Level 3 - External responders required, reportable/recordable with fine, release requiring remediation, hospitalization, losses of >$5K, suspension of activities beyond next business day, recurring level 2 findings. Ex. Biohazard bags found in the landfill

Notification Lists Level 2 Notification List: -Unit Contact -Appropriate PAs -PA Supervisor -AVP EH&S -Unit Environmental Representative

Notification for Level 2 findings may occur via a database that captures findings

Level 3 Notification List: All persons on Level 2 Notification List plus: -Unit Environmental Official -VP RM&PS -VP Research Administration -University Counsel -Risk Management

Notes: 1. Appropriate parties may include customer service, police, etc. Entering an incident in the Incident Database automatically makes a record in FiT, which emails EHS automatically within 1 hour.

2. For Incidents involving:

Petroleum - 2 hours Chemicals -

- immediately for any non-597 bulk liquids released, -2 hours, -24 hours for >RQ in secondary containment

3. Internal parties may include Cornell’s Public Relations Office. Decisions about contacting such groups should be made by the EH&S AVP.

6. It is an expectation that a post mortem be completed for all Level 3 findings. Items from the post mortem should be included as actions in FiT.

Finalize info in FiT and Incident Database. Close record when

corrective and preventative actions are complete. Enter date

post mortem complete, as appropriate6 .

Communicate resolution for Level 2 & 3 findings to SHE
