Large Envelope along with the completed Requisition Form, patient’s medication list, and photocopied insurance card(s). Once the items are inside, seal the Large Envelope by exposing the adhesive and pressing it closed.

10.  Place the complete Large Envelope into the UPS Express Pak for return shipment to PinPoint Clinical at the following address:

The UPS shipping package will contain all swab collections for that day.

Testing will begin immediately once the supplies are received in the laboratory.

Performing the Test1.  Fill out all information on the PinPoint Clinical Requisition

Form. Please make a copy of the patient’s insurance card(s), if possible, and attach this to the Requisition Form along with a list of the patient’s current medications. On the Sample Collection Envelope, write the Patient Name, D.O.B and Collection Date in the space provided.

2.  Wash and dry hands thoroughly and use gloves when possible (not supplied). Do not handle the swab heads at any point during the specimen collection process.

3.  Pull open the swab packages from one end being careful not touch the swab heads.

4.  Insert the swabs into the patient’s mouth and rub firmly against the inside of the cheek or underneath the upper and/or lower lips. Rub for one full minute.

5.  Allow the swab heads to air dry. Place the dry swabs into the Sample Collection Envelope swab head first. Seal this envelope by removing the plastic strip to expose the adhesive and pressing the envelope securely closed.

6.  Place one specimen identification number sticker from the PinPoint Clinical Requisition Form onto the Sample Collection Envelope on the space provided.

Your Buccal Swab Collection Kit contains all of the appropriate materials and paperwork for sample collection. Please read and understand the instructions below and collect all supplies before proceeding. Incorrect collection may result in delays in processing or recollection.
