Objectives• To create a permanent Social and Community Care Forum of stakeholders,

mayors, community care decision makers, experts, projects on community care, care associations from clinical and non-clinical settings that either are delivering care or producing policy instruments.

• Outcome• A data base in a website where all participants could upload their practices, discuss

and exchange ideas in a thematic scaled up, framework, in a B2B, B2G, G2G basis. the European Neighborhood and Partnership (ENP) Zones

• A permanent Forum of stakeholders, mayors, community care decision makers, experts, projects on community care, care associations from clinical and non-clinical setting that either are delivering care or producing policy instruments is set up .

• A publication of the data base results on Community Care in a White Paper . .

Activities and timeline• Survey & questionnaire sent to EU regions 'partners – July 2016• delivery of several thematic meetings in the form of webinars to consolidate

the Forum e.g. in social care, long term care, home care, with the sub-categories “best practices in regulatory/ financial/ governance/ business models/etc..” – Sept’ 2016.

• Organize and deliver a Virtual EU Conference on Community Based Chronic Conditions Integrated Care Best Practices across Europe and the European Neighborhood and Partnership (ENP) Zones as a solemn closure event – November 2016

Contribution of B3 members• Replying to the survey questionnaire• Participating to the webinars and the virtual conference • Participate and exchange to the Forum on voluntary

basis
