Administrative, Clinical and Research Expertise: Administrative: As Chief of Endocrinology at the VA I supervise 5 faculty, 3 nurse practitioners, 1 certified diabetes educator and administrative staff. We have a successful integrated program that combines in person consultative practice with ENDO TELE health for our VISN, ENDO Electronic E Consult program which has grown from 50 to 650 consults/year and ENDO ECHO SCAN programs in diabetes and endocrinology to provide subspecialty care outreach to veterans and primary providers. We have just received funding for PEERS: Program in Endocrine/Diabetes Education in Rural Settings which will test the effectiveness of 3 pilot models to improve care: Immersion Program, Transition of Care and TEAM clinics and provide funding for faculty NP, CDE and administrative staff.

At the University of Colorado, my focus is Reproductive Medicine and Pituitary disorders and have built a Pituitary Program with expansion of clinical, educational and research efforts in pituitary disorders. I supervise 2 faculty, 1 administrative staff and work closely with colleagues in neurosurgery, neuropathology and Neuro-oncology. We currently have 4-5 clinical research protocols ongoing. We have also created a multidisciplinary Adrenal Neuroendocrine Tumor program to build a clinical team with concomitant research and educational arms.

Within the UCSOM CTSA, I organized and run the KTR program which pre-reviews R type applications of K awardees or other junior investigators with an in house mock study section 3 times/yr that the applicants attend to improve their understanding of the review process and successful competition for R funding. Our outcome measures show increased funding success rates higher than national averages.

Clinical: Reproductive Endocrinology, pituitary and adrenal disease (2-1/2d clinics/wk, 1 mo endo attending, 2 wks Medicine attending) Expertise in endocrinology, with focus in hormonal disorders in women and men, pituitary tumors and adrenal and other endocrine neoplasms.

Research: -Our lab is interested in the ontogeny of the GnRH neuronal system, the developmental control of GnRH synthesis by peptide and steroid hormones and regulation of neuronal migration. Recent efforts focus on the epigenetic role of Class II histone deacetylases (HDACS) to modulate the developmental program of GnRH neurons. Insight into the control of the GnRH neuronal network may shed light on the normal physiology of the reproductive axis, a better understanding of the pathophysiology of the system and new treatment options for patients.

-A second translational focus is in the molecular pathogenesis of pituitary tumors. Using molecular and bioinformatics approaches we have identified novel candidates involved in pituitary tumors causing altered proliferation, migration and invasion. Analysis of human tumors coupled with use of mouse models and cell systems to explore mechanisms may give new biomarkers that can be used clinically for aggressiveness and lead to new treatments for our patients. We recently have identified a novel kinase, MST4, which mediates tumorigenesis and survival in response to hypoxia,that may prove to be a new targetable kinase in pituitary tumors. Other studies are identifying histologic and molecular markers for aggressiveness in GH tumors to predict response to medical therapies and dissect underlying mechanisms.

-Clinical research projects include: -Understanding the role of sex steroids in recovery from traumatic brain injury: I am coPI on a clinical investigation of the effects of testosterone therapy to hypogonadal men after TBI on the functional rate of recovery and neurocognitive response to acute rehabilitation. This is a placebo controlled study of physiologic testosterone therapy during an acute rehabilitation program after TBI. -We have 2 industry trials of the use of weekly compared to daily GH therapy for patients with hypopituitarism -We participate in a national database of patients with acromegaly being treated with a GH receptor antagonist -We are funded to test the effectiveness of a novel drug to treat recurrent Cushings syndrome -We are negotiating a trial of an oral somatostatin analog for treatment of acromegaly
