TriStarHealth.com Cardiac Rehabilitation Cardiac Rehabilitation Services at TriStar Centennial are designed to meet the needs of patients recovering from a heart attack, heart surgery or stable angina. Specifically, risk factors such as high blood pressure, high cholesterol, low functional capacity, obesity, tobacco abuse, and high stress levels are addressed. Our staff will provide your patients with the necessary information and guidance required to make crucial lifestyle changes. We will work with you to help your patients develop healthy living habits based on each patient’s individual needs. Scheduling Upon receiving a referral, our staff will gladly contact the patient, verify insurance coverage, (including precertification if necessary), send an order form to your office, and schedule the patient for his/her first appointment. To refer a patient, call (615) 342-6399. Physician orders may be faxed to (615) 342-6398. Physician orders must include: patient’s name and phone number, complete diagnosis, physician’s signature and the date. If the patient has already been discharged, please indicate the request is for Outpatient Cardiac Rehab. Inpatient Cardiac Rehabilitation Available Monday – Saturday While in the hospital, the cardiac rehabilitation staff meets with your patient prior to discharge to assess the needs of the patient’s recovery program. Staff members work with patients to improve strength, mobility and endurance. An individualized recovery program is developed with an emphasis on risk factor modification. Outpatient Cardiac Rehabilitation Following discharge, patients are seen on an outpatient basis, typically attending three sessions per week consisting of ECG-monitored exercise and continuing education related to CAD risk factor modification. Exercise sessions primarily emphasize aerobic conditioning along with light resistance training. Modalities utilized include treadmills, arm and cycle ergometers, and recumbent cross trainers for aerobic conditioning. Free weights and elastic bands are used for resistance training. Maintenance Program The cardiac rehabilitation maintenance program provides qualified personnel to assist with continued exercise progression and adherence. The goal of this phase is to help your patients maintain good habits and a healthy lifestyle for a lifetime.
