Reactions 688 - 14 Feb 1998 Clozapine interacts with fluvoxamine The addition of fluvoxamine, but not paroxetine, to clozapine treatment regimens resulted in a significant increase in serum concentrations of clozapine and its metabolites, report researchers from Germany. In the study, 20 men and 10 women with schizophrenia received clozapine 2.5–3 mg/kg/day; 16 of these patients then received fluvoxamine 50 mg/day and the other 14 patients received paroxetine 20 mg/day. Seven and 14 days later, recipients of clozapine and fluvoxamine showed an approximate 3-fold increase in their serum concentrations of clozapine, N- desmethylclozapine and clozapine-N-oxide. However, the addition of paroxetine to clozapine, did not result in significant changes in concentrations of clozapine or its metabolites. These effects were probably the result of inhibition by fluvoxamine of the enzymes catalysing the degradation of clozapine and N-desmethylclozapine. Thus, dose alterations will be required in patients treated with clozapine and fluvoxamine, say the researchers. Wetzel H, et al. Clozapine interacts withfluvoxamine
