I am a male adult withlD: 461112 5001 081living at 110 Bantry Road, Bronberrik, Centurion. lmet Dr DS Grieve whenhe hadthe practice in Jean Avefor medicaltreatment, and he became ourfamily doctor. This remained the case whenhe moved to Saxby Medical Center, we continued a doctor patient relationship untilhe soldthe practice. He told me that he was going intobusiness by buying the Cynthia's restaurant. By chance I ran intohimin towntowards the endof 2006. In an informal chathe related to me thatthe venture was successful, but pointed outthat he hadan excellent investment opportunity, and invited meto make an appointment to meet at the restaurant. We meton 1Sth January 2007, andtfie bridging solutions concept was explained to me,which was a secure investment, he"lidVe me his personal assurance and guarantee thatthe money would either be in his account or thatof the transfer attorney, or a financial institution, safeguarded andprotected at all times. We signed the agreement to invest whenhe presented mewitha document thatwas headed "Shareholders Loan Agreement". As it was explained to me many times by Dr DS Grieve and understood by me,thiswas an investment Not being a legal expert I did not see any reason to question the validity of the document, which had been drawn up by his attomeys. I transferred R300,000 to hisAbsaaccount 4066017726thesame day.I trusted him fullyas my doctor. On the 1't December Dr DS Grieve suneptitiously liquidated his company and sequestrated himself thereafter. Thiscaused me the lossof lifesavings andthe entire investment. 10.I have goodreasons to believe from newspapers andother reports that Dr DS Grieve has acted dishonestly anddefrauded me and allthe other investors withcriminal intent. 11.I knowand understand the contents of this declaration. I have no objection to taking the prescribed oath. lmet Dr DS Grieve when he had the practice in Jean Ave for medicaltreatment, andhe became our family doctor.

This remained the case when he moved to Saxby Medical Center, we continued adoctor patient relationship until he sold the practice. He told me that he was goinginto business by buying the Cynthia's restaurant.

By chance I ran into him in town towards the end of 2006. In an informal chat herelated to me that the venture was successful, but pointed out that he had anexcellent investment opportunity, and invited me to make an appointment to meet atthe restaurant.

We met on 1Sth January 2007, and tfie bridging solutions concept was explained tome, which was a secure investment, he"lidVe me his personal assurance andguarantee that the money would either be in his account or that of the transferattorney, or a financial institution, safeguarded and protected at all times.

We signed the agreement to invest when he presented me with a document that washeaded "Shareholders Loan Agreement". As it was explained to me many times byDr DS Grieve and understood by me, this was an investment

Not being a legal expert I did not see any reason to question the validity of thedocument, which had been drawn up by his attomeys.

I transferred R300,000 to his Absa account 4066017726thesame day. I trusted himfully as my doctor.

On the 1't December Dr DS Grieve suneptitiously liquidated his company andsequestrated himself thereafter. This caused me the loss of life savings and theentire investment.

10. I have good reasons to believe from newspapers and other reports that Dr DS Grievehas acted dishonestly and defrauded me and allthe other investors with criminalintent.

11. I know and understand the contents of this declaration. I have no objection to takingthe prescribed oath. I consider the oath to be bindlgg on my conscience.
