H.1 Raw materials, Intermediates, Products Handling

Introduction All materials in use at Stryker Orthopaedics are stored at different locations depending on the nature/status of the material. Stryker Orthopaedics is a medical devices company and prior to coming onsite all materials must receive a validation and approval from the Environmental Health and Safety Manager. This approval process captures the possible use of hazardous materials and identifies whether these materials can be eliminated or if approved, used under strictly control within the working environment. The range of materials on site can be categorized into the following groups.

• Raw Materials • Intermediate Products and Finished Products • Chemicals • Fuels • Waste

The handling - receipt, storage, internal distribution, bunding and containment - of each category of material is described in the following sections. Raw Material Raw materials are delivered on a daily / weekly basis to the site using expeditors who arrange the collection and delivery to Stryker Orthopaedics site. Materials arrive at Stryker Orthopaedics through three different supply route processes. § Materials are supplied directly and these items are stored internally within the

building. Directly supplied chemicals are stored in the raw materials chemical stores.

§ Kanban suppliers check stock levels on a weekly basis and ensure that supplies at the kanban locations are maintained at the agreed minimum stocking levels.

§ Materials and products are stored by a Stryker off-site company and items are called off and delivered to the site as required.

Stryker Orthopaedics maintains a safety stock level for raw materials and orders are raised on a planned basis to maintain the agreed safety stock levels. The raw materials are requisitioned from stores by the local departmental planners. The raw materials are issued to the floor and processed through to intermediate and finished products through work orders which are reviewed on a daily basis within the various production cells. Raw materials and intermediate products are stored within the various work cells and general stores areas situated within the various units of operation. Intermediate Products and Finished Products

All intermediate products and finished products are stored the general stores and also in the production areas where it is intended for use. Depending on the final product produced, finished products are stored in different areas of the plant as follows:

• Recon • Simplex • Biotech

Chemicals All chemicals in use at the plant are stored in dedicated containers or storage tanks, which are bunded to retain the contents of the tank in the event of a spill. All chemicals are segregated according to their individual properties and stored appropriately. Smaller containers are stored outdoors in the chemical stores area. Fuels The primary fuels use at Stryker Orthopaedics is natural gas. LPG gas is used in the ampoule filling process and in the chemical laboratories. Some diesel is used on site for generators and diesel pumps and the diesel is stored in bunded storage tanks. Waste A dedicated waste management area has been established at Stryker Orthopaedics and all wastes are segregated, sorted and/or baled as required prior to collection for disposal/recovery by a permitted waste contractor. Bunding and Containment Details of all onsite bunding and containment are detailed in the most recent testing of bunded structures which is attached in this section.

1.0 Introduction Chemstore were commissioned by Stryker to carry out Hydrostatic Leak testing at their Raheen, Limerick facility. The testing was carried out in July/August 2010 and our thanks to Ms Elaine Ni Chonmhara for her time and assistance. 2.0 Scope The scope of the testing and inspection was limited to the following:

• Concrete Bunds: Hydrostatically test the various designated bunds for ‘liquid tightness’ using the BS 8007 Part 9, a seven day leak tightness test. In certain situations where it was not possible to fully hydrostatically test the bund, a “partial” hydrostatic test was conducted and supported by a visual inspection of the bund walls.

• Steel/ Plastic Prefabricated Bunds: Hydrostatically test the various designated

bunds over a minimum 6 hour test period to prove the integrity of all bunds. Testing to cover the multiple bunds in a number of Chemical storage units.

3.0 Test & Inspection Methodology 3.1 Hydrostatic Test The bunds were pre filled by Stryker between 24 and 48 hours in advance of test readings. Where bunds could not be tested to a volume greater than 110% of the single largest container or 25% of the total stored volume, a detailed inspection of the bund walls was carried out to ensure that there were no obvious leak points. Readings were then taken each day and logged. A reference vessel was partially filled and used as a reference for rainfall/evaporation adjustment. The 6 hour test period was used as steel/plastic bunds which are impermeable materials so leaks will be clearly identifiable as no stabilization period is required. This method refers only to the Transformer Bunds and the Chemical storage units.

3.2 Test Water Levels At minimum, all bunds were hydrostatically tested above the main statistical leak point which is the vertical/horizontal joint where the vast majority of leaks occur. This excludes mechanical impact damage which is a reportable incident. Hydrostatic test water levels varied and were dependant on the following:

• Low level sensitive equipment • Time constraints due to water fill rates • Safety concerns • Risk of accidental spills occurring if bunds were fully charged over eight days

3.3 Readings Readings were taken at the front right hand corner of all bunds or the most appropriate alternative position. The reading point was marked and test water level was measured and the readings were logged. Readings are noted as ‘–‘ to indicate that the measure was taken from the top of the bund wall to the surface of the test water due to accessability and accuracy. Control: A Control Vessel was also appropriately placed to monitor atmospheric conditions and rainfall during the test period. Weather conditions were generally good during testing. 3.4 Visual Inspection Visual inspections were carried out where necessary to identify any indication of cracking, evidence of leakage or deterioration of the bund walls. All inspections were conducted from ground level.

4.0 Results Failure Criteria: During the 7 day test period the total permissible dip in level (allowing for climatic conditions) should not exceed 1/500th of the test water depth or a drop in volume of 10 mm. Volume Calculations: Average bund depth is used to calculate bund retention capacities. Results Summary

5.4 Recommendations Chemstore No. 2 on the left hand side needs to be put on a maintenance plan as there is some deterioration and a valve has been damaged by mechanical impact. Chemstore No.3 on the left hand side is being used to store acids on a metal sump. This is not suitable for long term resistance to acid attack. Simplex transfer pump bund should have the sealing coating repaired. Bulk Mother Liquor load cells to be checked for IP rating so that hydrostatic testing can be carried out to full bund height.

Attachment H2_1: Waste Handling

1 of 1

H.2 Waste Handling

Introduction All waste handling contractors and waste disposal facilities used by Stryker Orthopaedics must be fully permitted (by Local Authorities) and or licensed (by the EPA) as relevant. In addition, all transportation of waste materials from the site is undertaken by contractors who have valid Waste Collection Permits (WCP) in place. Stryker Orthopaedics also maintain a register of all waste contractors and carry out periodic inspections and checks to ensure that relevant information is up to date. The wastes that arise through the different production processes at the plant are dealt with in a variety of ways. Waste management is based on a hierarchy of Prevention, Minimization, Recycling and Safe Disposal. Detailed operational procedures have been developed for dealing with all non-hazardous and hazardous wastes generated at the Stryker Orthopaedics site. Non- Hazardous Waste Non-Hazardous wastes generated on site are stored in bins at source and subsequently transferred to skips and the waste recycling area, which is located at externally at the eastern side of the facility. Hazardous Waste Hazardous wastes are stored in designated locations within the waste management area prior to disposal off site through licensed waste disposal contractors.
