchange the land in Area A for the creation of the Schriever Space Complex on Area B and other new facilities. In February 2007, a new base exchange opened up on Area B.

Fort MacArthur

This area was the center of the early shipping activity in the harbor. In 1796, Father Lasuen built an adobe shelter here, where the present post engineers building now stands. During the Mexican-American War (1845-47), Commodore Robert Stockton used the area for storage of ordnance and supplies for his march on Los Angeles. Later the site of a customs warehouse until 1880, it was commissioned as a military reservation in 1888, in order to protect the port and to halt encroachments by railroads and local government, and developed for military purposes shortly before World War I. It is named in honor of Lt. Gen. Arthur MacArthur, father of Douglas MacArthur, who later commanded American forces in the Pacific during World War II.

In 1975 Fort MacArthur became a sub-post of Fort Ord, and the Army transferred ownership of the fort’s Upper and Lower Reservations to the City of Los Angeles two years later. The Upper Reservation is now a city park - San Pedro’s Angels Gate Park, home of the Korean Bell of Friendship. It is also frequently used by television and motion picture companies. The aforementioned artillery emplacements have been seen in the television series ‘24’ and in movies as varied as ‘Dragnet’, ‘Midway’ and ‘Tora, Tora, Tora’. The Lower Reservation was cleared off and dredged and is now the city’s Cabrillo Marina.

Fort MacArthur’s remaining Middle Reservation was transferred to the United States Air Force in 1982 for use by the Los Ange-les Air Force Base as an administration and housing facility.

The Space and Missile Systems Center (SMC) is a part of Air Force Space Command of the United States Air Force. According to the SMC, its mission is to conduct research and development of U.S. military space and missile systems. The command is also responsible for acquisition, on-orbit testing, and sustainment of several existing programs. It also provides functional staff support for silo based ICBM programs. In addi-tion, SMC supports Air Force Space Command at Peterson AFB, Colorado, with launch operations, on-orbit satellite tracking, data acquisition, and command and control for Department of Defense satellites. The center has an annual total budget in excess of $10 billion per year and manages between $50 and $60 billion in contracts at any one time.

61st Air Base Wine

The 61st Air Base Wing is the host unit at Los Angeles Air Force Base. It provides administrative and base ser-vices to the personnel assigned to the Space and Missile Systems Center (SMC). The wing was activated in 2006. Its direct predecessor is the 61st Air Base Group, which was activated at LAAFB in 1994. The 61st can trace its linage back to World War II.

Raytheon Space and Airborne Systems is a leading provider of sen-sor systems giving military forces the most accurate and timely actionable information available for the network-centric battlefield. With 2008 revenues of $4.4 billion and 12,000 employees, SAS is headquartered in El Segundo, Calif. Additional facilities are in Goleta, Calif.; Forest, Miss.; Dallas, McKinney and Plano, Texas; and several international locations. The systems we design and build allow our cus-tomers to see, know and understand everything around them. A leader in responsive space solutions, SAS developed within 15 months for the Air Force a hyperspectral imaging sensor known as ARTEMIS to rapidly supply target detection and identification data from space.

BOMB THREAT

When A Bomb Threat Is Called In, Perform the Following Actions:

• Remain calm. • Attempt to keep the caller on the line as long as possible. Ask him/her to repeat the message. Record every word spoken

by the person and use the telephone bomb threat checklist. • Ask for the exact location where bomb has been or is going to be planted. • Get as much information as possible about the caller, e.g., vocal characteristic, race, sex, group affiliation, why the bomb

was placed. • Clues from background noises, which might indicate caller's identification and location. • Immediately after the caller hangs up, report the threat to 911 and your supervisor. • Remain available, as security personnel will want to interview you. • Do not spread rumors.

JOINT ARMY NAVY AIR FORCE PUBLICATION UNIFORM TELEPHONE COMMUNICATIONS PRECEDENCE SYSTEM (JANAP 137)

A. There are four levels of the Joint Uniform Telephone Communications Procedures System (JUTCPS) in the telephone procedure system, established for telephone calls placed within the Defense Communication System. At this base their use is confined to the Defense Switched Network (DSN). For further reference, consult AFI 33-111. 1. FLASH: Has precedence over any other telephone call of lower precedence. May be preempted by the application of the FLASH OVERRIDE capability available to the National Command Authorities (NCA). Reserved for alerts, warnings or other emergency actions having immediate bearing on nation, command or area security. 2. IMMEDIATE: Has precedence over any other telephone call of lower precedence. Preempts lower precedence calls. Precedence is reserved for vital communications: (a) Which have an immediate operational effect on tactical operations. (b) Which directly concern safety or rescue operations. (c) Which affect the intelligence community operational role. 3.PRIORITY: Is reserved for calls that require prompt completion for national defense and security, the successful conduct of war, or to safeguard life or property, which do not require a higher precedence. Normally PRIORITY is the highest precedence assigned to administra- tive matters, for which speed of handling is of utmost importance (i.e. reports of priority land, sea, or air movement; administrative intelligence; operational or logistic activity calls requiring priority action; calls that would have a serious impact on military and administrative intelligence, operational or logistic activities if handled as a routine precedence). 4.ROUTINE: Has no precedence over any other call and is handled as placed by the calling party. Precedence is reserved for all other official communications.REQUEST FOR TELEPHONE SERVICE:The following procedures will apply when requesting new telephone installations, moves or new jack installations. Requests for telephone installations, moves or new jack installs should be sub-mitted to your TCOs, who will call them into the Communications Squadron Help Desk (4545). The Unit Telephone Control Officer (TCO) of the requesting agency is the only individual authorized to request these services. Any Unauthorized requests (not requested by the TCO) will be rejected and returned to the Unit TCO for their review and approval/disapproval.REPORTING OPERATOR SERVICE:Telephone operators are instructed to be courteous and alert at all times. However, in case of unsatisfactory service, ask for the operator’s number (only), as the operators are not permitted to disclose their names or other related information. 1. Report unsatisfactory service to the 90 CS/QAP at 2040. 2. Telephone users should not admonish operators or attempt to engage them in idle conversation. 3. Obscene or harassing language over the telephone will not be tolerated. Federal and state statutes deal with offenders of this type. Title -47 U.S.C. 223; May 1968 covers offenses of this nature. Military and DAF civilian employees engaging in this action are also subject to disciplinary action under the Uniform Code of Military Justice.BOMB THREAT:Bomb threat procedures are as follows: Try to keep the caller on the line and have another individual contact the security police at ext. 911. Do not hang up the telephone on which the bomb threat is received; try to keep the caller on line. Try to watch the time the call is received to help in the tracing action. If unable to notify security forces while the call was in progress, notify immediately after caller hangs up. Have someone standby outside the building to assist in search of the building upon arrival of Law Enforcement Personnel. Any person who has information or knows the identity of person(s) placing a bomb threat or threatening telephone calls can notify security forces.MINIMIZE: A. MINIMIZE is a condition imposed by commanders to control and reduce electrical/ voice communications during exercises or period of emergency. During these periods all except vital traffic must be drastically reduced. MINIMIZE may be imposed on all tele- phone, Teletype, commercial service and data systems. A telephone MINIMIZE will affect such services as DSN, long distance, telephone data, and administrative calls. All systems of services may be MINIMIZED as required by the imposing authority. B. Normally, the Base Communications Center will receive the initial notification and will advise all base agencies through their administrative sections. C. When MINIMIZE is in effect, subscribers may be denied direct dial access to the DSN, and commercial systems. Only calls approved by designated officials will be made. Telephone operators will inform all subscribers of the MINIMIZE prior to processing any outgoing calls through the switchboard.TELEPHONE CLASSIFICATIONS:Telephones installed on LAAFB are classified in accordance with AFI 33-111. A. CLASS “A”, telephones are authorized for the transaction of OFFICIAL GOVERNMENT business and are permitted access to all the city trunks by dialing “99” and the seven digit number. Personal long distance calls will not be placed from Class “A” telephones. Class “A” service has access to DSN by dialing “94” followed by the seven digit number. B. Class “C”, telephones are restricted to official inter-base business, and have no access to city or DSN trunks however, incoming city, pre-paid long distance and DSN calls may be accepted. Only the first number of a rotary type number should be listed and if you have a Class “A” and Class “C” telephone, only the class “C” should be listed in the telephone directory.DAMAGE TO COMMUNICATIONS EQUIPMENT:If equipment is tampered with, it will be removed from service immediately. Whoever willfully or maliciously injures or destroys any of the works or property of any radio, telephone, telegraph, cable, station, system or other means of communications operated or controlled by the USAF, whether constructed or in the process of construction or willfully or maliciously interferes in any way with the working or use of such line or system shall be fined not more than $250,000 or imprisoned for not more than ten years or both. (Title 18, U.S.C. 1362,Chapter 65).

PERSONAL TELEPHONE CALLS:Personal calls from government telephones are not permitted except in cases of emergency. Pay station telephones are conveniently located throughout the base and will be used for all personal calls. Persons having a telephone on their desk are responsible for its proper use. Supervisors are expected to cooperate in insuring strict compliance with this instruction.HOW TO USE SPECIAL TELEPHONE FEATURES*Note- Phones may not have these features. Single line phones refer to phones with only 1 or 2 line capability Digital phones refer to phones that can have more than 2 lines and have windows next to keys. A. Last Number Redial: (Will redial last successfully completed call) 1. Lift handset from cradle. 2. Press # # 3. Last number dialed will ring. B. Call Transfer: 1. Advise the calling party that you are going to transfer the call. 2. Press the link button on single line phones or transfer key on digital phones. (You will hear a second dial tone). 3. Dial the new extension. 4. Wait for ring, press transfer again. 5. Replace the handset in cradle. C. Three-Way Calling: (with the original caller still on the line) 1. Press the link key (single line) or the transfer key (digital) (You will hear a second dial tone) 2. Dial the third party’s extension. 3. When the Third party answers, press the transfer button again. 4. All three parties are connected and can talk. D. Call Forward: 1. To Activate: (a) On Single line and digital phones without Call Forward buttons: 1) Lift the handset from the cradle. 2) Dial the call forward activation code: *72 (You will hear stuttered dial tone and then a steady dial tone). 3) Dial the 4-digit number you want your calls to forward to. (You will hear 2 short beeps). 4) Replace the handset in the cradle. 5) To verify the number to which calls are being forwarded, dial your own number from your own telephone. Your call should be forwarded to that number. (b) On digital phones with Call Forward buttons: 1) With receiver in the cradle (hung up) press the Call Forward (Call Fwd) button. 2) An arrow will began to flash in window next to button. 3) Dial the 4-digit number to which calls are to be forwarded. 4) Press the Call forward (Call Fwd) button and the arrow will stay on steady. 5) To verify the number to which calls are being forwarded, dial your own number from your own telephone. Your call should be forwarded to that number. 2. To Deactivate: (a) On Single line and digital phones without Call Forward buttons: 1) Lift the handset from the cradle. 2) Dial the call forward deactivation code: *73 (You will hear 2 short beeps). 3) Replace the handset in the cradle. (b) On digital phones with Call Forward buttons: 1) With receiver in the cradle (hung up) press the Call Forward (Call Fwd) button. 2) The arrow should disappear. (c) Call forward is deactivated; this can be verified by having someone call you if you’re not sure. E. Ring Again: (works only on digital phones when calling base numbers) 1. To Activate: a) Lift the handset from the cradle. b) Dial extension number, c) If line is busy press the RAG or Ring Again button d) Replace the handset in the cradle. *Note: You may continue to place or receive other calls while waiting for the ring again recall e) Wait for ringing. f) Lift the handset from the cradle, and press the RAG button. F. Voice Mail: 1. To check to see if you have a voice mail message, one of the following will occur: a) On a single line phone you will receive a stuttered dial tone when the receiver is lifted or b) On a digital phone you will have a MWT button, which will show an arrow when you have a message. 2. To retrieve your messages do one of the following: a) Dial extension number 5980 on single line phones or b) Press the MWT key on digital phone that will take you directly to voice mail. *Note- all of the following instructions apply to both types of phones (single line & digital). c) When voice mail picks up follow instructions, enter last 4 of your phone number & # then enter your password and # to access your messages. d) Follow prompts to listen to messages e) After finished replace handset in cradle.IMPORTANT THINGS TO REMEMBER A) You will not receive a second dial tone after dialing “99” for off-base numbers or “94” for DSN numbers. B) You must dial all seven (7) digits. If the lines are busy, you will receive a busy tone. C) Any other questions on telephone features refer to your unit Telephone Control Officer (TCO).

Air Force World-Wide Locator The Air Force World-Wide Locator is based at the Air Force Personnel Head-quarters in Texas. It handles requests for Air Force active duty, Air Force re-serves, the Air National Guard, and retired Air Force members. There are two types of requests: official requests and unofficial requests. Official requests are defined as requests received from any government agency and the Depart-ment of Defense. All other requests are considered unofficial. All unofficial requests must be made in writing.

In order for the Air Force to properly locate the correct individual, your request must contain as much of the following information as possible: • Full name to include a middle initial, • Rank • Social Security number • Date of birth • Any known assignment information (places/dates)

A fee of $3.50, per individual request, is required for all unofficial requests. The fee must be paid by check or money order made out to “DAO-DE RAFB.” Requestors who are on active duty, National Guard, Reserves, or military re-tired are exempt from paying the fee. Your written request needs to include your name, address, and phone number. Put your written request in an un-sealed envelope with a return address, proper postage affixed and the indi-vidual’s (the person you’re looking for) name in the addressee portion of the envelope. Place this envelope in a larger envelope with your check or money and mail to the locator address at:
