CVS4 Cart The CVS4 cart has the following features: •There are four wheels with one that locks for safety.

•The drawer holds all of the cuffs, RD2 cuff deflator, MV10 segmental cuff selector and the manual.

•The HP printer is plugged in to the main power on the cart.

•The shelf for the computer also holds the TD312 (software remote and cuff inflator). The MD35 instrument is on the top shelf and the plastic instrument holder will hold the CW Doppler, Photo transducer and the pneumoplethysmograph hose and stopcock for easy access.

•The flat panel monitor is attached to the top of the cart with a single screw and the full keyboard with trackball mouse sits in front of the monitor. The speakers are built into the monitor.

•The ON/OFF switch is on the top of the cart to the back on the user’s right. The power for all instruments comes through this switch so if the switch is off you will not be charging any instrument plugged into the main power.

•Toggle switch on front of cart to switch from computer to chart recorder. To send waveforms to computer, it must be set to computer.

CVS4 Computer, Monitor and Printer

The CVS4 computer, monitor and printer:

•The CVS4 has the NIVP3 software, printer drivers and the associated drivers for audio visual capabilities installed.

•This system can be connected to a PACS system when the DICOM Extension is purchased and installed.

•All warranties are provided by the specific manufacturer of these devices.

CVS4 Instrument MD35 Procord The Hokanson CVS4 utilizes the MD35 instruments to perform complete physiological peripheral vascular testing. There are three modes of instruments: CW bi-directional 5 MHz Doppler, photoplethysmography (using infrared) and pneumoplethysmography. Some tests can be performed with more than one instrument so you can select the one that works best for your particular patient. If necessary the chart recorder/computer switch can be set so the two speed chart recorder in the MD35 can record the volume information from two plethysmographs or the velocity information from the Doppler. The Tests are listed on the next slide/page.

Power: Push in for on; push & release for off.

Chart Recorder: toggle on switch at rear of instrument.

Run Stop: start and stop chart recorder.

Speed Chart Recorder: slow is 5 mm/sec; fast

is 25 mm/sec.

Zero: to balance the

plethysmographs. Artery/Vein: Use Arterial when baseline level is not desired & only pulsatile information is wanted; Use Vein for veinous exams when long term baseline changes are important.

Position: slide control for neutral position of baseline on screen or chart recorder

MD35 Instrument Panel (front view)

Range sets the sensitivity of the instruments. The highest sensitivity is 0.01 and the lowest is 10.0. Range is also called gain. Dopplers usually use from 2 to 0.2. Pneumoplethysmogaphy does not use the range nor does photoplethysmography; however the range control sets the size of the signal on the screen or chart

recorder for both plethysmographs. Always set computer and MD35 range to same range setting.

Select: Push to select mode of operation. Note light under

each mode to the right turns on to show the selection.

Pneumo.

MD35 Instrument

How to connect instruments: MD35 Doppler: Depress the small black button on the top of the connector and insert into Doppler outlet. When in Doppler mode the light above the outlet lights.

Photo Transducer: Depress the small black button on the top of the connector and insert into the outlet. Do not pull on cable to remove from plug. Light above the connector lights to indicate photo plethysmograph mode is selected.

Pneumo: Connect the blue ended pneumo plastic tubing to the blue coupling under INF label then connect tubing to the MV10 or a cuff. Connect the TD312 metal connector to the black REC for inflating the cuffs.

Remember to use the select button to choose the mode for the test and confirm that the light turned on for that mode.

Pen Heat lets you change the thickness of the line drawn by the stylus on the

chart recorder using a small flat-bladed screwdriver. Clockwise makes the line wider and counterclockwise makes the

Segmental Pressure Accessories MV10 and Cuffs

The MV10 facilitates placing all segmental cuffs on the patient (up to 10) and then attaching the cuff inflator to the metal male fitting at the bottom for inflating cuffs easily. You can flip the switch to change the cuff to be inflated.

The pressure cuffs are straight segmental cuffs with the cuff width 20% larger than the limb diameter to compress all of the soft tissue evenly.

TD312 Using the TD312 as a Software Remote

TD312 Recharging

To Recharge the TD312 plug the AC charger cable into the TD312 and into an AC outlet and let it charge over night. Do not plug it into the CVS4 cart unless you leave the power on for the whole cart while charging the TD312. The AC charger is a trickle charger so the TD312 always runs off battery power not AC power. The battery should be at 12 or above when checked.

RD2 Rapid Cuff Deflation

To use the RD2 for rapidly deflating cuffs, you plug the TD312 into the side male fitting (next to the RD2 label) and put the CC17 cuff’s tubing into the 2 large Y openings. To deflate the cuff, push the plunger down and the cuffs instantly deflate.

MD35 Doppler

The MD35 Doppler has the transmitting and receiving elements aligned vertically in the face of the probe. The raised area on the probe surface

is the near focus lens which improves the performance for shallow vessels less than 1 cm from the skin surface. Keep red protective cover

on when not in use to protect the crystals.

The direction-sensing, 5 MHz CW Doppler can be used for most peripheral vascular exams. The volume switch is the thumb wheel. The toggle switch near the transducer can

invert the Doppler waveform in spectral Analysis. The white button on the end is a start/stop button for the chart

recorder.

The Select button on the MD35 must be set to Doppler.

MD35 Doppler Positioning the Doppler Probe

To position the Doppler probe correctly, first feel the pulse with your fingers and put some coupling gel on the spot. Use enough gel for a good coupling. Angle the Doppler 45-60 degrees (an acute angle) towards the head of the patient (towards the flow of arterial

blood) and move the Doppler slowly until you obtain the best Doppler signal (sound) and waveform. If the vessel is deeper than 1 cm do not use the dimpled or bifocal side of the probe tip.

MD35 Photoplethysmography

The photo transducer is placed with the diodes in direct contact with intact warm skin. The Velcro will keep the transducer from moving. Always use the finger or toe pads for best results and make sure the transducer fits snuggly (visible light can interfere) but

not too tight to impinge the circulation. You can also use plastic tape to insure adherence. If there is something constricting the circulation to the digit or if the patient is cold and their fingers or toes are cold, you will not get good or possibly any waveforms.

The Select button must be set to Photo.

MD35 Pneumoplethysmography

Pneumoplethysmography measures and records volume changes in a limb or digit using the pressure changes in a recording or sensing cuff which is applied at low pressure (below diastolic pressure.)

The Select button must be set to Pneumo inflate to inflate and Rec to record. The light turns on

over the specific function.

Loading paper into your MD35 Pull on the black tab that protrudes from the right side of the recorder to open the door. Remove the core of the previous roll of paper by lifting it off the spindle. Unroll enough of the new roll of chart paper so the words along the bottom of the chart paper can be seen.

Orient the roll with the words right side up on the bottom edge and place roll of spindle. Pull out about 6 inches of paper and close the door. Do not attempt to thread the paper under the bar on the right edge.

Run the recorder at fast speed and insert the loose end of the chart paper under the right side of the rollers on the left side of the chart recorder.

Run a few inches of paper to allow the chart to straighten out before stopping the recorder.

Cuffs and Cuff Inflator Included with your CVS4 System are (10) Hokanson brand straight segmental

vascular cuffs & a transmetatarsal cuff. Each

cuff consists of two separate parts; the cuff cover and cuff bladder, both of which may be replaced at any time. Hokanson brand cuffs

come standard with female Leur fittings. To inflate a

cuff simply connect it to the S300 cuff inflator

(included) and squeeze the bulb of the S300. The

corresponding cuff pressure can be read on

the S300 dial.

Cuffs and Cuff Inflator

The TD312 will inflate all cuffs included with the CVS4. The

MV10 can be used with all of the cuffs to facilitate rapidly moving

from cuff to cuff taking pressures.

The CC17 Contoured Cuff is

designed to provide a better fit for tapered heavy legs or arms.

Digit Cuffs

Included with your CVS4 System are Hokanson brand polyurethane digit cuffs and a sample disposable digit cuff

(one patient use only).

Hokanson brand cuffs come standard with female Leur

fittings. After applying the cuff to the toe, to inflate a cuff

simply connect the hose to the S300 cuff inflator and squeeze the bulb of the

S300. The corresponding cuff pressure can be read on

the S300 dial.

How to apply MD35 Photo transducer and Cuffs

The cuff is selected by the size of the toe to be tested. The cuff fits on the proximal phalanges between the foot

and the metatarsophalangeal joint.

The Photo transducer is placed on the pad of the

same toe and the Velcro is wrapped around the toe to

hold it securely in place. The patient’s toes must be warm. The TD312 can be used with digit cuffs as can the MV10.

How to Perform a TBI Equipment needed: • A selection of toe cuffs, aneroid sphygmomanometer, photo plethysmograph/transducer and chart recorder. • Patient is prone with legs and arms at heart level in a warm room with shoes and socks off. Procedure: • Measure cuff to a toe, width should fit between foot and first MTP joint (metatarsophalangeal joint) and the length should wrap

around the toe comfortably with the bladder of the cuff overlapping. Put the cuff on the toe. • The photo plethysmograph is connected to the chart recorder securely to record the waveforms. • Place the photo transducer on the toe pad so that it makes complete contact and then wrap the Velcro strap around the toe so

that the transducer cannot slip and makes good contact but does not compress the toe pad. This is important since the photo plethysmograph transducer is transmitting and receiving reflected light from red blood cells in the toe pad vascular system.

• Connect the sphygmomanometer to the cuff. Run a strip of the toe waveform. Adjust the waveform speed, range, position as necessary.

• Inflate cuff until waveform disappears and then inflate from 20-30 mm Hg above that pressure. • Slowly deflate cuff 2 mm Hg at a time until the waveform reappears. Stop deflating and note the pressure. This is the systolic

pressure for that toe. • Deflate cuff completely. • Repeat this process on a toe on the other foot. Key points: • Cuff should fit toe properly and be snug but not tight. • Photo transducer should maintain contact with toe pad but not compress tissue. • Toes should be warm enough for taking the pressure. • TBI’s should be taken only if incompressible vessels are suspected due to abnormally high ABI of 1.35 or above. • A TD312 cab be used instead of the S300 to inflate these cuffs. Use the ½ speed since these cuffs are small.

How to Perform a Pulse Volume Recording (VPR/PVR)

Equipment needed: • A selection of straight segmental cuffs (3 or 4 cuff method), aneroid sphygmomanometer, pneumo plethysmograph and chart recorder. • Patient is prone with legs and arms at heart level in a warm room with shoes and socks off. They can wear a patient gown or remove long pants and wear shorts

so their legs are bare. Procedure: • Measure cuff to each leg site, the cuff should fit around the limb comfortably with the bladder of the cuff overlapping. Put cuffs on the leg using either a 3 cuff

method or a 4 cuff method. • The pneumo plethysmograph is connected to the chart recorder securely to record the waveforms. • The cuffs act as sensing cuffs when inflated to a pressure high enough to record the pulse volume changes as a wave form. • Connect the MD6PN tubing with the white fitting to the stopcock, then connect the stopcock to a cuff with the Leur fitting and then connect the SC300 to the

stopcock. • To inflate the cuff turn the stopcock so the off points to none of the ports. Inflate to a moderate pressure from 20 – 50 mmHg. • You should use the same pressure reading for each site to compare like to like. • Turn the stopcock so that off points to the S300 inflator. Have the patient lie quietly and turn on the MD6VR. • Push the Run button and then the Zero to zero the pressure. Run a strip of the waveform. Adjust the waveform speed, range, position as necessary. • In Vein mode the baseline will move up and down on the chart paper with limb volume changes (you might keep the sensitivity lower to minimize this). • In Arterial mode the baseline stays steady, but the high filter will distort the waveform by filtering out long term changes. If the waveform flattens out hit Zero again

to re-zero the baseline and bring back the waveform. • You will repeat the process moving down or up the leg to each site. • Key points: • Cuff should fit toe properly and be snug but not tight. • Use the same pressure XXmmHg for each cuff for consistency. • Legs and feet should be warm. • Compare the waveforms looking for monophasic or abnormal waveforms.

Hokanson Cuffs

Hokanson manufactures and sells many cuffs of various sizes, conformations and components for taking blood pressures in the limbs and digits. All straight segmental or

tapered cuffs have a nylon fabric cuff with a replaceable bladder. There are single patient use only digit cuffs as well as digit cuffs that can be reused and are warranted for one year of

patient care.
