Next Valley Youth Movement CORNELL JORDAN | guest speaker A veteran in youth ministry for nearly 20 years, he now travels as an evangelist, spreading the Gospel to students and adults all across America and overseas. TravelWe will be traveling in church vans from Pleasant Valley Church to Punderson State Park, approximately 1 hour away in Portage County.

• Depart | Saturday, January 9 at 11:00 am• Return | Sunday, January 10 at 5:00 pm

Punderson. • Total cost is $40 | includes travel, cabins,

food (4 meals + snacks) and recreation.

What to Expect• Tubing (inner tubes provided), Ice Skating

(bring your own skates or rent at your own additional cost), and Swimming.

• Acoustic Worship, Prize Giveaways, and Guest Speaker Cornell Jordan.

• Every year we do a “midnight” run where we tube down hills in the dark. Very cool.

• Bring your snow clothes, swimsuit, towel, and be ready to have the time of your life!

For the above named participant, I consent to his/her participation in the Avalanche Winter Retreat on January 9-10, 2009. I acknowledge that he/she assumes the risk of injury. I realize that Pleasant Valley Evangelical Church intends to take reasonable precautions against accidents; nevertheless, I agree to release and hold harmless Pleasant Valley Evangelical Church, its staff, agents, volunteers and officers, with respect to injuries, claims, damages or demands which may arise out of any accidents which might occur during this outing. This release includes damage or destruction of the participant’s clothing and personal belongings. Further, I agree that the participant shall make restitution for any damages resulting from his/her negligent conduct. I also give Pleasant Valley Evangelical Church and any health care providers receiving referrals to render emergency medical care and treatment to the above named participant in connection with any illness or injury which he/she may encounter while participating.
