City of CincinnatiThe Creating Healthy Communities Program (CHCP) and the CHCP Coalition support healthy eating,

active living, and tobacco free environments in the City of Cincinnati. 

Community: With 7 farms and over 100 community participants, CHCP distributed ~5000 pounds of locally grown, healthy food. CHCP also assisted in creating a city-wide mobile produce vending policy championed by Councilmember Quinlivan.

Community / Healthcare Setting: CHCP instituted a Chronic Disease Self Management Program using the Standford University model in Cincinnati Health Department health centers to help patients self-manage chronic disease through personal goal setting, healthy behavior modification, and improved communication with health care providers.

Schools: Healthy Monday Campaigns encourage people to start healthy behaviors each Monday and continue them throughout the week. CHCP and local coalition members implemented Mentoring Meals Monday in our local public schools, to allow teachers to role model healthy eating with students during lunch-time.

Worksites: Through CHCP’s leadership efforts with the Tri-State Workplace Wellness Collaborative (TWWC), CHCP helped Remke Bigg’s and Northern Kentucky University establish tobacco free campuses that impacted 2000 employees. The TWWC educates over 100 large businesses in the tri-state area, and highlights best practices in workplace wellness.
