Objective To provide a corporate standard that outlines SCL’s procedure for managing hazards associated with ionising radiation.

Scope This corporate standard applies to any work location within a SCL operated and / or maintained site where radioactive substances / apparatus are used / stored, or any situation in which a SCL employee or SCL controlled contractor is required to work with, or in the vicinity of, a radioactive substance.

Definitions Carrier: Means any person, organisation or government undertaking the carriage of radioactive materials by any means of transport. Competent Person: A person who has through a combination of training, education and experience, acquired knowledge and skills enabling that person to perform correctly the specified task. Industrial gauging: Means where a radioactive substance incorporated in a sealed source apparatus is used for the measurement of thickness, level and density gauging or for control purposes. The sealed source apparatus is generally installed in a fixed position. Industrial radiography: For the purposes of this standard, means the use of ionising radiation to obtain information non-destructively about the physical condition of objects and materials. Ionising radiation: Means electromagnetic or particulate radiation capable of producing ions, but does not include electromagnetic radiation of a wavelength greater than 100 nanometres. Licensee: Means a possession licensee, use licensee or transport licensee. Non-ionising radiation: Means: electromagnetic radiation of a wavelength greater than 100 nanometres, or sonic radiation. Personal alarm dosimeter: A device that produces a visual or audible signal when: any radiation dose received by the device is more than a certain dose level, or any radiation dose received by the device during a particular period is more than a certain dose level. Personal monitoring device: A device designed to be worn by a person to monitor any radiation dose received by the person. Radiation: Ionising radiation or non-ionising radiation. Radiation safety officer: A competent person who holds a current approved radiation safety qualification / licence, issued by the statutory radiation safety authority for the particular state / territory. Radioactivity: The spontaneous transformation of an unstable atom and often results in the emission of radiation. This process is referred to as a transformation, decay or a disintegration of an atom. Radiation source room: A room, or fixed and discernable part of a room, or an identifiable discrete part of the room, in which a particular radiation source is energised or used to carry out radiation practises. Radioactive substance: Means radioactive material (whether or not sealed): containing more than the concentration or activity of a radionuclide prescribed under a regulation,

or prescribed under a regulation to be a radioactive substance.

Hazards In relation to radioactive substances, a safe system must be implemented to control risks to health and safety arising from hazards such as, but not limited to: release or leakage of a radioactive substance; unsafe handling of a radioactive substance; and exposure of personnel to a radioactive substance.

Controls General A radiation source is not to be used or stored on a SCL site unless the site holds the relevant Possession Licence issued by the statutory radiation safety authority for the particular state / territory. Refer also to Attachment 1, Statutory Radiation Safety Authorities contact details.

A radiation source is not to be used or stored on a SCL site unless a Radiation Safety and Protection Plan, refer also to Radiation Safety and Protection Plan, has been developed by the site and endorsed / approved by the statutory radiation safety authority for the particular state / territory.

All industrial gauges to be used on a SCL site are to be in accordance with NM009: 2010 -Standard for Sealed radioactive substances incorporated in sealed source apparatus used to carry out industrial gauging.

Work in the vicinity of an industrial gauge is not to be undertaken unless the work scope and access is discussed with a SCL Radiation Safety Officer (RSO) and any controls specified by the RSO are included as part of pre-work planning and access/work.

As part of the ATW process a Radiation Control Guide is to be completed prior to any industrial radiography activity being undertaken on site or any activity involving the removal of guarding/shielding from a radiation density gauge on-site.

If for any reason, an industrial gauge is required to be moved or worked upon, this type of activity is to be undertaken as part of ATW and Plant Isolation processes. Refer also to the Plant Isolation Corporate Standard. This type of activity will also require a Radiation Control Guide to be to be completed as per the General Section above.

Transport

Transport of a radioactive substance is to be undertaken in accordance with the Australian Radiation Protection and Nuclear Safety Agency (ARPANSA) - Code of practice for the safe transport of radioactive material (2008).

Only authorised carriers are to transport industrial gauges.

Except in emergency situations, industrial gauges are to be transported with the shutter locked in the shielded position at all times. This shutter or source control mechanism must be fail-safe.

Note: A radiation survey meter is to be used to verify that the radiation beam is properly attenuated.

Industrial gauges are not to be transported in the passenger compartment of a vehicle, and are to be placed as far away from the driver’s position as possible. Except those exclusively reserved for couriers specially authorized to accompany such packages.

Industrial gauges are to be adequately secured (e.g. strapped, chained etc.) to prevent movement, particularly in the case of a vehicular accident.

Appropriate transport signage, in accordance with the ARPANSA Code of practice for the safe transport of radioactive material (2008) including the UN number, is to be clearly affixed to both sides, and the rear of the vehicle. Industrial gauges that are to be transported outside of SCL site boundaries are to be packed in an outer container that is of strong, rigid construction.

Note: The gauge is to be effectively immobilised within the outer container (e.g. bolted to the outer

container).

Unless the radioactive substances can be transported as an accepted package (as defined in the Transport Code), it is to be consigned to a carrier licensed under the radiation safety legislation for the particular state / territory.

Note: A shipper declaration for transport of radioactive substances is to be correctly completed prior to

consignment.

Storage Premises where radioactive substances are stored are to comply with radiation standard PR100: 2010 Standard for Premises – Ionising Radiation Sources, and certificates of compliance from an appropriately accredited person are to be obtained before initial use and every five years thereafter.

Premises where industrial gauges are used are to comply with radiation standard PR100: 2010 Standard for Premises – Ionising Radiation Sources,, and certificates of compliance from an appropriately accredited person are to be obtained before initial use and every five years thereafter.

Storage facilities must have the hardware required to prevent access by unauthorised persons installed.

Ventilation is required where radioactive material is likely to emit a radioactive gas, the radiation source room must have separate and adequate ventilation to the outside air to reduce the concentration of gaseous radioactive materials to at or near background concentrations in a reasonable time.

Appropriate signage must be displayed in a conspicuous location, either prior to entry to the radiation source room, or immediately upon entry to the room.

Contact telephone number(s) of the radiation officer must be displayed either, at the entrance to the radiation source room or in the radiation source room. Note: The radiation source room must not contain, or be in proximity to, other dangerous goods.

Industrial gauges stored on SCL sites are to be stored with the source assembly fully retracted and locked in the shielded position. The industrial gauge must be securely fixed to the installation site.

Note: If a gauge cannot be stored in the above manner, the placement of the gauge into the storage

area is to be closely supervised by a Radiation Safety Officer to ensure that the radiation beam is properly attenuated, and checked with a radiation survey meter.

Access Control Where practicable, industrial radiography activities are to be undertaken during agreed designated meal breaks, or outside of normal hours of work to minimise the risk of radiation exposure to personnel.

Access points to the area in which industrial radiography activities are to be undertaken, and other potential exposure areas, are to be adequately cordoned off to prevent unauthorised access.

Access to the area is to be controlled by a SCL Radiation Safety Officer.

Note: Refer also to the Barricading and Restricted Access Corporate Standard – HB# 659518 and site

specific requirements where relevant for further information regarding barricading requirements. Equipment to be used to restrict access and to warn personnel that industrial radiography is being undertaken on site is to include: appropriate signage; blue radiation tape stating “Caution Radiation”; and blue flashing lights.

Note: Refer also to AS 1319-1994 – Safety Signs for the Occupational Environment for further

information regarding safety sign requirements. Appropriate signage and blue flashing lights are also to be erected at the main entrance to the site to warn personnel that industrial radiography is being undertaken on site.

Once the controlled area has been established, the Radiation Safety Officer is to perform a sweep of the area(s) to ensure that it is clear of all personnel. Prior to commencing, an industrial radiography warning is to be given over the radio (where applicable), clearly stating - “Radiography in the location is commencing. There will be no access to the area”.

Once radiography activities are completed, notice is to be given over the radio stating - “General announcement - Radiography is complete”.

Site Specific Management of Radiation Acquisition, Supply Relocation and Disposal of Radioactive Substances Approval from the statutory radiation safety authority for the particular state / territory is to be sought and obtained prior to SCL acquiring any radioactive substance.

If a radiation gauge is to be sold, lent, exchanged or hired to another person / entity, SCL is to ensure that the proposed new owner / user of the equipment has: the relevant licence to possess industrial gauges (possession licensee); and an approval to acquire the radioactive substance.

Note: A possession licensee who, under the licence, possesses a radiation source for a radiation practise does not supply the source to another person merely because the other person is being allowed by the licensee

Prior to the relocation of radioactive substances outside of a particular state / territory, approval is to be obtained from the statutory radiation safety authority for the particular state / territory for which the radioactive substance is being removed.

Note: A person must not relocate a radiation source without approval. For further details refer to Queensland Radiation Safety Act 1999 S25.

For Queensland, the Chief Executive - Radiation Health is to be notified within seven (7) days after the relocation happens. Disposal or radioactive materials must not occur unless; the concentration or activity of a radionuclide in the material is not more than the maximum

concentration or activity prescribed under the regulation; or the person is the holder of an approval to dispose of the material and disposes of it as required

under the approval Disposal of a radiation apparatus; a person who disposes of a radiation apparatus must give the Chief Executive written notice of the

disposal within 7 days after the disposal the notice must contain information adequate to identify the apparatus disposed of i.e. serial

number Prior to disposal of a radiation apparatus, the apparatus is to be rendered permanently inoperable. Note: A person must not abandon a radiation source. Note: For relocation / disposal notification requirements for other states / territories, refer to the relevant radiation safety regulation for the particular state / territory.

Radiation Safety and Protection Plan A Radiation Safety and Protection Plan is to be developed for each SCL site where radioactive substances are to be used and / or stored.

Note: Refer to Attachment 2, for a Radiation Safety and Protection Plan template. This template is

provided as a guide only and is to be developed in accordance with this Corporate Standard and any other specific statutory requirements for the particular state / territory.

The radiation safety and protection plan must state the following; particulars and an assessment of all the radiation hazards specific to the practise and source the

licensee knows. the radiation safety and protection measures to deal with the hazard any other measures necessary to deal with the hazard how the licensee proposes to monitor and review the implementation and effectiveness of the

measures the functions of the radiation safety officer as listed in Section 37 of the Queensland Radiation

Safety Act 1999 Reprint 4C particulars of training program for persons carrying out the practise other particulars under the regulation also if the operator may receive from carrying out the practise a radiation dose higher than

prescribed under the regulation the supply of a personal monitoring device assessment of the device

the plan must be written in a way likely to be understood by persons carrying out the practise i.e. non-English speaking

the plan must be signed and dated by the licensee

Once drafted, the Radiation Safety and Protection Plan is to be forwarded to the statutory radiation safety authority for the particular state / territory for written endorsement / approval.

Once endorsed / approved, the Radiation Safety and Protection Plan is to be dated and signed off by the Licensee for the SCL site, and the Plan is to be made readily available to all relevant personnel on site.

Radiation Monitoring Under the Queensland Radiation Safety Act 1999, and the Radiation Regulation 2010, personal monitoring devices are to be provided to a person if the person may receive an annual radiation dose greater than 1mSv.

Note: For other state / territory requirements, refer to the radiation safety legislation for the particular

state / territory.

Personal monitoring devices are to be provided for and worn by Radiation Safety Officers when isolating gauges, setting boundaries for radiography, and for all other activities involving work where a person may be exposed to a radioactive substance.

Requirements for the possession licensee to supply the monitor to persons who may be exposed are; details of the persons who are required to use the monitor, described by reference to the nature of their

work details of how and when the monitors are to be used details of the monitors having the sensitivity accuracy, range and energy response appropriate to the

details of where the device is to be stored when not being worn details of the person who will be engaged to check the monitor and their qualifications

All personal monitoring devices to be used on SCL sites are to be of a type assessed and approved by ARPANSA.

At the end of each working period, personal monitoring devices are to be stored away from all radiation sources.

Personal monitoring devices are not to be worn if the monitored person is undergoing a radiographic examination.

Personal monitoring devices are to be assessed by a competent person (e.g. Radiation Safety Officer) every six (6) months (minimum)

As soon as practicable following the assessment of a personal monitoring device, SCL is to: arrange for a copy of the results to be sent to the relevant statutory radiation safety authority for the

particular state / territory; advise all monitored persons of their radiation assessment results; keep the personal monitoring records for the duration of the wearer’s working life and for not less

than 30 years after the last exposure assessment, and at least until the person has reached the age of 75 years;

If the person stops being a person who is required to wear a monitoring device, they may apply in writing for those records and the licensee must as soon as practicable give the person a copy

Personal monitoring records are to be checked by a Radiation Safety Officer to ensure radiation doses are below the prescribed limits as specified in the relevant radiation safety legislation for the particular state / territory, and that doses are as low as reasonably achievable. Note: If any unusual doses are received, the work practices of the wearer are to be investigated and, if

necessary, appropriate remedial action taken.

Personal alarm dosimeters are to be used where monitoring of relevant personnel is required (i.e. when boundaries have been set during radiography activities).

Personal alarm dosimeters are to be checked every twelve (12) months (minimum) for sensitivity, accuracy, range and energy response by the statutory radiation safety authority for the particular state / territory.

A personal alarm dosimeter is not to be used after it has been repaired, or suspected to have been damaged, until it is checked and deemed safe by the statutory radiation safety authority for the particular state / territory.

Radiation surveys are to be undertaken during all industrial radiation activities, and whilst work is being carried out on industrial gauges, to ensure that radiation levels are kept at acceptable levels.

Radiation monitoring devices (i.e. radiation survey meters) are to: have a dose rate range of at least between 1µSv/h and 1000µSv/h, or equivalent for the radiation

emitted from the radioactive substance; have an appropriate energy response; have a measurement uncertainty not greater than ±25% over the energy range of the radiation

emitted from the radioactive substance; and

continue to indicate, either visibly or audibly, when radiation levels exceed the maximum readings in their measurement range.

Note: The above requirements are based on Queensland requirements only, thus reference should be

made to the radiation safety legislation for a particular state / territory to check for any other state / territory specific requirements.

Radiation survey meters are not to be tampered with or misused by any personnel. Prior to use, the radiation survey meter is to be subjected to a battery test, and a test to ensure it responds to radiation.

Note: This is done by checking the strength of radiation being emitted from a gauge or radiography

camera against the reading of another meter. Note: Some gauges incorporate a neutron source. As the survey meter may only detect gamma

radiation, there is a need to be aware that the radiation levels detected may only provide an indication of the total amount of radiation being emitted. A conversion factor may assist in the estimation of actual dose. The supplier of the industrial gauge may be able to assist in this regard.

Personal Protective Equipment SCL Radiation Safety Officers, and other relevant personnel, are to be provided with suitable personal protective equipment, including, as a minimum: portable alarm dosimeters, and film badges.

The worker must use the equipment in accordance with and information, training or instruction in the use of the equipment. The worker must not intentionally misuse or damage any of the equipment and must notify the Radiation Safety Officer if damage or a defect is noticed.

Note: A requirement of the possession licensee is to detail; they types of equipment to be supplied

described by reference to the nature of their work. How and when the equipment is to be worn. The intervals at which the equipment is to be checked for wear and tear, and correct operation. The person who will check the equipment and details of the persons who are required to wear the equipment.

Safety Checks The following checks / tests are to be performed by, or under the direction of a SCL Radiation Safety Officer.

As part of a preventative maintenance program, all SCL owned industrial gauges, and associated equipment, are to be checked to ensure they are free from build-up of materials (e.g. slurry), and that all signage is clean and legible.

On an annual basis, the following preventative maintenance and checks are to be carried out on all SCL owned industrial gauges (as a minimum): shutters are to be operated and locked to check for isolation ability; approximate exposure levels at a 1 meter distance from all gauges are to be checked and recorded

with the shutter both open and closed; gauges are to be inspected for corrosion; gauge mountings are to be checked to ensure they are secure;

the location of all gauges is to be verified and all features of the installation are to be checked to ensure they are sound (e.g. all guards and interlocks are working effectively).

Source leakage testing is to be undertaken: as a precautionary measure prior to service maintenance (particularly for source changes or

shutter maintenance); on industrial gauges following an incident; and every six (6) months after a sealed radioactive substance reaches the end of its recommended

working life, as specified by the manufacturer.

Source leakage tests are to be undertaken in accordance with Annex A.3 of ISO9978-1992 – Radiation protection – Sealed radioactive sources – Leakage test methods.

Repair and Maintenance All maintenance and repair work not intended by the manufacturer to be undertaken by the owner (i.e. SCL), is to be conducted by a competent person qualified to perform such work. This person is to hold a current licence to use radioactive substances in industrial gauges during maintenance, repair or commissioning.

Note: A Radiation Safety Officer is to verify (through sitting), that the person engaged to perform

repair and maintenance work holds the relevant licence.

Modifications to industrial gauges are not to be undertaken without the prior approval of the statutory radiation safety authority for the particular state / territory. Following modification to an industrial gauge, the gauge is not to be used until it has been certified by an accredited person as complying with NM009: 2010 – Standard for sealed radioactive substances incorporated in sealed source apparatus used to carry out industrial gauging. A calibration check of all SCL owned radiation survey meters is to be performed once every twelve (12) months, and / or following suspected damage or repair.

Note: The survey meter is also to be calibrated if the calibration check yields erroneous results. Calibration checks are only to be undertaken by the statutory radiation safety authority for the particular state / territory. Records of all safety checks, maintenance and any actions taken are to be kept in specific equipment logbooks.

Notification of Incidents Notification of incidents involving a radioactive substance is to be undertaken in accordance with Attachment 3 - State / Territory Notification Requirements.

Contractor Management Where contractors are procured to undertake specific on-site work involving the use of industrial gauges and / or industrial radiography etc., the contractor is to provide request and review the following information: a Radiation Safety and Protection Plan for any work involving radioactive apparatus / substances, request training and competency evidence of all those contracted workers who will be involved in

the work, and details of a competent person who shall be directly involved in the supervision of the work.

SCL is to ensure (through sitting) that all contractors procured to undertake work involving the use of industrial gauges and / or industrial radiography etc. on a SCL site, hold the relevant radiation safety qualification(s) / licence(s), issued by the statutory radiation safety authority for the particular state / territory (e.g. Industrial Radiography Operator Licence).

Where relevant, SCL is to provide contractors with all relevant information relating to on site radiation hazards, including SCL safe work procedures / standards relating to radiation safety.

SCL Radiation Safety Officers are to monitor the contractor’s methods of work and the implementation of the proposed controls to ensure that SCL standards for managing work activities involving radioactive apparatus / substances is achieved.

Site Specific Records The following records (as a minimum) are to be maintained by all SCL sites where radioactive substances are used / stored: Possession Licence issued by the statutory radiation safety authority for the particular state /

territory; the Radiation Safety and Protection Plan, approved by the statutory radiation safety authority for

the particular state / territory; approvals to acquire radioactive substances; reports produced by Radiation Safety Officers; equipment maintenance reports; results of all safety checks performed; information / records regarding personal monitoring; inventory and location of industrial gauges; assessment reports for gauges and premises; results of source leakage tests; calibration check certificates for radiation survey meters; radioactive materials store log; and incident reports.

Training & Competency All SCL Radiation Safety Officers are to be deemed competent, and are to hold a current approved radiation safety qualification / licence, issued by the statutory radiation safety authority for the particular state / territory.

All contractors procured to undertake industrial radiography activities on site are to hold the relevant licence (i.e. Industrial Radiography Operator Licence), issued by the radiation safety authority for the particular state / territory.

All personnel involved in the maintenance and repair of industrial gauges, are to hold a current approved radiation safety qualification / licence, issued by the statutory radiation safety authority for the particular state / territory.

Radiation Safety Officers and their Assistants, required to work in the vicinity of radioactive apparatus / substances etc, are to receive radiation safety training. All persons working on a SCL site where industrial gauges are used, and / or industrial radiography is undertaken, are to be notified through the site induction process, that radiation hazards may exist on site, and are to be given information regarding the types of signage and warnings used.

Dangerous event the source is, or appears to have been, lost or stolen;

there is a radiation incident in relation to the source, for which there are no remediation procedures stated in the licensee’s approved radiation safety and protection plan;

equipment that use, measures or controls radiation emitted from the source malfunctions with the result, or likely result, that there is, or will be, an unintended emission of the radiation or a person is, or will be, unintentionally exposed to the radiation.

if a personal monitoring device has recorded in respect of a person a dose equivalent exceeding 2,000 microsieverts in a period of 1 month.

Emergency Procedure – Industrial Gauges (Example Only)

1. Immediately take action to protect human life, to limit injury and to provide first aid if necessary. 2. Allay panic. 3. Cordon off the area and prevent unauthorised and unnecessary access to the secured area. 4. Contact a Radiation Safety Officer. 5. Do not attempt to move or interfere with the radiation source unless directed by the Radiation

Safety Officer. 6. Remain at an appropriate distance (i.e. as far away as possible) from the source to minimise

exposure. 7. The Radiation Safety Officer is to conduct a radiation survey around the industrial gauge and

compare the results with previous monitoring results. 8. If the measurements differ significantly from the established values, or the radioactive

substance cannot be returned to the shielded position, the Radiation Safety officer is to determine the course of action to be taken to render the situation safe (e.g. placing additional shielding over the gauge). Pending advice from the Radiation Safety officer, access control is to be maintained.

9. The Radiation Safety Officer is to immediately notify the possession licensee and the statutory radiation safety authority for the particular state / territory.

10. The gauge involved in the incident is not to be used again until the Radiation Safety Officer confirms the safety of the gauge by obtaining a Certificate of Compliance for the gauge from an accredited person.
