Introduction for the Physician

Why you, as a physician, needthis guideThe job of lowering blood cho-lesterol takes work, and tocomply with physician advice,patients will need help with theday to day aspects of changingeating habits or taking drugscorrectly. In addition, physiciansmay want to expand their knowl-edge of cholesterol manage-ment issues. To cover these twoareas, this guide is divided intotwo sections. The first is for physi-cians. The second section is forpatients.

The physician sectionIn the physician section, differentresources are listed. Theseinclude CME opportunities onthe medical and scientificaspects of cholesterol and heartdisease and on the physician'srole in diagnosis and patientmanagement. Additional mater-ials for professional audienceson the dietary aspects of bloodcholesterol lowering are alsogiven. Information to give topatients on cholesterol loweringdrugs is listed too.

Publications and CME mater-ials are listed by title. Those witha (S) are for sale, and prices arenoted where known. Itemswithout the (S) symbol are free.

The patient sectionThe patient section guidesreaders to information sourcesthat offer practical tips toachieve the goals set by thephysician. Since dietarychanges are the first line oftreatment, information on threemajor topics are covered. Theseare: (1) fat and cholesterolcontent of foods, (2) guidelinesfor cooking and choosing foods,and (3) how to locate a dietitianto give personalized nutritioncounseling for blood cholesterol

lowering efforts. This section maybe photocopied and given topatients for their own use.

Locating nutrition counselingresourcesFor patients who do not respondwell to initial attempts at dietaryapproaches and who need in-dividualized nutrition counseling,physicians may want to take thelead in making referrals to quali-fied dietitians. The AmericanDietetic Association (ADA) main-tains a roster of dietitians andthe national office will respondto written requests for assistancein locating registered dietitiansin a given state or area. Regis-tered dietitians are qualified todo nutrition counseling for bloodcholesterol lowering. The ADAalso has a network of stateaffiliate associations. Officers instate ADA affiliates could behelpful in identifying localdietitians who will acceptreferrals from physicians fornutrition counseling. To locatethe state ADA affiliate, physiciansshould also write to the nationalADA office. Once informationhas been gathered from thenational or state ADA compo-nents, physicians can selectivelytransfer local information to theworksheet provided with thephysician section of this resourceguide. This tailored list of dieti-tians can then be photocopiedand given to patients whorequire nutrition counseling tocomplement the physician's ownadvice for blood cholesterollowering.

Some local divisions of theAmerican Heart Association mayalso maintain lists of dietitianswho will do counseling for hearthealthy nutrition, and willgenerally give this information todoctors and patients alike. Refer-rals for nutrition counseling may

also be available through hospi-tals, state or local health depart-ments, or nutrition departmentsof universities. These latter threesources may be researched bypatiants themselves and a pa-tient worksheet is provided.

Since the mainstay of loweringblood cholesterol is a change ineating habits, this list will helpyou locate information about thepractical aspects of (1) choosingand cooking foods in new ways,(2) fat and cholesterol content offoods, and (3) finding a dietitianwho can give personalized nutri-tion counseling to help you inyour cholesterol lowering efforts.

How the list is organizedThe list is divided into sectionsaccording to whether infor-mation is available fromgovernment agencies, healthassociations, or local communitysources. Each leads you toinformation in different ways.

In all of the sections, somebooks and pamphlets are listedby titles which you can request.Those with a ($) symbol are forsale, and prices are given whenthey are known. Others, the oneswithout the ($) symbol, are free ifyou ask for one copy of each.Publications by the federal gov-ernment may be obtained assingle copies from the agencylisted or may be purchaseddirectly from the U.S. Govern-ment Printing Office by followingthe instructions at the end of thisintroduction.

How you can use this fistMuch valuable information isoffered through the associationswhich are listed in the secondsection. The address of eachnational office is given first,followed by instructions on howto find state or local offices, ifthey exist. Although mostpamphlets and books are

produced by the national office,they are usually availablethrough local offices as well.Local offices may have addi-tional materials of their own.Services such as classes ornutrition counseling areavailable only through the localchapters, so it is a good idea tocontact a local office first.

The third section gives ideasfor finding information in yourown community: Often you willhave to consult the telephonedirectory and make a fewphone calls. Many state andlocal health departments haveexcellent pamphlets on fats andHeart disease. Some may offerclasses or nutrition counseling aswell. To record the informationyou have tracked down, work-sheets are provided.

A fourth section gives nutritioncontacts for state health depart-ments. Although the list is currentas of fall of 1986, it is subject tochange. However, by contactingthe office listed for your state youcan be referred to any person oroffice that may handle heartdisease nutrition information inthe future.

How to find nutritioncounseling servicesIf your doctor has recom-mended that you consult adietitian to help you with thedietary changes needed tolower your blood cholesterol, heor she may give you a list ofpersons to contact. However,your doctor may also advise youto seek this type of consultationon your own. If so, ideas forfinding qualified registereddietitians are given in thisresource guide.

In addition, ExecutiveOrder 11141 prohibits discrimination on thebasis of age by contractors and subcon-tractors in the performance of Federalcontracts, and Executive Order 11246 statesthat no federally funded contractor maydiscriminate against any employee orapplicant for employment because of race,color, religion, sex, or national origin.Therefore, the National Heart, Lung, andBlood Institute must be operated incompliance with these laws and ExecutiveOrders.
