was a whole day event where team Shifa won the match. Mr. Ammar Bilal from Shifa International Hospital also joined the event. The full day event ended with a prize distribution ceremony hosted by Pediatric Surgeon MIH Dr. Farhan Ansari, where cups were distributed amongst the top performers by Chairman MIH, Ch. Naseer Ahmed, CEO MIH Ch. Haroon Naseer, Consultant Surgeon Dr. Faisal Murad and Consultant Gynecologist Dr. Ghazala Bashir also distributed shields and certificates amongst the participants and organizers.

Attendees found the training very helpful in terms of understanding the significance of patient handling, active response and positive attitude.

The training was closed with certificate distribution ceremony among the attendees.

The key focus areas of the training session were value of a customer, customer care model, positive attitudes, active body language, responsiveness and empathy. What is intra-cytoplasmic sperm insemination (ICSI) and how does it work?Intra-Cytoplasmic Sperm Insemination (ICSI) differs from conventional In-Vitro Fertilization (IVF) in that the embryologist selects a single sperm to be inseminated directly into an egg, instead of fertilization taking place in a dish where many sperm are placed near an egg. Is ICSI for me?ICSI enables fertilization to happen when there are very few sperms available. Your clinic may recommend ICSI if:• You have a very low sperm count • Other problems with the sperm have been identified, such as poor morphology (abnormal shape) or poor motility (not moving normally)

• During previous attempts at IVF there was failure of fertilization or an unexpectedly low fertilization rate • You need sperms to be collected surgically from the testicles or epididymis (a narrow tube inside the scrotum, where sperm are stored and matured); for example because you have had a vasectomy, you do not ejaculate sperm, or because you have extremely low sperm production • You are using frozen sperm in your treatment which is not of optimum quality • You are using embryo testing.

How does ICSI work?Before your treatment starts, you will need to complete various consent forms and you, and if applicable your partner, may need to have blood tests to screen for HIV, Hepatitis B and C and Human T Cell

Lymphotropic Virus (HTLV) I and II. Treatment then typically involves the following:For womenYou take fertility drugs to stimulate your ovaries to produce more eggs, as for IVF, and your progress will be monitored through vaginal ultrasound scans and possibly blood tests. The eggs are then collected using the same procedure as IVF and each egg is inseminated with a single sperm from your partner or donor. The rest of the process is also the same as IVF. You are more likely to become pregnant with twins or triplets if more than one embryo is transferred so your clinic will recommend Single Embryo Transfer (SET) if they feel it is the best option for you. An embryologist will examine your sperm under a microscope and

decide whether ICSI could increase your chances of fathering a baby. The next step depends on whether you are able to provide sperm without a medical procedure:• If you can, you produce a fresh sperm sample on the same day as your partner’s eggs are collected. Or: • Sperm can be collected directly from the epididymis using a type of fine syringe. This is known as ‘Percutaneous Epididymal Sperm Aspiration’ or PESA. • Sperm can also be retrieved from the testicles, a process known as ‘Testicular Sperm Aspiration’ or TESA. • It is also possible to remove tiny quantities of testicular tissue from which sperm can be extracted. This procedure is called ‘Testicular Sperm Extraction’ or TESE. For more information about PESA, TESA and TESE, speak to your doctor. • If you have stored sperm, it will be removed from frozen storage, thawed and prepared for treatment. A single sperm is then injected into each egg. ICSI provides the opportunity for fertilization to happen, but it is not guaranteed to succeed.Finally, if fertilization does take place, the embryos will be cultured in the laboratory for up to six days and then between one and three of the best-quality embryos will be transferred to the womb.

Pneumonia: Epidemiology and Prevention

According to World Health Organization (WHO), globally pneumonia is the leading cause of death in children, killing approximately 1.2 million children under 5 years of age annually, which is greater than malaria, tuberculosis and AIDS combined. In developing nations, such as Pakistan, 1 in every 5 children dies of pneumonia, placing Pakistan among the top 5 countries of the Asian continent w.r.t pneumonia mortality (Laufer, 2013). The infection is caused by a large variety of microorganisms, ranging from viruses to bacteria and fungi. The etiology of pneumonia depends on the age of the patient. In neonates, maternal flora, like

B streptococcus, are commonly transmitted vertically. Overall, Streptococcus pneumoniae is the most common bacterial agent that causes pneumoniae, whereas 14% to 35% cases are viral (Lynch III and Zhanel, 2010, Pereda et al., 2015).

Epidemiology and Mortality in Pakistan As has been discussed earlier, no exact epidemiology of the disease can be established due to lack of appropriate relevant data. However, from the sources available to us a few conclusions have been drawn. The infant mortality rate in Pakistan is 59.3 death/1000 live births. Due to such a high mortality ratio, Pakistan is placed on position

the pathogens that were isolated. The most commonly identified pathogen was Haemophilius influenza. This yield has been reported in most of the pediatric populations (41/64) (Butt et al., 2005). Another study was conducted as a part of the international survey that consisted of 9 countries. 200 samples from Pakistan were included, and Haemophilus influenza and Streptococcus pneumonia were equally detected (Zafar et al., 2008). Observational retrospective study, which was conducted in a tertiary care hospital include radiological as well as clinical presentation along with lab investigations in order to evaluate the mortality in patients suffering from CAP, 11% mortality rate was observed. However, complications such as septic shock, multipole organ failure, ARD,

study was that it was conducted in rural Punjab where most people were of low socio-economic class and illiterate. Interestingly in this study the age group most effected was that of 20-40 years. 124 patients were enrolled in another study, that identified etiologic agents in 41.93% cases, of which, 69% were found to be Chlymadia and Myoplasma pneumoniae. However, such a trivial number of microorganism identified in routine study do not show the true picture of pneumonia etiology in Pakistan (Ansarie and Kasmani, 2014).Retrospective study conducted from 2002 till 2008 to identified etiology of severe community acquired

ventilator were linked to high rate of mortality. In case of microbiological specimens, low yield was observed, due to the unavailability of serologic test for atypical pathogens (Irfan et al., 2009).Another original retrospective study conducted at a tertiary care hospital, specifically for the S. pneumonia positive samples, two years medical data was reviews. Sample size was 16785, of which only 0.7% showed S. pneumoniae as the causative agent. However a wide variety of limitation were

linked with this study. The study was conducted at a single center that has a very low isolation rate. Age variation of the patients tested positive was quite highranging from 2 months – 75 years. Due to these limitation, conclusions for entire population cannot be derived (Choudhry et al., 2011) Another group of researchers conducted a questionnaire based pre-designed study to assess their demographic profiles at tertiary care center in Bahawalpur on 150 hospitalized patients of CAP. The striking feature of this

pneumonia (SCAP) in adults in accordance with ATS guidelines 2001. Out of 832, 9 were CAP effected while 189 suffered from SCAP of which 179 needed ICU and only 10 could be managed in ward. The pathogens identified were 15.85 S. aureus, 14.7% S. pneumoniae and 9.5% P. aeruginosa. Pakistan Chest Society guidelines has a document that was initially published by the pneumonia guideline community. Taking into consideration the etiologies that are most commonly identified; PCS classified treatment based on British Thoracic Society and American Thoracic Society recommendations (Ansarie and Kasmani, 2014).Pneumonia cases are recorded

in the Demographic and Health Surveys (DHS) and Multiple Indicator Cluster Surveys of the United Nations Children’s Fund (MICS); but due to the nonspecific classification of the pneumonia cases countrywide the numbers generated are not 100% accurate. The estimates for the annual incidence of the Acute Respiratory Infections (ARI) in Pakistan for children under the age of 5 is just for a group that is representative of just 22% of the population. Most of the studies on the Asian continent with reference to pneumonia, its pathogens, prevalence and epidemiology report lack of data from Pakistan (Hazir, 2008).

Be Aware of Your General HealthSince pneumonia often follows respiratory infections, be aware of any symptoms that linger more than a few days. Good health habits prevent sickness from viruses and respiratory illnesses. They also help promote fast recovery when you do get a cold, the flu or other respiratory illness.Stay Healthy, Stay Happy!

Prevention:Pneumonia can be prevented by adopting some of the very simple habits which include:

Get VaccinatedGet flu shots annually to prevent influenza, which is a common risk factor for pneumonia development. Pneumococcal conjugate vaccine should be administered specially in children younger than 5 and adults elder than 65 years of age.
