I hope every-one is enjoy-ing the last few weeks of sum-mer. Our first day of school is Tuesday, Sep-tember 8, 2015 and the staff at Fillmore is excited to wel-

come all of our students back to school on opening day. Communication is the key to getting off to a good start. At Fillmore School we want to make sure our parents and students receive the information they need to start the school year off right. At the end of last year students were provided with supply lists. If you need an additional copy of the supply list for your student’s grade level you can retrieve it on the right hand side of our school home page by going to www.fillmorecsd.org or stop by the school to pick up a copy at any one of our offices. Parents of 7th grade students will also want to attend our new 7th grade orientation program on Monday, August 31 at 6pm in the auditorium. When students attend school they will also receive a copy of the handbook and code of conduct. You may want to put both of these in a safe place to refer to as needed when you have a ques-tion. Our handbooks and code of conduct are also located on our home page of our web site under the heading students. Click on students and handbook to view both handbooks for elementary and sec-ondary levels. Over the next few weeks, Fillmore parents will receive a packet of information that includes school forms that need to be completed. We appreci-ate your time and effort to complete and return these forms. Throughout the school year, please be sure to contact your student’s teacher or principal if you have any concerns. We want to hear from you in order to make sure your child has the best experience possible. The main number to contact the school is 585-567-2251. Establishing a team at the seventh and eighth grade level is another way Fillmore School is hoping to increase communica-tion with parents and students. Teaming is when you have a group of core (math,

science, social studies, English and special education) teachers who partner together to teach the same group of students. These teachers meet during team planning to dis-cuss and plan together to meet the varying social, emotional, academic and intellec-tual needs of all students. Teaming will lead to increased support for students and communication with parents. Parents will have the opportunity to schedule a confer-ence and meet with the entire team when it is needed as opposed to only individual teachers. Currently, Fillmore School has a team at grades 5/6 and this year we will add a 7/8 team. I am excited about the many ways teaming will positively impact the educational experience of our students at Fillmore. Fillmore School is also hoping to increase the number of students who are eating a healthy breakfast every day. Last year we increased the number of secondary students eating breakfast each day from around a dozen to nearly 70 students eating breakfast every day. This year we hope to increase the number of elementary students eating breakfast as well. Did you know that eating breakfast improves concentration and performance in the classroom. It provides students with the strength and endurance to engage in physical activity. A complete breakfast can be purchased at Fillmore for $1. Stu-dents who receive free or reduced lunch also receive a free or reduced breakfast as well. If you are an elementary student you can decide to get breakfast at school and eat it as a snack during snack time. In closing, I want to thank our ad-ministrators and teachers for participating in staff development activities that will improve the instructional program we provide to our students. I also want to thank our administrators, administrative assistants, transportation staff, and most importantly the custodial and cleaning staff for working diligently to get us ready for the first day of school. WHY MUSIC?Research has shown: • Students who engage in a musical instrument from ages 6-18 have greater attention skill, anxiety management and emotional control. • Musical training has proven accelerated cortical organization. • Music can serve as a powerful treatment of cognitive disorders such as ADD and ADHD. • Music training assists in read-ing development, reading compre-hension and verbal abilities as well as the ability to solve complex math problems. • In these ways, music helps to close achievement gaps.

It is hoped the following informa-tion will be helpful in the transition back to school.

FIRST DAY OF SCHOOL The first day of school for all stu-dents Pre K-12 will be TUESDAY, Sep-tember 8, 2015. All students Pre K-12 should be in their classrooms no later than 8:10 am. The buses will arrive at school by 8:00 am. September 8th will be a full day of school with dismissal at 3:15 pm. Breakfast and lunch will be served in the cafeteria starting on the first day of school. Students in grades 7-12 should report directly to their first period class as noted on the student’s schedule which will be mailed to them. Any secondary student who has not received a schedule should report directly to the Guidance Office. Morning BOCES Career and Technical Education students should report directly to the bus. Students in grades Pre K-6 should report to their assigned grade levels and teachers as noted in this newsletter.

KEEPING RECORDS UP-TO-DATEParents/Guardians: It is essential that we keep our school records of your child(ren) as current as possible. Please report any changes in your address, phone number, or guardianship of your children during the school year to the appropriate office as soon as possible. ALL INFORMA-TION IS CONFIDENTIAL. If any changes have occurred since last school year, please notify either the PK-4 office or the 5-12 office. Also, if you anticipate moving from our district anytime during the school year, please inform the appropriate office (PK-4 or 5-12 office for 5th and 6th graders, and the guidance office for 7-12th graders) as soon as possible as to the date you expect to leave the district. This infor-mation is helpful in preparing transfer documents for your children.

REQUIRED IMMUNIZATIONS FOR SCHOOL ATTENDANCE Regular checkups are a necessity to insure your child’s good health and normal development. Immunizations are an important part of these visits. As a reminder, the following vac-cines are required for your child to at-tend school (See chart on page 7): • 3 doses of diphtheria – contain-ing toxoid (4 doses for New York City Schools) • 3 doses of poliovirus vaccine • 3 doses of hepatitis B (K-12 stu-dents born on or after 1/1/93 and chil-dren on or after 1/1/95 beginning with their enrollment in any school) • 2 or 3 dose series of hepatitis B vaccine is required for all students enter-ing seventh grade • 2 doses of measles vaccine • 1 dose of mumps vaccine • 1 dose of rubella vaccine • 3 doses of haemophilus influenza type b (Hib) conjugate vaccine for all children less than 5 years of age who are enrolled in a daycare, pre-kindergarten or nursery school. For a child who is 15 months or older, it is acceptable to have received a single dose of the vaccine at or after the age of 15 months. • 1 dose of varicella vaccine for all children born on or after 1/1/00 and enrolled in any school. Also for all chil-dren born on or after 1/1/98 and entering kindergarten in September 2015. Proof of these immunizations must be in the form of a certificate by a physician; this proof must be presented before any child will be admitted to school in September. Children who have not completed the immunization program by August 31, 2015 will not be admitted to a kindergarten room until proof of full immunization is presented to the school nurse. Section 2164 of the Public Health

Law addresses students transferring into a school district from another state or country. A school may grant a student entering New York State schools from another state or country permission to attend school for a period not to exceed 30 days provided such student can show a good faith effort to get the necessary certification or other acceptable evidence of immunization. The permission for a “grace” period does NOT apply to stu-dents transferring within New York State. The intent of this law is to increase the immunization levels of children and thereby protect children from serious vaccine-preventable diseases.

NO CHILD LEFT BEHIND The 2002 No Child Left Behind leg-islation requires every district receiving Title I funds to notify parents of their right to request the qualifications of their child’s teacher(s). Fillmore Central School receives Title I funds. Parents/Guardians of students attending school at Fillmore Central have the right to request the following information about their child’s teacher(s): A. whether the teacher has met the State qualification and licensing criteria for the grade levels and subject areas, which the teacher provides instruction; B. whether the teacher is teaching under emergency or other provisional status through which State qualification or licensing criteria have been waived; C. the baccalaureate degree major of the teacher and any other gradu-ate certification or degree held by the teacher, and the field of discipline of the certification or degree; and D. whether the child is provided services by paraprofessionals and, if so, their qualifications. The information will be provided to the requesting parent/guardian within two business days from the District Office.

Join The FillmoreMusic Boosters

Our inaugural year of the Fillmore Music Boosters was a success thanks to many supportive community members. We achieved many of the goals we set out to reach one of which being able to provide two scholarships to two out-standing senior music students. These two young ladies have been committed to music for all of their high school careers. Corrin Marriott received the instrumental music award and Anna Schilke received the vocal award. Congratulations to them both! The Fillmore Music Boosters (FMB) sponsored Summer Music Camp for students in grades K-2. The camp consisted of song, dance, shadow dancing, and musical instrument mak-ing. The week, which was directed by Jessica Chapman, culminated in a family performance! The FMB also sponsored summer lessons in order to keep the cost of summer lessons down for families. We have prepared food for students who participated in the musical during dress rehearsal week. We will continue to work hard pro-viding multiple musical opportunities for students at FCS in conjunction with Jessica Chapman and Adrene Reding. We hope to continue to offer the events as listed above as well as a trip to the Buffalo Philharmonic Orchestra (BPO) and we are hoping to host a musician or musical group at school. A special thanks to our officers, willing to donate their time. PreK-4 Parent-Teacher Conferences

School VisitorsSCHOOL VISITORS We are pleased to have visitors in our school. Many parents and community members enter the building throughout the school day. PLEASE REMEMBER THAT ALL VISITORS ARE TO REPORT TO EITHER THE DISTRICT OFFICE OR THE 5-12 OFFICE UPON ARRIVAL TO REGISTER. Visitor or volunteer tags will be issued to our guests. We ask that you wear the visitor tag while in the building. For the safety, security and convenience of everyone, please contact the teacher in advance to arrange a mutually convenient time to visit. Thanks in advance for your cooperation.

School Closings and Delayed Openings

Parent-Teacher conferences for grades PK - 4 have been scheduled for Monday, November 23 (Noon– 8 pm) and Tuesday, November 24 (8 am - Noon). Length of each

conference will be fifteen (15) minutes. Families having two or more children in grades PK-4 will be given preference as to their scheduled appointments in an attempt to avoid parents having to make

more than one visit to school. If small children must accompany parents during the conferences, provisions will be made for child care.

There may be times when it is necessary to close school or delay the opening of school. Please note the following procedures regarding delayed opening and/or closing of school. ONE HOUR DELAY in school opening (the school day will begin at 9:10 a.m. instead of 8:10 a.m. with the school buses running approximately one hour later). TWO HOUR DELAY in school opening (the school day will begin at 10:10 a.m. instead of 8:10 a.m. with the school buses running approximately two hours later). When it is necessary to CLOSE SCHOOL because of inclement weather or for other reasons, it will be announced over the radio and TV stations listed below. It may be necessary also to close school and send the children home prior to regular dismissal times. If you aren’t home during the day, we should be informed as to where your children should go. Emergency forms were sent home with your children at the start of school. If you have any changes to that information, please call the appropriate office. IT IS IMPORTANT THAT THIS INFORMATION IS KEPT CURRENT. This information is needed for your child’s safety and protection.

2015-16 NYS Immunization Requirements NOTES: Children in a prekindergarten setting should be age-appropriately immunized. The number of doses depends on the schedule recommended by the Advisory Committee for Immunization Practices (ACIP). This schedule reflects the minimum doses that are required for grades kindergarten through 12. Intervals between doses of vaccine should be in accordance with the ACIP-recommended immunization schedule for persons 0 through 18 years of age. Health Office RemindersTdap Vaccine for Children Entering 6th Grade All students enrolled in 6th grade who are 11 years old must have a Tdap vaccine and all students enrolled in 6th grade regardless of age must have 2 Varicella vaccines. Both vaccines can be obtained at your family doctors or the health department. Without both vaccines your child will not be permitted to start 6th grade. This vaccine needs to be obtained by September 8, 2015.

IllnessSome reasons to keep your child home from school: • Complaints of not feeling well, accompanied with a fever or any other symptoms. • Upset stomach-vomiting • Red crusty eyes (pink eye) • Fever of 100 degrees or higher • Unexplained rash • Flu or cold symptoms • Head lice (they can not return to school until they are checked and cleared by the school nurse) FCS has a no nit policy. • Any other contagious illnessIf your child is sent home from school due to illness please do not allow him/her to return to school the next day.

Attendance Please remember when ever your child is absent from school a call will be made to the parent to verify the absence. Please call the health office at 585-567-8584 to explain the absence. A note from home explaining the absence must be brought in to school when your child returns. It is very important we receive the note the day they return to school.

Classroom Physicals Class room physicals will be performed again this year by our school doctor for 2nd, 4th, 7th, and 10th grades. This is a very basic physical. By New York State Education Law, this year we will be adding body mass index or “BMI” status to the class room physical. It is calculated using the student’s height and weight. The BMI helps to determine if the student’s weight is in a healthy range or is too high or too low. We will be reporting to New York State Department of Health information about our students’ weight status groups. Only summary information is sent. No names and no infor-mation about individual students are sent. The information sent to the New York State Department of Health will help health officials develop programs that make it easier for children to be healthier. However, you may choose to have your child’s information excluded from this survey report.

NYS Law & Dental Health Certificates As a part of your child’s requirements for school, a physical examination is required for new students from out of the state, as well as students in Pre- Kindergarten or Kin-dergarten and in grades 2, 4, 7 and 10. A law was enacted that expands health screenings to include the dental health of students in New York State.

When we require that your child have a physical ex-amination, we will be requesting a dental certificate as well. There is a certificate available for you to take to your child’s dentist and once it is completed, it should be returned to the School Nurse as it will be filed in your child’s Cumulative Health Record. New students from out of state, pre kindergarten and kindergarten parents are responsible for the physical exam with your pediatrician as well as the dental health certifi-cates. Students in grades 2, 4, 7 and 10 will have physicals at school with the school physician Dr. Mayhle. The dental health certificates will be the responsibility of the parent. Please call the school’s Health Office if you have any questions or concerns at 585-567-8584.

Going Miles For Healthy Smiles The program is designed to benefit those who are un-insured, who do not have transportation, or have difficulty accessing dental care. The Program: Education and prevention are two of our main goals for our program. Dental health/nutrition education is offered in the classroom setting to all students and staff. The dental health presentation is evaluated by the staff and then shared with our quality assurance committee, school administration, public health directors and the State Department of Health. In 2009, we educated over 21,000 students within our school sites. A thorough medical history and authorization form is sent home with each student. These forms must be completed by a parent or guardian and returned to the school prior to any dental services. All the forms can be found on the FCS website under the Health Office. A free dental screening is offered to each student along with a toothbrush, floss and instructions. The parent/guardian may also choose to have a dental exam by a pediatric dentist that will complete a dental certificate. The parent may also chose to have a dental cleaning, fluoride treatment, and/or dental sealants if recommended for their child. If additional treatment is necessary (ie: restorations dental fillings, extrac-tions, and/or orthodontics) a referral list of dental providers will be sent home and a phone call will be made to the parent/guardian to help the family find a dental home. We are responsible for all billing of services. Payment for services will come from either the patient's insurance company, Child Health Plus, Medicaid, BC/BS Dental In-surance or directly from the patient. A sliding fee discount is also available. If you have any questions about this program please give the school nurse a call. It is a wonderful program and I encourage all that do not have a regular dentist to apply. It is free to most that apply with a sliding fee for those that don’t qualify for the free program. You will not be billed until all procedures are explained to you and the possible charge is explained.

Medication & Treatment Administration Policies

The NY State Education Department regulates the administration of all medications (prescription and over-the-counter, including cough drops) and has detailed regulations in the administration of medications in public and private schools. Our Policies below are based on those mandates. • Parents and physicians are required to attempt to arrange all medication/treatment administration around school hours. Drugs requiring three times a day administration are to be given before school, after school and before bedtime. • All physicians’ orders must be in writing; verbal physician orders are not accepted. • The School Nurse (or trained school personnel under the supervision of the school nurse) must administer all prescriptions and over-the-counter medications. • All medication is administered in the Nurse’s Office. Parents arriving at school to administer medication to their child must do so in the Nurse’s Office. • Narcotic (opiate) medications, such as Vicodin (hydrocodone), Oxycontin (oxycodone), etc. are very potent pain relievers. No child taking these medications is allowed to come to school while taking them. These drugs will not be administered at school, are not to be brought to school, or taken when coming to school, to do so is in direct violation of our school policies. • All medications, whether prescription or over-the-counter, (OTC medications include but are not limited to: Tylenol, Advil, cough drops, Chloraseptic, Anbesol, topical ointments and sun screen) requires a physicians order for school administration as well as parental consent. When a child has multiple medications each medication ordered requires a separate form. Forms are available on the school web site and the school nurse’s office. • The physicians order must include the name of the student, the name of the medication, doses, route, and time to be administered during the school day. • It is the parents/guardians responsibility to be aware of any expiration dates on medications they provide for

school administration and to provide the school with up-to-date medication prior to any expiration dates. • Management plans (diabetic, asthmatic, allergy, etc.) are not acceptable physician orders. Physician order forms are required for all medications and treatments. • All medications, including over-the-counter medications, are to be delivered to the school nurse by a parent. This is state law. Please, DO NOT send any medication to school with the student. • Medications need to be in original prescription bottle(s) or original over-the-counter bottle(s). Pharmacies will provide “school bottles” if requested. The container must be properly labeled with the students name, name of medicine, dose and time to be administered. • All medication is kept in the Nurse’s office. No student is to carry any medications, prescribed or over-the-counter, on their persons, in their backpacks, or lockers. • Children with life threatening issues who must carry emergency medications on their persons are allowed to do so only with physician and parent written authorization. • The school has a nebulizing machine so students with the appropriate physician orders and parent consent may receive these treatments during school. Parents are required to provide the individual tubing, medication reservoirs, and any associated medications. • New medications orders must be provided for each school year. All medication orders expire the last day of school at which time all medications must be picked up by parents/responsible adult. Any medication not retrieved after close of school on the last day of school is destroyed. Discontinued medications must be retrieved by parent/guardian within one week of discontinuation. After seven days medication will be destroyed. • Our school physician provides standing orders for over-the-counter medications for pain relief and fever. The purpose of these medications are to assist children to complete the school day when suffering minor headaches, pain, menstrual cramps, etc. These medications are available to students only with parental written request. The Standing Order Parental Permit forms are issued at the beginning of the school year. • Our school physician will also provide a Non-Patient Specific order for anaphylactic agents (epinephrine, Benadryl), Albuterol Metered Dose Inhalers and liquid Albuterol for use in a nebulizer. This order is for emergencies only.

If your child needs prescription medication during the school year the Prescription Medication Form must be completed and on file in the nurse’s office. This form needs a doctor’s signature as well as the parent’s signature. An OTC Medication Permission Form must be completed and on file in the nurse’s office to administer any OTC medication including acetaminophen, ibuprofen, anti-itch creams, antibiotic creams, throat spray, Tums, sun screen, Solarcaine etc. Without this form your child will not be able to receive any OTC medication from the nurse’s office. If acetaminophen and ibuprofen needs to be given to your child on a regular basis (more than a few times

NO CHARGING POLICY: Please be advised that we now have a “No Charge Policy” in effect in our school cafeteria. Our computerized registers and card system afford the convenience to pre-pay on accounts much like a debit card. Students that get a full or reduced price meal should: prepay/maintain an account balance, or bring money daily. Students who were approved for free or reduced meals for the 2015-16 school year will continue their status for the month of September. In order for the free or reduced meal status to continue, a new application must be received and approved before October 1, 2015. The application for the free and reduced program is included in this newsletter. Please take a moment to complete and return the application even if you are not sure if you will qualify. In reality, the more students we have who are eligible

for free and reduced meals the more the school district could receive in State aid and grant monies. So please send in those applications. Please remember that charging will not be allowed and that all students and parents will be allowed to pre-pay on accounts. If a student does not have money in his/her account or pocket money, the student will be provided toast and juice for breakfast and a sandwich and milk for lunch. These will be provided at no cost to the student for no longer than (10) ten school days. If after (10) ten days a parent continues to send a child to school without either food from home or the means to purchase a meal at school, the school will contact the appropriate agency regarding this neglect. At no time will a student be allowed to go hungry. Your cooperation is greatly appreciated.

Bus Rules• As soon as students are dismissed

from school, they are to go directly to their buses. Do not push or crowd getting on the bus or within the bus.

• Once on board the bus, students are to go to their seats in a quiet and orderly fashion.

• Students are to read, do homework, sit or talk quietly.

• Observe same conduct as in the classroom.

• Be courteous; use no profane language.

• Do not eat or drink on the bus.• Keep the bus clean.• Do not smoke.• Do not be destructive• Do not move to another seat.

Remain in your seat until you get to your stop [home].

• Keep head, hands and feet inside the bus.

Busing Request

• Make room for others to sit in your seat when there is room.

• Put your books in your lap and hold onto them instead of leaving them on the seat if someone needs a seat.

• Don’t throw anything on the bus or out the windows – don’t litter.

• Cooperate with the bus driver.• The bus driver may give other

directions as necessary.• The bus driver is authorized to

assign seats.• Violation of these rules will result

in disciplinary action possibly jeopardizing a student’s privilege of riding the bus.

Baggage on the School Bus The NYS Department of Transporta-tion requires certain regulations concern-ing items, which may be transported on our school buses. We want to comply with these regulations while permitting students to carry necessary items on the

school bus whenever possible. Generally, baggage may be carried on the bus, which can be held comfortably on the student’s lap. Most items that are too big and/or bulky to be carried on the lap may be tied down under the seat or secured by other means. No items may be permitted which protrude into the aisle hampering normal or emergency exiting. Some items, which may NOT be carried on our school buses, are glass containers, flammable products, and aerosol cans, etc. The Department of Transportation indicates that the person that should have responsibility for determining what will be carried on the school bus is the bus driver. Any questions may be directed to your child’s driver or Mr. Rose, Head Bus Driver at 567-2276.

Annual Pesticide Notification [Education Law 409-h] State Law now requires us to notify everyone who occupies the school building of all applications of pesticide chemicals and the district pest control policy. The maintenance department at Fillmore Central School is trained and well aware of the potential hazards of casual or careless use of pesticides. We have a pest control policy that focuses on prevention without chemicals. The Pesticide Notification Law deals with the most hazardous pesticide products and chemicals. In our routine control plan Fillmore Central School does not use any pesticide products in the school buildings that would require notification. Control of insects is achieved through good housekeeping, sanitation and maintenance. However, in the spirit of the regulation we want to inform parents that we do, when necessary, use the following approved products to control ants, cockroaches and other unwanted insects: baits, jells and granular products that contain boric acid and/or disodium octaborate

tetrahydrate. We may also use growth regulators so that pests do not mature and breed if they hatch out from eggs brought into the school on food products, containers, or by persons entering the building. Also, at times it is necessary to use an aerosol product to protect our students and staff from stinging and biting insects. Any application of a pesticide will be done by certified pest control applicators, or in an emergency, by trained staff. Good housekeeping and sanitation practices also achieve primary rodent control. If necessary, only mechanical trapping devices will be used, never poison baits. Weeding will control unwanted vegetation: herbicides will be used only to control poison ivy or other harmful plants. Herbicides, when used, will be done over the summer recess or a long weekend to nullify any exposure. We apply only fertilizer and a mild crabgrass control to our playing fields. In the unlikely event of an

infestation of insects or rodents that threatens the health and safety of our students and staff or could cause significant monetary damage to our building or grounds, then other control methods might have to be used. Such control methods may require the use of pesticides other than those listed above. We are required to give forty-eight hours notice of the use of such products to employees and adjoining property owners. Parents who wish to be informed of the use of such pesticides may request 48 hours notice by completing the form BELOW. All use of pesticides will be closely monitored and if possible, done when the building is unoccupied. This notice is to inform you of the district’s plan to use the least toxic approach to pest control. The district has a comprehensive plan in place to inspect for, monitor, and control pests designed to protect the health and safety of all building occupants.

Dear Parent/Guardian: Children need healthy meals to learn. Fillmore Central School offers healthy meals every school day. Breakfast costs $1.00; lunch costs for Grades 7-12 is $1.70 and Grades K-6 is $1.55. Your children may qualify for free meals or for reduced price meals. Reduced price is $0.25 for breakfast and $0.25 for lunch.

1. DO I NEED TO FILL OUT AN APPLICATION FOR EACH CHILD? No. Complete the application to apply for free or reduced price meals. Use one Free and Reduced Price School Meals Applica-tion for all students in your household. We cannot approve an application that is not complete, so be sure to fill out all required information. How to Apply: To get free or re-duced price meals for your children you may submit an Eligibility Letter for Free Meals received from the NYS Education Department, OR carefully complete one application for your household and return it to the designated office. If you now receive SNAP, Temporary Assistance to Needy Families (TANF) for any children, or par-ticipate in the Food Distribution Program on Indian Reservations (FDPIR), the ap-plication must include the children's names, the household food stamp, TANF or FDPIR case number and the signature of an adult household member. All children should be listed on the same application. If you do not list a food stamp, TANF or FDPIR case number for all the children for whom you are applying, the application must include the names of everyone in the household, the amount of income each household member, and how often it is received and where it comes from. It must include the signature of an adult household member and the last four digits of that adult's social security number, or check the box if the adult does not have a social security number. An application that is not complete cannot be approved. Contact your local Department of Social Services for your food stamp or TANF case number or complete the income portion of the application. Reporting Changes: The benefits that you are approved for at the time of applica-tion are effective for the entire school year. You no longer need to report changes for an increase in income or decrease in household size, or if you no longer receive SNAP. Income Exclusions: The value of any child care provided or arranged, or any amount received as payment for such child care or reimbursement for costs incurred for such care under the Child Care Devel-opment (Block Grant) Fund should not be considered as income for this program. Facts About Free & Reduced Price Meal Application

SNAP/TANF/FDPIR case number: This must be the complete case number supplied to you by the agency including all numbers and letters, for example, E 123456, or whatever combination is used in your county. Refer to a letter you received from your local Department of Social Services for your case number or contact them for your number. All children with the same case number may be listed on the same application. If anyone in your house-hold receives SNAP, all children living in your household are eligible to receive free meals at school.

Direct Certification: If you receive SNAP or TANF, send in the Eligibility Letter from the NYS Education Department instead of com-pleting the application. Make a copy for your records.

Foster Child: A child who is living with a family but who is under the legal care of the welfare agency or court may be listed on your family application. List the child’s “personal use” income. This includes only those funds provided by the agency which are identified for the personal use of the child, such as personal spending allowances, money received by his/her family, or from a job. Funds provided for housing, food and care, medical, and therapeutic needs are not con-sidered income to the foster child. Write “0” if the child has no personal use income.

Household: A group of related or non-related people who are living in one house and share income and expenses.

Adult Family Members: All related and non-related people who are 21 years of age and older living in one house.

Financially Independent: A person is financially independent and a separate eco-nomic unit/household when his or her earnings and expenses are not shared by the family/household.

Gross Income: Is money earned or received by each member of your household before deductions. Examples of deductions are federal tax, State tax, and Social Security deductions.

Gross Income Continued:Examples of gross income are: • Wages, salaries, tips, commissions, or income from

self-employment • Net farm income – gross sales minus expenses only

– not losses • Pensions, annuities, or other retirement income

including Social Security retirement benefits • Unemployment compensation • Welfare payments (does not include value of SNAP) • Public Assistance payments • Adoption assistance • Strike benefits • Supplemental Security Income (SSI) or Social

Security Survivor’s Benefits • Alimony or child support payments • Disability benefits, including workman’s

If you have more than one job, you must list the income from all jobs. If you receive income from more than one source (wage, alimony, child support, etc.), you must list the income from all sources.

Current Income: Your income at the present time before deductions. Only farmers, self-employed workers, migrant workers, and other seasonal employees may use their income for the past 12 months reported from their 1040 Tax Forms.

Income Exclusions: The value of any child care provided or arranged, or any amount received as payment for such child care or reimbursement for costs incurred for such care under the Child Care Development (Block Grant) Fund should not be considered as income for this program.

To apply for free and reduced price meals, submit a Free Meals/Milk Eligibility Letter received from the Office of Temporary and Disability Assistance OR complete only one application for your household using the instructions.. Sign the application and return the application to Priscilla Beardsley. If you have a foster child in your household, you may include them on your application. A separate application is no longer needed. Call the school if you need help: 567-2289. Ensure that all information is provided. Failure to do so may result in denial of benefits for your child or unnecessary delay in approving your application. PART 1 ALL HOUSEHOLDS MUST COMPLETE STUDENT INFORMATION. (1) List a current Food Stamp, TANF or FDPIR (Food Distribution Program on Indian Reservations) case number of anyone living in your household. Do not use the 16-digit number on your benefit card. The case number is provided on your benefit letter.

(2) An adult household member must sign the application in PART 4. SKIP PART 3. Do not list names of household members or income if you list a food stamp case number, TANF or FDPIR number. PART 3 ALL OTHER HOUSEHOLDS MUST COMPLETE THESE PARTS AND ALL OF PART 4.

(1) Write the names of everyone in your household, whether or not they get income. Include yourself, the children you are applying for, all other children, your spouse, grandparents, and other related and unrelated people in your household. Use another piece of paper if you need more space.

(2) Write the amount of current income each household member receives, before taxes or anything else is taken out, and indicate where it came from, such as earnings, welfare, pensions and other income. If the current income was more or less than usual, write that person’s usual income. Specify how often this income amount is received: weekly, every other week (bi-weekly), 2 x per month, monthly. If no income, check the box. The value of any child care provided or arranged, or any amount received as payment for such child care or reimbursement for costs incurred for such care under the Child Care and Development Block Grant, TANF and At Risk Child Care Programs should not be considered as income for this program.

(3) The application must include the last four digits only of the social security number of the adult who signs PART 4 if Part 3 is completed. If the adult does not have a social security number, check the box. If you listed a food stamp, TANF or FDPIR number, a social security number is not needed. OTHER BENEFITS: Your child may be eligible for benefits such as Medicaid or Children’s Health Insurance Program (CHIP). In order to determine if your child is eligible, program officials need information from your free and reduced price meal application. Your written consent is required before any information may be released. Please refer to the attached parent Disclosure Letter and Consent Statement for information about other benefits.

PRIVACY ACT STATEMENT Privacy Act Statement: This explains how we will use the information you give us. The Richard B. Russell National School Lunch Act requires the information on this application. You do not have to give the information, but if you do not, we cannot approve your child for free or reduced price meals. You must include the last four digits of the social security number of the adult household member who signs the application. The last four digits of the social security number are not required when you apply on behalf of a foster child or you list a Food Stamp, Temporary Assistance for Needy Families (TANF) Program or Food Distribution Program on Indian Reservations (FDPIR) case number or other FDPIR identifier for your child or when you indicate that the adult household member signing the application does not have a social security number. We will use your information to determine if your child is eligible for free or reduced price meals, and for administration and enforcement of the lunch and breakfast programs. We MAY share your eligibility information with education, health, and nutrition programs to help them evaluate, fund, or determine benefits for their programs, auditors for program reviews, and law enforcement officials to help them look into violations of program rules.

Cole Crowned State Champion Fillmore Central School finally has a State Champion, and her name is Elsa Cole. Going into the NYSPHSAA State Meet undefeated and ranked 5th, Elsa ran the perfect race and landed at the very top of the podium. Crossing the line in a time of 7:02.71, she bettered her 2000m Steeplechase time by 14 seconds, and broke her 8th Varsity Record of the season. With this win Elsa became the Division 2 State Champ and also placed 6th in Federation (D1 and D2 combined). The previous evening she ran a Fillmore all-time best in the 3000m (10:18.22) and earned a 6th place D2 State medal in that event as well. Elsa wasn't the only Fillmore athlete to earn a chance to compete at States as Tess Schilke made her third return trip in the 1500m and the Girls 4x800m Relay team (Julia Hotchkiss, Carmen Mancuso, Elsa and Tess) took the surprising win at State Qualifiers. Their time of 9:43.05 crushed the record they set at the beginning of the season by almost 30 seconds, and gave them the honor of being the very first relay team to represent Fillmore at States. Tess came into the 1500m ranked 16th and ran a very strong race with a time of 4:45.66. She placed 9th and missed out on a State medal by less than 2 seconds. The 4x800 Relay team ran their second fastest time of the season (9:45.35) and also placed 9th in Division 2. With three rising Juniors and one Freshman in this group, the future of Girls Track and Field at Fillmore should be rather exciting. Congratulations, girls!
