The interviewer to contact the participant before the interview to check that they are still happy to take part
The following information could be completed before the interview. •PCT area –the name of your PCT (e.g. East and Coastal Kent)•NHS user ID (e.g. NHS ID) •Date of interview•Participant’s gender •Participant’s date of birth•Name of interviewer

Participant safetyIf the participant shows signs of distress
The interviewer should draw the interview to a close sensitively
If the participant reveals new health conditionsThe interviewer should ask whether they would like their GP, PCT or other health professional to be notified
If the participant reveals difficulties that they feel have not been responded to appropriately
The interviewer should ask whether they would like their GP, PCT or other health professional to be notified
If the interviewer feels that the participant is uncomfortable with a question they should not force them to answer

Interviewer safety• If interviewing in a participant’s home
– Tell someone who you are interviewing and the address– Tell the person when the interview should be finishing
• All interviewers should be aware of the following– Is there any history of violence?– Does participant hold any prejudices (e.g. racism)? – Any recent events likely to cause distress (e.g.

Data Protection
• Names and addresses are kept separately from the personal information collected on the questionnaire
• Only the participant’s NHS ID and the unique ID should be used on the questionnaire
