How to Complete an Annual Activity Profile Lawrence S. Bloomberg Faculty of Nursing

This guide is intended to assist Adjunct Appointees to the Lawrence S. Bloomberg Faculty of Nursing in completing their

annual activity profile on the LinkingHealthProfessionals (LHP) system.

Supported browsers for the LHP system are Internet Explorer 7+ and Firefox 3+.

Step-by-step instructions are provided starting on the next page. If you have any questions, you can contact us at

[email protected]. We are in the office between the hours of 9 am to 5 pm Monday to Friday,

excluding holidays.

Step 1: You will receive an email from the linkinghealthprofessionals system, as per the example below. Click on the link

provided and if you are successful, go to step 7. Due to security reasons, please note this link is time sensitive and will

not function after a few days. If this happens to you, follow the instructions on the next page.

Step 2: Using IE or Firefox enter this address: https://www.linkinghealthprofessionals.com/index.cfm?

Step 3: Sign in by entering your credentials here and click on “Sign In”.

Step 4: Note the options on the left menu will vary depending on your profile. “Applications” refers to system modules

Click on “Applications” to open up a drop down list of possible system modules.

Step 5: Click on “Appointments”.

Step 6: Select “Annual Activity Profile”.

Step 7: Click on “Start”.

Step 8: This screen will appear and you can respond to each question.

Step 9: This screen below represents the end of the list of questions. You can save your responses and come back at a

later time to complete the form. If you are ready to submit your form, go to Step 13 for further instructions.

Step 10: If you have saved your annual activity profile form and you are ready to complete it, you can go back to the

original email that was sent to you and click on the link provided. If the link is no longer active follow the steps starting

at Step 2.

Step 11: This screen will come up. Click on “Continue”.

Step 12: Your form will show up with the entries you had saved.

Step 13: When you have responded to the questions and you are ready to send it, click “Submit”.

Step 14: You will see the green message indicating that the form was successfully submitted to us.

Step 15: You are done and you can sign out.
