CONDITIONS OF PRE-QUALIFICATION: 1. The Ministry of Housing reserves the right to request the production of any evidence in support

of all details entered hereunder.

2. The Applicant is to complete the form attached with the following details and return it to the address shown below or as otherwise advised.

3. Where insufficient space is available, expand the form or attach documents referencing them

individually within the form.

4. The Applicant shall attach the following along with his application:

4.1. Provide details of the Applicant’s organisation including:

Details of Local Agent, Partner or Joint Venture in Bahrain;

Copy of current valid Commercial Registration;

A statement from Ministry of Labour confirming the compliance of approved percentage of Bahranisation in the workforce;

Copy of company ownership agreement;

Copies of ISO/Other QA Certifications; and

Copy of Company’s Safety Policy. 4.2. Provide financial capacity information including:

Bankers reference(s) - Certificate from Bank to be attached;

Audited Annual Financial Statement for the last two years; and

GOSI Statement of account for the last 2 months. 4.3. Provide technical capacity information including:

Curriculum Vitae for all management staff and engineers including copies of degree/diploma certificates;

Photocopies of Organising Engineering Professional Practice Cards for all Engineers;

Company’s organization chart;

Client/Consultant's reference for the three largest projects completed within the last three years along with copies of Contract Agreement and Completion Certificates; and

Photocopies of registration documents for all plants and equipments.

5. The Applicant shall submit all the above details in one original hardcopy and one softcopy in a CD (in pdf. format) within 14 days from the date of issue of pre-qualification form. Failure to submit any of the above or to complete the attached form IN FULL within the above period will result in the application being returned unprocessed and the applicant may then be excluded from all Pre-Qualification Lists.
