Print the S313 panel to notify ELL Contact to test student.

SURVEY DATE For Pre-Registration prior to beginning of school, the Survey Date is the first day of school. After the first day of school the Survey Date is the date in which the parent answers the Language questions and dates the Student Enrollment form.

Special Attention to Entry Code E09 (Enrolling from out of country)

If the student is enrolling from out of the country; verify if the student has previous schooling in the United States and notify ELL staff.

If prior ELL program is selected on the registration form, notify ELL Staff.

Student must be tested within 20 days of enrollment.

ELL Committee as needed with reference to cum for prior ELL documentation.

Note: The US territory extends to Puerto Rico (PR) and the Virgin Islands (VI)

Use for the following fields: Co (Country verification) = PR or VI PD (Previous District) = #99 PS (Previous State) = PR or VI PC (Previous Country) = US

Immigrant: If the student is between the ages of 3 and 21; is born outside the United States; and had less than 3 years of education in the US, then the student should be coded as Immigrant and the date they entered the country completed.

Essential Data Elements on the S313 Demographic Panel

It is imperative that these fields are completed accurately.

• Home Language Survey (HLS) The 3 language questions on the Enrollment Form

• Hm = Language spoken at home • Pl = Primary language spoken by student • Sl = Secondary language spoken by student

If the Parent/Guardian indicates that a language other than English is spoken on at LEAST one

question above then student is coded “LY” on the Lp field directly to the right of the language codes until further evaluation can be conducted.

• Home Language Survey Date The HLS date matches the date the Parent/Guardian signs the Enrollment Form.

NOTE: When student pre-registers prior to school starting the HLS Date is the first day of school. Example below:

• Date First Entered US (DtEntUS) DtEntUS must be collected on every student. This date is the day the student first sat in a US

school grade levels KG through 12th. If the Parent/Guardian does not have the exact date use the last day of the month and year of

when they started.

Give Form to Data Entry; Contact must indicate which “L” code to be entered. Once information is entered in TERMS, a Student Educational Plan must be printed.

Review, sign and date the new Student plan and file along with Committee form and testing (if applicable) in the blue folder.

Make 2 copies of the form; file original in blue folder and send one copy home to the Parent/Guardian and one copy to SSPAR Office Box

Date is the Conference Date on the ELL Committee Form (Rev. 05/02/14).

For the Cd field: ELL Contact should supply this code for you. If not, use the F4 function key to choose which “L” code best suits the

reason for the ELL Committee.

Most of the “L” codes have been placed on the ELL Conference Committee & Parent Notification Form to assist you.

After EVERY ELL Committee has been entered on the S322 panel and all other information is updated the plan date on the S706 panel is updated to the same date as the conference date on the form.

A new Student Educational Plan must be printed using the F6 function key.

DATA ENTRY

When to print a Student Educational Plan:

Beginning of each school year after scheduling has been completed.

New ELL Record created.

ALL Schedule changes.

After EVERY ELL Committee is held for student. (Update the Plan date on S706 prior to the conference date)

Transfers from within CCPS. (Update Plan date on S706 to date of enrollment in new school)

Exiting of the ELL program. (Update the Plan Date on S706 to the exit date)

After Review D has been entered on the S706 and student has become LZ.

How to print a Student Educational Plan:

Printing of mass school: From L100 panel SL725

The Student Educational Plan is the entire overview of the student’s ELL record. The plan includes:

Name, address and contact information from the Demographic S313 panel.

Entry and Exit dates and test information from the Limited English S706 panel.

The testing dates and scores related to ELL from the Test Summary S734 panel.

ELL Committees held on student from the Activities S322 panel.

Student Instructional Program (courses) from the Schedule S615 panel.

The student’s Current Year Grades from the Student Grades S517 panel.

The Student Educational Plan MUST be signed by a certified person and filed in the Blue folder.

How to audit an ELL Student Educational Plan

The Educational Plan shows the student’s entire ELL record as entered in TERMS. With this tool you are able to cross check the following:

Basis of Entry testing dates and scores, Survey Date, Classification Date, Program Entry date, Plan Date, and Level of Fluency all gathered from the Placement Form.

Annual testing information gathered from the Parent Notification Form.

Re-Evaluations held for numerous reasons gathered from the Committee Conference Form.

The student’s schedule with proper coding of 130 PGM and Instructional Model.

Basis of Exit testing dates and scores, Plan date, Exit Date, and the schedule cleared of all 130 PGM and Instructional Model.

2 year Follow-up period in which the student’s ELL status is “LF”.

A complete view of the student’s ELL record after the completion of the 2 year follow-up period ELL status is “LZ”.

Follow Up (Review) Period: The 2 year period after exiting from the ELL Program. See page 31.

ELL Committee: A meeting held to discuss the student Educational Program. Possible Attendees: Parent, Teacher/Instructor, Guidance Counselor,

ESE Specialist, ELL Contact and Administrator/Designee

Re-Classification: A student currently coded as “LF” and an ELL Committee has decided to place the student back into the ELL Program.

Re-Exit: A Re-classified student who is now ready to re-exit the program. An ELL Committee must be held to exit this student.
