All questions should be answered for the 2009 calendar year Information about individual schools will not be released, nor will government and non government schools be compared. The information you provide will be used to describe patterns in each study area. High School Questionnaire for High School Principals

We would like each School Principal to answer some questions about your High School with a

particular focus on healthy eating and physical activity. All questions should be answered for the

2009 calendar year

Information about individual schools will not be released, nor will government and non government

schools be compared. The information you provide will be used to describe patterns in each study

area.

The questions and process for this study have been approved by the Department of Health and the

Department of Education and Children’s Services (DECS) Ethics committees. You are free not to

complete this questionnaire. If you decide not to, services received from the eat well be active

Community Programs will not be affected in any way.

This questionnaire should take about 10-15 minutes to complete.

You are free not to answer any specific question or end your involvement in this questionnaire at

any time.
