    Fungal (Mycotic) Infections

    y Fungi: tiny members of a subdivision of the plant kingdom

    y Thrive on organic matter and in a warm, moist environment

    y Commonly affect only the skin and appendages (hair and nails)

    y Infections of the skin and appendages usually respond to topical treatment

    y Systemic fungal infections that affect internal organs are of major concern as

    well as secondary infection with bacterial Candida

    y May be diagnosed by a skin scraping or by using Woods light (fluoresces)

    y Contagious fungal infection of the hairshaft

    y Common in children

    y Oval, scaling, erythematous patches

    y Small papules or pustules on the scalp

    y Brittle hair that breaks easily

    Treatment:

    y Griseofulvin for 6 weeks

    y Nizoral or Selenium Sulfide Shampoo

    y Begins with red macule, which spreadsto a ring of papules or vesicles with

    central clearingy Lesions found in clusters

    y Many spread to hair, scalp and nails

    y VERY PRURITIC!!!

    y An infected pet may be the source

    Treatment:

    y Mild conditions: topical antifungalcreams

    y Severed infections: Griseofulvin orterbinafine

    plaquesy VERY PRURITIC!!!

    y Clusters of pustules may be seenaround borders

    Treatment:

    y Mild conditions topical antifungalcreams

    y Severe conditions griseofulvin orterbinafine

    y Soles of one or both feet have scalingand mild redness with maceration in

    the toe websy More acute infections may have

    clusters of clear vesicles on dusky base

    Treatment:

    y Soak foot in vinegar and water solution

    y Resistant infections: treat withgriseofulvin or terbinafine (Lamisil qd x 3wks

    y Entire nail may be destroyed

    Treatment:

    y Itraconazole (Sporanox) in pulses of 1week a month for 3 months in cases ofterbinafine failure
