Requirements


In the absence of a PSA Birth Certificate, the

parent or guardian must submit a Birth Certificate

(late registration) from the local civil registrar or a

barangay certification containing the basic

information of the child such as:

• Name of the child

(first name, middle name, last name)

• Name of Parents

• Date of Birth

• Sex


If the documents mentioned above are not

submitted by the end of Early Registration, learners will

be given until August 21st of the school year they will

enroll in to produce and submit the required

documents.

Submission of the learner’s Birth Certificate from

the PSA or the local civil registrar, or barangay

certification for purposes of enrollment will only be done

once during the duration of a child’s basic education.

As provided in DO. 58, s. 2017 the birth certificate shall

be an attachment to the learner’s permanent record.

UPDATES: On Kindergarten nomenclature : Use

Kindergarten to the level before Grade 1 (5 years old). No Kinder 1 and Kinder 2. You may use other category for children below five years old, e.g. Casa 1, Casa 2, Toddler, Nursery, etc.


FYI: The RO has requested the BEA if the

endorsement for PVT can be done by the SDO, request was not granted. Hence endorsement for PVT is being done at the Regional Office.


UPDATES: On issuance of Special Orders (SO) Special Orders is no longer issued to

Junior High School completers SO are issued to Senior High School

graduates SDO did the evaluation RO is doing the validation and

issuance of SO


UPDATES: On issuance of Special Orders (SO) :

DepEd Order No. 10, s. 2018 Issuance of SO is being done at the RO

to more than 167,000 SHS graduates. It was last February 2018 that the

issuance about SO from CO was released.

There will be a regional memo to be issued to effect processing of SO beginning October 2018.

Guidelines on Awards and Recognition for the K to 12 Basic Education Program Paragraph No. 7 – Any modification by the Private Schools should be subject to the approval of the DepEd Regional Office.

was last February 2018, was extended until April 2018.

Application for permit as SHS provider (DepEd Memo No. 4, s.2014) – but the submission of application is to the RO, processing and issuance is done at the RO.

Additional track/course offering should be issued with permit.


UPDATES: No issuance yet for the Central Office

about granting of Government Recognition for SHS providers.

The granted Provisionary Permit is still enforced, no yearly renewal.
