What’s the purpose of the Behavior Plan Review? Psychologists and behavior specialists review individual behavior plans when developed. This information is an important part of the Quality Management System. It’s used as an element in the certification of service providers.
When is it used? This review is used for all individuals who have transitioned from Agnews Developmental Center into the community and who have a Behavior Plan. The Behavior Plan Review should be completed: (1) 60 days after a transition (or sooner) from Agnews (or another home) when the Behavior Plan has been revised based on the new living environment; or (2) when a Behavior Plan is first developed. Does it need to be completely filled out each time? Yes, it’s important that the Psychologist or Behavior Analyst completes the entire form. It can also be used, in part, for follow-up (see Follow-Up section for more information).

In addition to a simple yes/no rating system, there is also space to indicate a need for follow-up by the Psychologist/Behavior Analyst, Nurse, Service Coordinator, QMS Specialist, or Other.

Follow-up can be recommended even if an answer indicates a positive outcome.

On the last page of the Behavior Plan Review, you’ll find space for narrative information about what’s going well, what could be better, and additional remarks. In the electronic version of the review (embedded in the Quality Management Information System), this field will hold 70,000 characters.
Is there anything that you became aware of that is or is not working well? (Note: You can also use this space to describe examples of best practices that are reported or observed.)
Behavior plans are well developed and easy-to-understand. Many staff are new and it will take additional training and practice for them to use the plan consistently.
ADDITIONAL REMARKS, IF ANY:
It’s critical that the Service Coordinator deal with the PRN medicine issue immediately. It’s my recommendation that until an IDT is scheduled, the medicine not be used.

Follow-UpIt’s the Psychologist/Behavior Analyst’s responsibility to make referrals for follow-up activities as noted in the review. Once the review is entered into the Information Integration System, the Quality Management Specialist can develop a report which summarizes needed follow-up and projected dates of completion. The QMS Specialist monitors the status of follow-up activities and ensures their completion. When the Psychologist/Behavior Analyst is completing a follow-up activity, the Behavior Plan Review tool can be used to focus on only those areas (it would not have to be completed in its entirety). This information should also be entered into the QMIS.
