Search instructions for claims administrators’ or representatives’ offices

General search tips • The below examples are provided to help you search for the assigned uniform name of claims

administrators’ offices or representatives’ offices using the Web search form • The assigned uniform names are necessary for filing forms in EAMS

• You may search by name, address, city, state, or ZIP Code • Use small search terms of about three or four characters in the name, address, and/or city boxes

provided to avoid typing or spelling errors that could cause incorrect search results • Search terms are not case-sensitive, so a names search using the terms “Jones”, “JONES”, and “jones”

are all valid and will all yield rows with “JONES” in the name. Keeping the above bullet in mind, a search of “jon” will also give you the results for “Jones”

• Special characters (dashes/hyphens, apostrophes, etc.) have been removed from names in this database. For instance, in names like “O’Brien”, the apostrophe has been removed. Searching for “obrien” or even “obri” would be appropriate

• In segmented names like “Mac Donald,” the blank space between “Mac” and “Donald” has been removed and the name in the database will be “MACDONALD.” In this case, searching for “macd” would achieve appropriate results. Keep in mind that when you file a form in EAMS the name in the assigned uniform names database is the one you must use

• If your search results in a list that is too long, you may specify criteria in an additional column or columns. For instance, “John” may return so many rows that you can’t review them easily. In that case, adding three characters to the city field, for instance, as an additional search criterion may greatly reduce the number of rows returned and make your job easier. In the example below, the searcher is looking for the name of “Johnson” in the city of “Santa Clara”
