Benefits of partnership working• It is essential that everyone concerned

with the safeguarding, protection and welfare of children and vulnerable adults work together in partnership.

• Information needs to be shared accurately and effectively

• Failure to share and act on information given could have serious consequences for the welfare of children and vulnerable adults.

Review of Baby P Case• Baby P died in 2007 after months of abuse.

In 2009 a serious case review stated that Baby p was failed by all agencies involved.

• All three children living in the house at the time were on the Local Authority’s child protection register.

• The GP did not raise concerns after bruising was found on Baby P’s head and chest.

• Police were criticised for not investigating suspicious injuries.

• The school attended by Baby P’s siblings did not share information about concerns over the mother.

• Social workers and their managers did not think he was being harmed or at risk of harm.

• No agency involved realised that a man with known violent tendencies was living in the house.

Why is it important to have secure systems?

Why breach confidentiality?

• This may need to happen when a service user discloses a serious risk, or a safeguarding issue.

• A health and social care practitioner must never tell a service user everything they tell you is a ‘secret’.

• If a disclosure is serious and affects the persons well-being you need to explain you will have to pass the information on.
