GENERAL NOTES

GENERAL INFORMATION

1. LABOR, MATERIALS, AND METHODS OF CONSTRUCTION SHALL BE IN STRICTACCORDANCE WITH THE MINIMUM ENGINEERING AND CONSTRUCTION STANDARDSADOPTED BY CITY OF CAPE CORAL, THE PLANS, AND CONSTRUCTION SPECIFICATIONS.WHERE CONFLICTS OR OMISSIONS EXIST, CITY OF CAPE CORAL'S STANDARDS SHALLDICTATE. SUBSTITUTIONS AND DEVIATION FROM PLANS AND SPECIFICATIONS SHALLBE PERMITTED ONLY WHEN WRITTEN APPROVAL HAS BEEN ISSUED BY THEENGINEER.

2. DURING CONSTRUCTION, IT SHALL BE THE CONTRACTOR'S RESPONSIBILITY TOPERFORM THE WORK IN ACCORDANCE WITH O.S.H.A. REQUIREMENTS. THECONTRACTOR'S SIGNATURE AFFIXED TO THE CONTRACT AGREEMENT WITH THEOWNER IS CONSIDERED CERTIFICATION OF CONFORMANCE TO SUCHREQUIREMENTS.

3. IT IS THE RESPONSIBILITY OF THE CONTRACTOR TO ENSURE THAT REQUIRED PERMITSARE OBTAINED AND IN HAND BEFORE BEGINNING ANY CONSTRUCTION. NOCONSTRUCTION OF ANY ITEM SHALL BEGIN UNTIL THE CONTRACTOR HAS RECEIVEDALL PLANS AND ANY OTHER DOCUMENTATION FROM ALL OF THE PERMITTING ANDANY OTHER REGULATORY AUTHORITIES. ANY PENALTIES, STOP WORK ORDERS ONADDITIONAL WORK RESULTING FROM THE CONTRACTOR BEING IN VIOLATION OFTHE REQUIREMENTS ABOVE SHALL BE FULLY BORNE BY THE CONTRACTOR.

4. CONSTRUCTION INSPECTION WILL BE PROVIDED BY CITY AND ENGINEER. THECONTRACTOR SHALL NOTIFY THE ENGINEER AT LEAST 48 HOURS PRIOR TOBEGINNING CONSTRUCTION AND AT LEAST 48 HOURS BEFORE REQUIRINGINSPECTION ON EACH AND EVERY PHASE OF WORK. THE CONTRACTOR SHALLNOTIFY THE ENGINEER A MINIMUM OF 48 HOURS NOTICE PRIOR TO SCHEDULEDTESTING. NO FINAL TESTING WILL BE ACCEPTED UNLESS WITNESSED BY THEENGINEER'S REPRESENTATIVE.

5. THE CONTRACTOR SHALL MAINTAIN A CURRENT SET OF CONSTRUCTION PLANS ANDPERMITS ON THE JOB SITE DURING CONSTRUCTION.

6. THE CONTRACTOR IS RESPONSIBLE FOR SECURING ALL REQUIRED AREA FOR SET UPAND EQUIPMENT & MATERIAL STORAGE. THE CONTRACTOR MUST PROVIDE THECITY COPIES OF ANY AGREEMENTS BETWEEN LAND OWNERS FOR THE USE OF THEIRLAND FOR THIS PROJECT.

7. THE CITY HAS ADOPTED THE MANUAL ON UNIFORM TRAFFIC CONTROL DEVICES FORSTREETS AND HIGHWAY (MUTCD). IN ORDER TO ENSURE COMPLIANCE WITH THESEPROCEDURES, THE CONTRACTOR MUST OBTAIN A PUBLIC WORKS DEPARTMENTRIGHT OF WAY USAGE PERMIT FOR ALL WORK TO BE PERFORMED WITHIN THEPUBLIC RIGHT-OF-WAY. IN ADDITION, THE CONTRACTOR MUST SUBMIT AMAINTENANCE OF TRAFFIC (MOT) PLAN WHEN APPLYING FOR THE PERMIT ANDPRIOR TO START OF WORK. THE MOT MUST BE APPROVED BY THE CITY ENGINEER,AND MUST BE IMPLEMENTED BY IMSA CERTIFIED STAFF. REFER TO THE CITY OF CAPECORAL'S PUBLIC WORKS DEPARTMENT ENGINEERING DESIGN STANDARDS FORFURTHER INFORMATION.

8. AREAS DISTURBED DURING CONSTRUCTION ACTIVITIES SHALL BE COMPLETELYRESTORED TO A CONDITION EQUAL TO OR BETTER THAN EXISTING PRIOR TOCONSTRUCTION.

EXISTING UTILITIES

1. CONTACT SUNSHINE STATE ONE CALL OF FLORIDA (SSOCOF) (811 or 1-800-432-4770)FOR ASSISTANCE IN FIELD LOCATING UNDERGROUND UTILITIES AT LEAST 48 HOURS

BEFORE STARTING WORK IN A PROJECT AREA. UTILITIES IDENTIFIED AS POTENTIALLYBEING WITHIN THE PROJECT LIMITS INCLUDE:

· CAPE CORAL UTILITIES COLLECTION DISTRIBUTION DIVISION (239) 242-3400· LEE COUNTY ELECTRIC CO-OPERATIVE (239) 656-2206· CENTURYLINK TELEPHONE (239) 263-6276· COMCAST CABLE (239) 415-4750· FLORIDA POWER & LIGHT (800) 375-2434

2. DISRUPTION OR DAMAGE TO EXISTING UTILITY LINES SHALL BE REPORTED TO THEUTILITY OWNER, CITY OF CAPE CORAL AND ENGINEER IMMEDIATELY.

3. CONTRACTOR SHALL EXCAVATE TEST PITS FOR THE PURPOSE OF PROTECTING ANDCONFIRMING LOCATION AND DEPTH OF EXISTING UTILITIES OR STRUCTURES INADVANCE OF CONSTRUCTION. TEST PITS SHALL BE DUG BY HAND AND BACKFILLEDIMMEDIATELY AFTER THEIR PURPOSE HAS BEEN SATISFIED AND SHALL NOT REMAINOPEN DURING NONWORKING HOURS.

4. LOCATIONS AND ELEVATIONS OF EXISTING UTILITIES EXPOSED DURINGCONSTRUCTION SHALL BE ACCURATELY RECORDED ON THE CONSTRUCTIONDRAWINGS. THE ENGINEER SHALL BE IMMEDIATELY NOTIFIED OF ANY CONFLICTSWITH THE PROPOSED CONSTRUCTION.

CONSTRUCTION SURVEY1. ALL FIELD LAYOUT SURVEYING FOR CONSTRUCTION AND LOCATING EASEMENTS,

ROW, AND PROPERTY LINES FOR THIS PROJECT SHALL BE PROVIDED BY THECONTRACTOR AT THEIR EXPENSE; UNDER THE DIRECTION OF A LICENSEDPROFESSIONAL LAND SURVEYOR.

2. BENCHMARK FOR CONSTRUCTION HAS BEEN PROVIDED ON SHEET C-101.

3. BENCH MARK DATUM IS NATIONAL GEODETIC VERTICAL DATUM OF 1929 (NGVD1929).

4. PROPERTY MARKERS, SURVEY MONUMENTS AND PERMANENT MARKERS SHALL BEPROTECTED BY THE CONTRACTOR. NOTIFY OWNER IF ANY MONUMENTS MIGHT BEDISTURBED. THE CONTRACTOR'S REGISTERED SURVEYOR SHALL REPLACE ANYMARKERS THAT ARE DISTURBED. ONLY AS APPROVED BY THE CITY SURVEYOR.

5. CONTRACTOR SHALL PROVIDE CORRECTED AS-BUILT INFORMATION DISPLAYING THELOCATION OF THE PIPING, CONTROL PANELS, ETC. BASED ON PROFESSIONALSURVEYOR STAKING.

EARTHWORK AND SITEWORK

1. EXCESS SOIL FROM EXCAVATION AND MILLED OR EXCAVATED ASPHALT SHALL BE THEPROPERTY OF THE CONTRACTOR. INCLUDE THE COST FOR DISPOSING OF THESEWASTE MATERIALS IN THE UNIT PRICE OF THE ITEM TO WHICH IT PERTAINS OR ISMOST CLOSELY ASSOCIATED. NO SEPARATE PAYMENT ITEM FOR THESE ACTIVITIESWILL BE PERMISSIBLE.

2. PIPES SHALL HAVE 3 FEET MINIMUM COVER UNLESS OTHERWISE SPECIFIED IN PLANS,CONTRACTOR SHALL TAKE CARE TO PROVIDE PROPER GRADE ELEVATIONS ANDALIGNMENTS.

3. COMPACTED BACKFILL SHALL BE PLACED ALONG SIDE OF AND OVER ALL UTILITIES.

COMPACT UTILITY TRENCHES TO 95% OF THE MODIFIED PROCTOR MAXIMUMDENSITY (AASHTO T - 180).

4. ORGANIC SOILS BELOW UTILITY TRENCHES SHALL BE REMOVED AND REPLACED WITHSUITABLE MATERIAL AND COMPACTED TO NO LESS THAN 98% OF THE MODIFIEDPROCTOR MAXIMUM DENSITY (AASHTO T - 180).

5. THE CONTRACTOR SHALL BE RESPONSIBLE FOR OBTAINING THE NECESSARY TESTINGTO ASSURE THAT THE PROPER COMPACTION HAS BEEN ACHIEVED. THECONTRACTOR SHALL BEAR ALL COSTS ASSOCIATED WITH TESTING AND RETESTING OFTHE AREAS AND SHALL PROVIDE THE OWNER AND THE ENGINEER WITH COPIES OFTHE CERTIFICATION OF COMPACTION FROM THE TESTING COMPANY.

6. THE CONTRACTOR SHALL PROVIDE ALL DEWATERING EQUIPMENT NECESSARY TOKEEP ALL EXCAVATIONS DRY AND SHALL PROVIDE ALL SHORING, SHEETING, ANDBRACING NECESSARY TO PROTECT WORKMEN, ADJACENT STRUCTURES, UTILITIES,EXISTING PAVEMENT, OR TO MINIMIZE TRENCH WIDTH.

7. PROTECT AND MAINTAIN DRAINAGE SYSTEMS. CLEAN DRAINAGE SYSTEMS AFFECTEDBY THE WORK AT THE END OF CONSTRUCTION. FURNISH, INSTALL AND MAINTAINSITE SPECIFIC EROSION AND SEDIMENT CONTROL PROCEDURES TO PROTECT BOTHEXISTING AND CONSTRUCTED FACILITIES, PROPERTY AND WATER BODIES INACCORDANCE WITH THE CONTRACT DOCUMENTS, OWNER'S WATER USE PERMIT,AND APPLICABLE FEDERAL, STATE, AND LOCAL REGULATIONS. OBTAIN NPDES PERMITWHERE REQUIRED AS SPECIFIED IN SECTION 01066 NPDES REQUIREMENTS. KEEP ACOPY OF ALL APPLICABLE PERMITS ONSITE.

8. CONCRETE WORK SHALL BE CONSTRUCTED WITH 3,000 PSI CONCRETE, OR ASOTHERWISE SPECIFIED.
