Diltiazem for maintenance tocolysis offered no benefit over nifedipine in women experiencing preterm labour before 35 weeks’ gestation. This is the finding of US-based researchers who compared the effects of diltiazem with those of nifedipine in a prospective study involving 69 such women. The women had been successfully treated with IV magnesium sulfate and after this drug was tapered off they were randomised to receive either oral nifedipine 20mg every 4–6 hours or oral diltiazem 30–60mg every 4–6 hours for tocolysis. Mean gestational age at delivery was 35.5 and 33.4 weeks among the nifedipine and diltiazem groups, respectively. 41.7 and 15.1% of women in the respective treatment groups were delivered at 37 weeks’ gestation.
