Marty Ryan, the former owner ofMarty’s Sneaker World in Hones-

dale, is grateful he has both his legs andthat he’s still in the shoe business after35 years.

Wayne Memorial Hospital staffrecently surprised Ryan with a cake athis current shoe store in Waymart tocongratulate him on his anniversaryand his success at battling a debilitat-ing disease – lymphedema. Ryan, 67,was quick to thank them for muchmore than the cake.

“I wouldn’t have my leg here,” he said,pointing to his right leg, which waswrapped in compression bandages,“and maybe both of them if it hadn’tbeen for these folks. I owe the doctorsand nurses who worked so hard to healmy many wounds and these therapistsa lot!”

The praise was reciprocated by AmyKrempasky, RN and clinical coordina-tor of the hospital’s Advanced WoundCare & Hyperbaric Center and Occupa-tional Therapists Christine Carlozziand Sean Phillips.

“Marty would not be where he istoday if he hadn’t shown up for hisappointments and done the work,”said Krempasky. “He was vigilant anddiligent and that’s why he still has hisleg.”

Ryan, a US. Army veteran who servedin Korea during the Vietnam War peri-od, admitted he was not an easypatient when he was first diagnosed in2007.

“When the Wound Care Clinic staffwanted me to start therapy, I was defi-nitely not interested,” he said, “I didn’tlike change and I fought it all the way.But finally the nurses convinced me Ihad no other choice if I wanted my legsto get better. I had to give in, and now,I’m glad I did!

Ryan undergoes treatment in theWound Care Center with Edward(“Ted”) Murray, DPM, the Wound Careteam and Carlozzi and Phillips. Thetwo therapists underwent trainingsupported by Wayne Memorial Hospi-tal to become certified in CompeteDecongestive Therapy (CDT) for lym-phedema. A chronic condition inwhich excess protein-rich fluid collectsin the tissues, lymphedema causesextreme swelling and possible skinchanges in the body that don’t go awayand may even get worse over time.

Carlozzi and Phillips use the VodderTechnique which moves the lymphfluid through and out of the bodythrough very gentle skin stretchesalong the normal pathways of the lym-phatic system. This technique is com-bined with skin care, compressionbandaging and specific exercises. .”

“I’m really happy to share my ownstory if it can help even one other per-son out there who has lymphedema toseek help from these folks,” said Ryan.Wayne Memorial Rehabilitation Serv-ices, with two locations in Honesdale,offers physical, occupational andspeech therapy, as well as specialtytherapies for patients with lymphede-ma, Parkinson’s Disease and cancer.

Shannon Bruning felt the bite,swatting away the tiny darkinsect from her lower right leg.

She’d been standing amongst the tallgrass of her Kauneonga Lake backyard, watching husband Butch test anew weedwhacker.

But within seconds of the bite, “Ihad a blister bigger than a silver dol-lar,” she recalled.

“And the itch was unbelievable.”Ice, peroxide, anti-itch cream –

nothing salved it, and Shannon could-n’t resist scratching.

That burst the blister, and suddenlyshe was confronted with a nasty openwound straight out of a horror movie.

“The black and yellow are poison,”Shannon pointed out during a visitlast week.

She was changing the dressing on apainful, two-inch-long gash in herleg – four months after the initial bite.

Doctors at Hudson Valley HospitalCenter’s Institute for Wound Care andHyperbaric Medicine in Peekskill tell

her it could take another eight monthsfor the wound to heal.

And she will likely never know exact-ly what bit her.

Shannon thinks it could be a brownrecluse spider, but their range doesn’textend this far north.

“They’re not common here,”affirmed Sullivan County PublicHealth Director Nancy McGraw. “Sowe really can’t say whether it was abrown recluse spider or not.”

Allergic reactions, immune systemproblems and cardiovascular issuescan all aggravate insect bites, sheadded, meaning a person’s particularreaction to a bite may be very differentfrom another’s – whether it was a spi-der or something else.

McGraw sympathized with thealarming nature of Shannon’s woundbut said brown recluse spider bites arenot a local problem.

“From a public health perspective,it’s not something the average personshould be concerned about,” sheassured.

Indeed, “if I were to caution anyoneto be more vigilant about bugs,” saidMcGraw, “it would be ticks.”

A bite from a tick may or may not benoticeable, but they can transmitLyme Disease, which, left untreated,can have long-term health effects.

Regardless, if you’re wearing cloth-ing that exposes arms, legs or otherparts of the body to vegetation, beready for insect bites in SullivanCounty – and to react swiftly.

Shannon initially visited her doctor,who gave her antibiotics to stave offfurther complications. The team atHudson Valley Hospital Center helpedher recover from an infection in thewound, but she’s still got a long roadto travel.

which helps her clean out the dead tis-sue three times a day. “I’m going to gobroke!”

And Butch still has to take her downto Peekskill weekly for further cleaningand inspection of the bite area.

Some of that’s covered by Medicareand private insurance, some not.

The itch, thankfully, is gone, but so isShannon’s independence.

“I haven’t driven, but I can walk.Sometimes it’s very painful,” Shannonrelated – made more painful by the factshe broke her toe after stubbing it onfurniture in her home.

The grandmother of eight misses vis-its with family, though Butch and theirdog Sam have been constant compan-ions.

Her pets, in fact, are what broughther from Westchester County to Sulli-van County 14 years ago.

“I wanted a place where I could havemy animals,” she said with a smile.

She didn’t count on the kind of crit-ters that might feast on – or be irritatedby – her presence, however. So she’sspreading the word to be careful.

“I want people to be aware of this,”she affirmed, “that there’s somethinghere that bites you and eats your fleshaway.”

Back in the Fall of 1984, Dr.Rajan Gulati was the well-respected Director of the

Emergency Department at the now-closed Horton Memorial Hospital inMiddletown.

After a decade in this position, hedecided to branch out on his ownand open “Immediate MedicalCare,” walk-in Urgent Care servicesand the first of its kind in the region,at a small location on Ridge Street inMiddletown, just down the roadfrom Horton, and with just threeemployees.

Fast forward 30 years later to 2014,and “Immediate Medical Care” isnow “Middletown Medical,” a multi-specialty physicians’ group with 15locations throughout Orange,Sullivan and Ulster counties, andconsidered to be one of the HudsonValley’s premier healthcare servicesprovider known for its compassion-ate and comprehensive services.

And those three employees have

now blossomed into 350, makingMiddletown Medical a leadingemployer in the area, as well.

“We simply wanted to makeurgent care services accessible topeople, as the local emergencyrooms were overwhelmed,” notesDr. Gulati, who continues to seepatients while also serving asPresident & CEO of MiddletownMedical. “We were in the right placeat the right time.”

Success leads to expansionAfter just a few years on Ridge

Street, “Immediate Medical Care”

was growing out of its space,prompting Dr. Gulati to build a5,000 square foot building at 2Edgewater Drive in Middletown.With this move came expandedservices – primary and family care,specialty medicines such as cardiol-ogy, laboratory services and more –and the name “MiddletownMedical” was born.

Then in 1993, it was time toexpand again – to a new and consid-erably larger 30,000 square footbuilding at 111 Maltese Drive (justnext door to 2 Edgewater). Thisbuilding underwent significantexpansion in 1999, and is still themain “hub” of MiddletownMedical’s operation.

HARRIS — In recognition of itsexpertise in treating stroke patients,Catskill Regional Medical Center’sHarris Campus has been named adesignated Stroke Center by the NewYork State Department of Health.According to New York State proto-col, emergency responders shouldalways transport stroke patients tothe nearest designated stroke centerwhen they present symptoms andsigns of a stroke.

As a designated Stroke Center,Catskill Regional has an expertStroke Team comprised of highlytrained physicians, nurses and tech-nicians that can treat stroke quicklyand effectively, using the latest med-icines and techniques, to minimizebrain damage. Having quick accessto these types of services within thefirst three hours of showing symp-toms of a stroke is essential to pre-venting long-term neurological

damage.“This designation signifies our

ability to rapidly assess our patientsand get them the treatment theyneed, states Chief Medical Officerand Chief Executive Officer, GerardGalarneau, MD. “Now, people whoshow signs of a stroke can be caredfor right in their own community,receiving quality stroke care andminimizing the affects that a strokecan cause. ”

A stroke, or brain attack, occurswhen a blood clot blocks a blood ves-sel or artery, or when a blood vesselbreaks, interrupting blood flow to anarea of the brain. When a brainattack occurs, it kills brain cells in theimmediate area. Doctors call thisarea of dead cells an infarct. Thesecells usually die within minutes to afew hours after the stroke starts.

“We want to remind area residentsto call 911 immediately if they begin

to experience symptoms of a stroke— sudden numbness or weakness ofthe face, arm or leg, especially onone side of the body; sudden confu-sion; trouble speaking or under-standing; sudden trouble seeing inone or both eyes; sudden troublewalking, dizziness, loss of balance orcoordination; and/or sudden, severeheadache with no known cause,”said Medical Director of the StrokeCenter Dr. Carlos Holden. “In thecase of a stroke, every minutecounts.”

Additionally, Catskill Regional’sHarris and Callicoon Campuses haveconstant contact with qualified spe-cialists, regardless of the time of dayor weather, through the use of“Doctor Polly” or a “Doc on Wheels.”Doctor Polly is state-of-the-art tech-nology used to help diagnose strokeand other illnesses by remote con-sultation. ed with a New York State HEAL 125grant and was specifically allocatedfor enhancing services to CatskillRegional’s Emergency Department.Doctor Polly is manufactured byPolycom and is a mobile cart usedfor bedside consultations betweenpatients and off-site specialists thatcan also be used to facilitate rapiddiagnosis of stroke.

ELLENVILLE — EllenvilleRegional Hospital has been award-ed a three-year term of accredita-tion in mammography as theresult of a recent review by theAmerican College of Radiology(ACR). Mammography is a specifictype of imaging test that uses alow-dose x-ray system to examinebreasts. A mammography exam,called a mammogram, is used toaid in the early detection and diag-nosis of breast diseases in women.

The ACR gold seal of accredita-tion represents the highest level ofimage quality and patient safety. Itis awarded only to facilities meet-ing ACR Practice Guidelines andTechnical Standards after a peer-review evaluation by board-certi-fied physicians and medical physi-cists who are experts in the field.Image quality, personnel qualifica-tions, adequacy of facility equip-ment, quality control procedures,

and quality assur-ance programsare assessed. Thefindings arereported to theACR Committee onAccreditation, which subsequentlyprovides the practice with a com-prehensive report they can use forcontinuous practice improvement.

any animals hibernate throughout thewinter. Humans are afforded no suchluxury. In fact, living a sedentarylifestyle during the colder months of

the year can actually prove detrimental to humanhealth.

Exercise is beneficial any time of the year, but itcan be especially so during the winter monthswhen colder temperatures force many peopleinside, where they live more sedentary lifestylesthan they do during the rest of the year. Adaptingyour exercise habits in the winter can help youmake it through the colder months in great shape.

There is no reason to stop exercising when thetemperature drops. The American HeartAssociation says working out in the cold weatherhas distinct advantages over working out in hotand humid conditions. When the weather is cold,you may be able to work out longer and harderbecause the heat won't zap your energy levels, andexercising outdoors in the winter is a great way toget small doses of sunlight that can improve moodand help your body produce more vitamin D.

The Centers for Disease Control and Preventionsays exercise can help boost your immune system,which can help you fight colds and flu symptoms.Just a few minutes of exercise each day can help pre-vent simple viral and bacterial infections as well.

Working out in the winter may help you burnmore calories than in warmer seasons. Researchpublished in Medicine & Science in Sports &Exercise found that race times are faster in coldweather than in warmer temperatures. Quickerruns or walks can burn more calories.

If exercising outdoors is too uncomfortable,break up your routine into smaller, more manage-able sessions. Aim for 10 minute sessions severaltimes per day. This quickly can add up to the 30minutes of daily recommended exercise. Dress inlayers so you can feel comfortable, adjusting yourclothing as necessary. You don't want to be freez-ing, but you don't want to wear so many clothesthat you start sweating and risk hypothermia.

Consider less traditional exercises when wintersets in. These include shoveling snow, sledding,skating, skiing, and snowshoeing.

Diabetes is a disease that makesit difficult for the body tomanage its blood-glucose lev-

els. Diabetes is broken down into twotypes: type 1 diabetes, which occurswhen the pancreas cannot adequate-ly produce enough insulin to regulateblood sugar; and type 2 diabetes,which occurs when the body eitherresists the effects of insulin or doesnot produce enough to maintain anormal blood sugar level.

The Centers for Disease Controland Prevention reports type 2 dia-betes has been occurring withincreasing frequency amongAmerican children and adolescentsover the past 20 years. The MayoClinic says 90 percent of diabetescases are type 2, and 80 percent ofthese people are overweight.

Being overweight or obese is a sig-nificant risk factor for type 2 dia-betes, but many people do not knowwhy. There is some evidence that fatcells are more resistant to insulinthan muscle cells. In addition, beingoverweight taxes the body in other

ways and can put more pressure onthe body's ability to properly controlblood sugar and use insulin.

Carbohydrate-rich diets full of sim-ple sugars and refined foods cancause unhealthy spikes in blood-glu-cose readings. Individuals whoovereat and lead sedentary lifestylesmay not burn enoughenergy, and excesscalories are then storedas fat, compoundingthe problem.

The Obesity Societysays type 2 diabetes islargely preventable,and research showsthat lifestyle changesthat include smallamounts of weight loss(between 5 and 10 per-cent of body weight)can prevent or delaythe onset of type 2 dia-betes. In fact, manag-ing weight is the bestthing a person can doto reduce their risk for

diabetes. Losing as few as 10 poundscan markedly improve insulin resist-ance and make it much easier tomanage blood sugar levels.

In addition to losing weight, eating ahealthy diet and exercising regularlycan reduce a person's risk of develop-ing diabetes. Lean proteins, leafy

green vegetables andcomplex carbohydrateswill not spike bloodsugar levels. Exercisehelps burn calories andboost metabolism, andmay even help the bodyuse insulin more effec-tively.

Type 2 diabetes canbe a chronic, debilitat-ing condition thatleads to other illness-es. But type 2 diabetesis largely preventable,and losing weight andmaintaining a healthyweight are great waysto lower your risk ofdiabetes.
