Froedtert Health Community Memorial Hospital PGY1 Pharmacy Practice Residency Program Fact Sheet

PGY1 Pharmacy Practice Residency Statement of Purpose Residents who successfully complete this 12 month PGY1 Pharmacy Practice residency will be competent to confidently provide safe, patient centered, cost-effective, pharmaceutical care across Acute and Ambulatory Care settings. This residency will provide postgraduate education for licensed pharmacists enhancing knowledge and problem solving skills in the various aspects of pharmacy practice in a community hospital setting. Community Memorial Hospital is located northwest of Milwaukee in Menomonee Falls, Wisconsin. The hospital is part of the Froedtert Health System which is comprised of Froedtert Hospital, St. Joseph’s West Bend Hospital, West Bend Clinic and the Froedtert Health Medical Group clinics. Residents who successfully complete the Froedtert Health Community Memorial Hospital PGY1 Pharmacy Practice Residency will be able to provide integrated, safe, cost-effective, patient centered pharmaceutical care across the continuum. This residency will provide candidates with the opportunity to work as a respected member of the healthcare team.

260 licensed bed community hospital, 63% overall occupancy rate 21 bed ICU 60 ED visits/day; 50% of admissions to CMH come through the ED 27 Pharmacist, Supervisor and Director FTEs 22 Technician and support staff FTEs 2.8 Informatics pharmacists FTEs, 2 Informatics technician FTEs Clinical pharmacist services offered 24 hours daily Hospitalist and Intensivist services; 24 hour eICU capabilities Extensive pharmacist managed pharmacokinetic and anticoagulation services Pharmacy conducted medication histories in ED and Home ToDay surgery Clerkship site for pharmacy and technician students, teaching and pharmacy clerkship preceptor opportunities ED pharmacy coverage Antibiotic Stewardship program Cancer Care Center pharmacist Outpatient Anticoagulation Clinic (400 clinic visits/month, 200 phone managed visits/month) Level 3 trauma center Joint Commission Certified Stroke Center Ambulatory Outpatient Pharmacy, Medication Therapy Management On site pharmacy informatics team (Epic Willow, Pyxis, Plum A+ infusion pumps, Bar coded medication administration) CPOE Implementation early May, 2012 Pharmacist managed community vaccination clinics Community Outreach Health Clinic on site

PGY1 Pharmacy Practice Residency Requirements and Expectations This 12 month PGY1 Pharmacy Practice residency offers comprehensive training in a variety of inpatient and ambulatory clinical areas. It also offers exposure to administrative activities including compliance with accreditation standards, departmental staffing & performance indicators, revenue cycle & budgeting, policy & procedure development and quality & safety measures. PGY1 Pharmacy Practice Residency Rotations Pharmacy services are provided by pharmacist teams within an integrated practice model. The team pharmacists are decentrally located and provide care as part of an interdisciplinary team through patient care rounds in the ICU; clinical monitoring and medication order review of inpatients; admission histories in ED and Home ToDay surgery; first dose teaching; participation on code blue, trauma and stroke response teams; drug information resource and medication use stewardship. Required Rotations
