Claim Procedures SCHEDULED MAINTENANCE PROGRAM 1 Secure a copy of the customer’s MasterTech Vehicle Service Contract, the contract number, the Vehicle Identification Number, or the customer’s complete name and address, and the name of the selling dealer. 2 Verify the service contract time and mileage limits to ensure the vehicle service contract is still in force, and that the vehicle in for repairs is the covered vehicle. If the contract has expired, please advise the customer accordingly. 3 Review the maintenance records and receipts provided by the customer to confirm that the required vehicle maintenance has been performed. 4 To facilitate prompt claim handling, we require repair costs to be itemized including part numbers and labor times, separated between covered and non-covered repairs. Please include the authorization number and the customer’s signature when faxing the claim for payment. 5 Review the deductible with the customer. Deductibles will vary depending on the coverage selected. 6 Rental reimbursement is authorized according to the terms of the stated rental benefit and will not exceed the rental limits of the contract. See the contract for additional details. When a MasterTech service contract holder reports a mechanical problem that may be covered by the vehicle service contract, follow these procedures: 7 Sublet repairs pay an additional 10% of the repair bill to cover your cost of handling the additional paperwork and shall not exceed $35.00. 8 All claims require pre-approval. Contact the administrator prior to initiating any teardown or repairs. 9 The contract does not cover a mechanical breakdown or failure due to abuse or neglect. The service contract does not cover any failure while under the manufacturer’s warranty or recall, whether the manufacturer honors such warranty or not. Modified or converted vans, and/ or modifications to any other types of vehicles are not covered under this VSC, unless such modifications or conversions are performed at the factory by the vehicle’s manufacturer or by an authorized conversion company and the appropriate surcharge has been paid. See the contract for additional details. 10 Obtain the customer’s authorization for any teardown and/or diagnosis. The customer will be responsible for teardown or diagnostic charges for non-covered repairs. 11 Make an estimate of repair costs prior to calling for authorization. All MasterTech plans require the appropriate rate based on Odometer Mileageand Vehicleage. 2 VEHICLE AGE. Vehicle eligibility is determined by the model year for each term available. January 1st of the model year is used to determine Vehicle age. A model year eligibility range of “Current + 4” means the current model year plus the next consecutive model years. For Example: As of January 1, 2016, current plus 4 is available on 2012 and newer model years. As of January 1, 2017, current plus 4 is available on 2013 and newer model years. 3 CONTRACT SALE DATE. MasterTech service contracts must be purchased at time of Vehiclesale for any Vehiclethat does not have at least one (1) month and one thousand (1,000) miles of the manufacturer’s warranty remaining. Pre-Owned Coverage is not available after the Vehicle has been sold. MasterTech service contracts must have a corresponding odometer statement and bill of sale (buyer’s order, retail installment contract, etc.) verifying the mileage and Vehicle purchase date. 4 MANUFACTURER’S POWERTRAIN WARRANTY. Proof of manufacturer’s powertrain warranty is required. Manufacturer’s powertrain warranty must be transferred to the subsequent owner (if applicable). 5 INELIGIBLE VEHICLES. Any truck, van, or SUV rated more than 1-ton, gray market Vehicles, Vehicles with mechanical or electrical alterations, or modifications, not performed by the manufacturer (includes but is not limited to the use of oversized tires or wheels, installation of header pipes or lift kits, removal of any emission control system parts, performance modifications, etc. Modification/alteration to the suspension is not allowed unless the appropriate Suspension Modification Surchargehas been paid, selected, and underwriting guidelines and Suspension Modification Surcharge criteria have been met.); Vehicles with engines more than ten (10) cylinders, Vehicles not listed on the Vehicle Classification Guide; all rare, limited production, exotic, high performance, or elite Vehicles (including but not limited to, Acura NSX, Aston Martin, Audi R8, Bentley, Bugatti, Chevrolet Camaro ZL1, Chevrolet Corvette ZR1, Ferrari, Ford GT, Hummer H1, Lamborghini, Lotus, Maserati, Maybach, McLaren, Mercedes-Benz SLR McLaren, Mercedes-Benz SLS, MG, Nissan GT-R, Panoz, Prowler, Rolls Royce, Shelby GT500, Viper, Volkswagen Phaeton, cab and chassis, flat beds, limousines, motorhomes, stake beds); and Vehicleswith compressed natural gas engines. 6 INELIGIBLE USAGE. The following Vehicle uses are ineligible for coverage under any circumstances: hauling for hire, racing and competitive driving, shuttle, taxi or limousine service, police or other law enforcement services, municipal/public use, emergency services, security services, Vehicles with snow plows or used for snow plowing, cable or line installation, tow truck/wrecker services, or any Vehiclethat is rented. 7 DEDUCTIBLE. The standard deductible is one hundred dollars ($100), reducing to fifty dollars ($50) at the selling dealer. Alternative deductibles are also available with different pricing. 8 OPTIONAL SUSPENSION MODIFICATION COVERAGE (SURCHARGE REQUIRED). Coverage is provided if the surcharge has been paid and the option has been selected on the Proof of Registration for Suspension Modification (with the following criteria). a. Lift kit manufacturer’s recommendations, including tire size, wheel size, and offset and/or backspacing must be followed. b. Bolt-on lift kits only. No welding or framemodifications. c. Only Vehicles with a maximum of a six (6) inch lift from the original manufacturer’s specifications are eligible for coverage with the Suspension Modification Surcharge. d. Drop kits or Vehicles that have been lowered are ineligible. e. Non-factory lift kit parts will not be covered by the Contract. f. If You pay the Suspension Modification Surcharge, Wewill provide Coveragefor repair or replacement of Covered Parts arising out of the normal use of Your Vehicle. Damage caused by the failure of or improper installation of modified or aftermarket parts will not be covered. The 6, 12, 18, or 24 month interest-free installment plan is perfect for customers buying their vehicle with cash, who have pre-arranged financing, or who are in the service drive and want to purchase the MasterTech Vehicle Protection Program®(VPP) after they have purchased their vehicle. Selling a Service Contract 1. During the sale of a MasterTech VPP via Darwin or F&I Express, select the SPP option for interest-free financing. 2. For e-contract sales, the 1st payment (plus tax) will be due at the time of sale as a down payment, replacing the 10% down payment. SPP WILL COLLECT THE 1ST AND ALL SUBSEQUENT PAYMENTS FOR E-CONTRACT SALES. For paper contract sales, the dealer will collect and keep the 1st payment (plus tax). 3. The customer can pay via a monthly debit to a checking, savings, or credit card. SPP accepts Mastercard, Visa, Discover, and American Express. 4. The customer can e-sign using a mouse or other e-signature devices. 5. If the customer does not e-sign (or if you complete a paper contract sale outside of Darwin, F&I Express, or CDK MenuVantage Platinum), print the completed VSC application and SPP Payment Plan Agreement, have the documents signed by the customer, and send to: American Financial Warranty Corporation P.O. Any truck, van, or SUV rated more than 1-ton, gray market Vehicles, Vehicles with mechanical or electrical alterations, or modifications, not performed by the manufacturer (includes but is not limited to the use of oversized tires or wheels, installation of header pipes or lift kits, removal of any emission control system parts, performance modifications, etc. Modification/alteration to the suspension is not allowed unless the appropriate Suspension Modification Surcharge has been paid, selected, and underwriting guidelines and Suspension Modification Surcharge criteria have been met.); Vehicles with engines more than ten (10) cylinders, Vehicles not listed on the Vehicle Classification Guide; all rare, limited production, exotic, high performance, or elite Vehicles (including but not limited to, Acura NSX, Aston Martin, Audi R8, Bentley, Bugatti, Chevrolet Camaro ZL1, Chevrolet Corvette ZR1, Ferrari, Ford GT, Hummer H1, Lamborghini, Lotus, Maserati, Maybach, McLaren, Mercedes-Benz SLR McLaren, Mercedes-Benz SLS, MG, Nissan GT-R, Panoz, Prowler, Rolls Royce, Shelby GT500, Viper, Volkswagen Phaeton, cab and chassis, flat beds, limousines, motorhomes, stake beds); and Vehicles with compressed natural gas engines.

6 INELIGIBLE USAGE. The following Vehicle uses are ineligible for coverage under any circumstances: hauling for hire, racing and competitive driving, shuttle, taxi or limousine service, police or other law enforcement services, municipal/public use, emergency services, security services, Vehicles with snow plows or used for snow plowing, cable or line installation, tow truck/wrecker services, or any Vehicle that is rented.

7 DEDUCTIBLE. The standard deductible is one hundred dollars ($100), reducing to fifty dollars ($50) at the selling dealer. Alternative deductibles are also available with different pricing.

8 OPTIONAL SUSPENSION MODIFICATION COVERAGE (SURCHARGE REQUIRED). Coverage is provided if the surcharge has been paid and the option has been selected on the Proof of Registration for Suspension Modification (with the following criteria).

a. Lift kit manufacturer’s recommendations, including tire size, wheel size, and offset and/or backspacing must be followed.

b. Bolt-on lift kits only. No welding or frame modifications.

c. Only Vehicles with a maximum of a six (6) inch lift from the original manufacturer’s specifications are eligible for coverage with the Suspension Modification Surcharge.

d. Drop kits or Vehicles that have been lowered are ineligible.

e. Non-factory lift kit parts will not be covered by the Contract.

f. If You pay the Suspension Modification Surcharge, We will provide Coverage for repair or replacement of Covered Parts arising out of the normal use of Your Vehicle. Damage caused by the failure of or improper installation of modified or aftermarket parts will not be covered.

Payment PlansThe MasterTech Payment Plan is an interest-free installment plan available for your customers to finance a vehicle service contract (VSC) independent from the vehicle financing. The 6, 12, 18, or 24 month interest-free installment plan is perfect for customers buying their vehicle with cash, who have pre-arranged financing, or who are in the service drive and want to purchase the MasterTech Vehicle Protection Program® (VPP) after they have purchased their vehicle.

Selling a Service Contract1. During the sale of a MasterTech VPP via Darwin or F&I Express,

select the SPP option for interest-free financing.

2. For e-contract sales, the 1st payment (plus tax) will be due at the time of sale as a down payment, replacing the 10% down payment. SPP WILL COLLECT THE 1ST AND ALL SUBSEQUENT PAYMENTS FOR E-CONTRACT SALES. For paper contract sales, the dealer will collect and keep the 1st payment (plus tax).

3. The customer can pay via a monthly debit to a checking, savings, or credit card. SPP accepts Mastercard, Visa, Discover, and American Express.

4. The customer can e-sign using a mouse or other e-signature devices.

5. If the customer does not e-sign (or if you complete a paper contract sale outside of Darwin, F&I Express, or CDK – MenuVantage Platinum), print the completed VSC application and SPP Payment Plan Agreement, have the documents signed by the customer, and send to:

1 Secure a copy of the customer’s MasterTech Unit Service Contract, the contract number, the VIN/Hull ID Number, or the customer’s complete name and address, and the name of the selling dealer.

2 Verify the service contract term limits to ensure the unit service contract is still in force, and that the unit in for repairs is the covered unit. If the contract has expired, please advise the customer accordingly.

3 Review the maintenance records and receipts provided by the customer to confirm that the required unit maintenance has been performed.

4 To facilitate prompt claim handling, we require repair costs to be itemized including part numbers and labor times, separated between covered and non-covered repairs. Please include the authorization number and the customer’s signature when faxing the claim for payment.

5 Review the deductible with the customer. Deductibles will vary depending on the coverage selected.

6 Rental reimbursement is authorized according to the terms of the stated rental benefit and will not exceed the rental limits of the contract. See the contract for additional details.

When a MasterTech service contract holder reports a mechanical problem that may be covered by the service contract, follow these procedures:

7 Sublet repairs pay an additional 10% of the repair bill to cover your cost of handling the additional paperwork and shall not exceed $35.00.

8 All claims require pre-approval. Contact the administrator prior to initiating any teardown or repairs.

9 The contract does not cover a mechanical breakdown or failure due to abuse or neglect. The service contract does not cover any failure while under the manufacturer’s warranty or recall, whether the manufacturer honors such warranty or not.

10 Obtain the customer’s authorization for any teardown and/or diagnosis. The customer will be responsible for teardown or diagnostic charges for non-covered repairs.

11 Make an estimate of repair costs prior to calling for authorization.
