ABSTRACT

Objective: To investigate the anti-hyperglycemic effects of Plathymenia

reticulata hydroalcoholic extract and related changes in body weight, lipid profile

and the pancreas. Methods: Diabetes was induced in 75 adult male Wistar rats

via oral gavage of 65 mg/Kg of streptozotocin. Rats were allocated to one of 8

groups, as follows: diabetic and control rats treated with water, diabetic and

control rats treated with 100 mg/Kg or 200 mg/Kg of plant extract, and diabetic

and control rats treated with glyburide. Treatment consisted of oral gavage for

30 days. Blood glucose levels and body weight were measured weekly. Animals

were sacrificed and lipid profile and pancreatic tissue samples analyzed.

Statistical analysis consisted of ANOVA, post-hoc Tukey-Kramer, paired

Student’s t and chi-square tests; the level of significance was set at 5%.

Results: Extract gavage at 100 mg/Kg led to a decrease in blood glucose levels

in diabetic rats in the 2nd, 3rd (198.71±65.27 versus 428.00±15.25) and 4th weeks

(253.29±47.37 versus 443.22±42.72), body weight loss (13.22±5.70 versus

109.60±9.95) and lower cholesterol levels (58.75±3.13 versus 80.11±4.01) in

control rats. Extract gavage at 200 mg/Kg led to a decrease in glucose levels on

the 4th week in diabetic rats, body weight loss in the 2nd, 3rd and 4th weeks in

control rats, and lower cholesterol levels in diabetic and control rats. Islet

hyperplasia (p=0.005) and pancreatic duct dilation (p=0.047) were observed in

diabetic and control rats. Conclusion: Plathymenia extract reduces blood

glucose levels in diabetic rats, and body weight in control rats, and promotes

pancreatic islet hyperplasia in diabetic and control rats.

Objetivo: Avaliar o efeito anti-hiperglicêmico do extrato hidroalcoólico de

Plathymenia, alterações no peso, lipídios e efeito sobre o pâncreas. Métodos:

O diabetes foi induzido pela administração de estreptozotocina 65mg/Kg, em

75 ratos Wistar adultos machos, divididos em 8 grupos diferentes: ratos

diabéticos e controle + água, ratos diabéticos e controle + 100mg/Kg e

200mg/kg de extrato, ratos diabéticos e controle + glibenclamida. O tratamento

foi realizado por gavagem (oral) por 30 dias. Glicose e peso semanalmente. Os

animais foram sacrificados e os lipídios e pâncreas foram analisados. A análise

estatística aplicada: ANOVA, post-hoc Tukey-Kramer, teste t de Student

pareado e teste qui-quadrado, com nível de significância de 5%. Resultados:

O extrato 100mg/Kg promove redução nos níveis de glicose sanguínea em

ratos diabéticos na 2a, 3a (198,71±65,27 versus 428,00±15,25) e 4a semanas

(253,29±47,37 versus 443,22±42,72), perda de peso (13,22±5,70 versus

109,60±9,95) e diminuição do colesterol (58,75±3,13 versus. 80,11±4,01) em

ratos controle. Com extrato de 200mg/Kg houve redução dos níveis de glicose

na 4ª semana, nos ratos diabéticos, peso na 2ª, 3ª e 4ª semanas nos ratos

controle e colesterol nos animais, em ambos os diabéticos e controle. Houve

hiperplasia de ilhotas (p=0,005) e dilatação dos ductos pancreáticos (p=0,047)

em ratos diabéticos e controles. Conclusão: O extrato de Plathymenia reduz

os níveis de glicose em ratos diabéticos, peso em ratos controle e promove

hiperplasia de ilhotas pancreáticas em diabéticos e controles.

INTRODUCTION

Diabetes mellitus, one of the most common chronic diseases, affects more than

245 million people worldwide and is the fourth or fifth leading cause of death in

developing countries. The number of cases amounted to 135 million in 1995,

and 415 million in 2015 globally and is estimated to reach 642 million in 2040,

with two thirds of affected individuals living in developing countries.(1) Brazil saw

a 61,8% increase in diabetes cases over the course of 10 years.(2)

Increased survival of diabetic individuals led to higher odds of chronic

disease-related complications resulting from time of exposure to hyperglycemia.

These complications can be highly debilitating to affected individuals, with

reduced life expectancy and quality of life, not to mention the cost burden on the

health system.(3)

Several plant extracts have been shown to lower blood glucose levels in

animals and the great diversity of chemical compound classes suggests a

variety of underlying modes of action. Despite therapeutic potential in some

cases, in others hypoglycemia may result from toxicity, particularly

hepatotoxicity.(4,5)

Plathymenia reticulata Benth, of the Leguminosae family, is a typical

plant of the Cerrado, rich in phenolic compounds such as tannins and

flavonoids. Hydrolysable tannins have the ability to inhibit the development of

insects, fungi and bacteria.(6) Flavonoids are widely distributed in nature and

have important biological effects, including antimicrobial and cardiovascular

activity.(7)

The plant known by the common name vinhático, a tree belonging to the

genus Plathymenia Benth, has been studied and two species described: P.

reticulata Benth (vinhático of the field) and P. foliolosa Benth (vinhático of the

forest).(8) A third species, P. modest (Speg.) Burk., has been reported and

grows in Argentina, according to Rizzini(9). According to Pius Correa et al.,(10) the

scientific name of P. reticulata Benth is "Chrysoxylon vinhático casar". However,

Antezana,(11) refers to P. reticulata Benth as P. foliolosa Benth.

The chemical characteristics of P. reticulata have been associated with

anti-inflammatory activity(6) and two cassane diterpenes have been described,(12)

as well as antimicrobial activity. ThePr5 fraction has been defined as the

condensed tannin-rich fraction (CTPr), and is a good source of natural inhibitors

of the local inflammation induced by components of a snake venom.(13) Inhibition

of 84.7% of staphylococci by hydroalcoholic P. reticulata extract at 0.625 mg/mL

has been reported.(14) Despite the use of this plant extract to treat diabetes in

popular medicine, its hypoglycemic potential has not been scientifically proven.

OBJECTIVE

To evaluate the anti-hyperglycemic effects of hydroalcoholic Plathymenia

extract and related changes in body weight, lipid profile and the pancreas.

METHODS

This experimental study was approved by the Ethics Committee for Animal

Experimentation (CEEA 002/2011). Seventy-five young adult male Wistar rats

weighing between 180 and 220 g obtained from the University vivarium were

used. Animals were housed in temperature controlled environment (22° to

25°C) and received animal feed and water ad libitum. Animals were submitted

to a 10-day adaptation period, then randomly allocated to different experimental

groups.

Induction of diabetes

Animals were submitted to a 24-hour fasting period, then treated with

intraperitoneal injection of 65 mg/Kg of aqueous streptozotocin solution, which

was previously prepared in 10 mmol/L sodium citrate buffer (pH 4.5).(15,16) Blood

glucose level monitoring was started 7 days after diabetes induction; tests were

performed on blood samples collected from the caudal vein using Accu-check

Performa. Diabetes was defined as body weight loss associated with fasting

blood glucose levels higher than 200 mg/dL.

Plant study

Collection and botanical identification:

Exsiccate samples used for botanical identification and secondary stem inner

bark samples used for pharmacological tests were obtained from a Plathymenia

reticulata Benth specimen (family Leguminosae, subfamily Mimosoideae)

collected in September (bloom time) 2006, at the following geographical

location: 15° 47' 16.65 " S and 48° 33' 26.96 " (Google Earth, 2010), of

Edilândia, Municipality of Cocalzinho - GO/Brasilia, Distrito Federal. The

voucher plant specimen was prepared according to standard herborization

methods(17,18) and stored in the Biology Department Herbarium of Faculdade de

Filosofia, Ciências e Letras de Ribeirão Preto da Universidade de São Paulo

(FFCLRP-USP), Department of Biology (SPFR), Ribeirão Preto, São Paulo,

registration number 13120 SPFR, collector name and number M. Groppo 2073.

Plant extract preparation:

Plant extracts were obtained from previously selected and fragmented inner

bark samples using alcohol solution (70ºC, room temperature). The extract was

dried in steam drying then resuspended in distilled water at 100 and 200 mg of

plant/Kg of animal (??)/mL concentration for pharmacological test purposes.

Pharmacognostic control of plant material and plant extract:

Tests were performed according to the Brazilian Pharmacopoeia (4th edition),(19)

and Simões et al.(20) Pharmacognostic control consisted of the following tests:

ash content, acid ash content, moisture content, total volatile and ethanol

extractable substances, lipid and resin content, extract density and pH

determination, plant and plant extract dry matter content, physicochemical and

colorimetric confirmation of chemical groups (saponins, phenols, tannins and

flavonoids) according to references provided.

Treatment of animals

Animals in this study were allocated to one of 8 groups, as follows: diabetic and

non-diabetic animals treated with oral gavage of 100 mg/kg (D100 and C100) or

200 mg/kg (D200 and C200) of hydroalcoholic plant extract, or 600 mcg/Kg of

glyburide (DG and CG), or not treated (CD and CC – oral gavage of water).

Plant extract or water (controls) were given for 30 days.

Blood glucose and body weight measurement

Blood glucose was measured weekly from blood samples collected from the

caudal vein using Accu-check Performa, following overnight fasting of at least

12 hours. Body weight was measured and recorded prior to the above

described procedure.

Animal sacrifice

On the thirtieth and last day of treatment, animals (63) were subjected to a 12-

hour fasting period, then anesthetized with intraperitoneal sodium thiopental

(50 mg/kg). Abdominal and thoracic organs were removed en block and blood

collected via cardiac or inferior vena cava puncture using a 10-ml syringe and

25 x 8 gauge needle. Blood samples were transferred to dry tubes and spun

down at 4,500 rpm for 15 minutes. The serum was then transferred to a second

tube for determination of HDL-cholesterol, total cholesterol and triglyceride

levels using commercial kits (LABTEST). Tests were performed at the

biochemistry section of the Clinical Analysis Laboratory of the Universidade de

Uberaba.

Pathological examination

Thoracic and abdominal organ samples were preserved in 10% formalin and

sent to the Animal Pathology Laboratory of Hospital Veterinário de Uberaba,

State of Minas Gerais, Brazil, for slide preparation. Samples were cut to obtain

fragments of pancreas measuring approximately 1 cm3. Fragments were fixed in

10% formalin and sent to the Histology Laboratory of Universidade de Uberaba.

Samples used for slide preparation were dehydrated in increasing alcohol

concentrations (up to 100%), then bleached in xylene solution and embedded in

paraffin. Sections (6 micrometer) were cut using a microtome. Samples were

rehydrated, stained with hematoxylin-eosin (HE), dehydrated, cleared, covered

with resin and protected by a coverslip.

Statistical Analysis

Data were entered into SPSS 14.0 data base. Results were expressed as

mean±SEM (standard error of the mean). The analyses among the groups were

performed using analysis of variance (ANOVA) of a track and the Tukey-

Kramer multiple comparisons test (post-hoc). Intragroup analyses were

performed using the paired Student's t test. Pathological findings were

submitted to the chi-square or the Fischer test as necessary. The level of

significance was set at = 5%.

RESULTS

As shown in Table 1, there was a significant decrease in blood glucose levels in

diabetic animals treated with 100 mg/Kg hydroalcoholic extract (D100)

compared to non-treated diabetic animals (DC) in the 2nd (DC 374.78±32.77

versus D100 246.22±45.33 mg/dl; p=0.039), 3rd (DC 428.00±15.25 versus D100

198.71±65.27 mg/dl; p=0.005) and 4th (DC 443.22±42.72 versus D100

253.29±47.37 mg/dl; p=0.021) weeks of treatment. Treatment with 200 mg/Kg

of hydroalcoholic extract (D200) also led to a significant reduction of blood

glucose levels in diabetic animals (DC 443.22±42.72 versus D200

201.00±66.97 mg/dl; p=0.013), but only after 4 weeks of treatment. The

average change in blood glucose levels during the experiment (i.e., the

difference between final and baseline glycemia) was significantly lower in the

D200 compared to the control group (- 243.50±92.66 mg/dl and 87.77±64.01

mg/dl respectively; p=0.012) (Table 1).

Variations observed within diabetic animal groups revealed reduction in

blood glucose levels compared to baseline in the first week of treatment in

animals receiving 100 mg/Kg (t=3.254, p=0.010). In animals treated with 200

mg/Kg, blood glucose levels decreased in the second compared to the first

(t=2.609, p=0.048) and in the fourth compared to the third (t=2.815; p=0.067)

week of treatment. Blood glucose levels did not decrease in animals treated

with glyburide or water (controls) (Figure 1).

Data presented in Table 2 show that treatment of non-diabetic rats with

100 mg/Kg hydroalcoholic extract (C100) led to a significant decrease in blood

glucose levels in these animals compared to controls in the 1st (CC 93.60±2.68

Variations observed within non-diabetic animal groups revealed reduction

in blood glucose levels in animals treated with 100 mg/Kg of extract in the first

week compared to baseline (t=2.594, p=0.029) and in the fourth compared to

the third week (t=3.690, p=0.006) (Figure 2).

Effects on body weight gain were also evaluated. Treatment with 100

mg/Kg (D100) or 200 mg/Kg (D200) of hydroalcoholic extract did not promote

significant weight gain in diabetic animals, except in the 2nd week in those

receiving 200 mg/Kg of extract (DC 213.78±11.42 versus D200 165.17±3.04;

p=0.037). There was no significant body weight variation (final minus initial body

weight) in diabetic animals (Table 1). In contrast, natural weight gain was

significantly lower in non-diabetic animals treated with 200 mg/Kg of

hydroalcoholic extract (C200) in the 2nd (CC 273.20±9.21 versus C200

225.17±9.42 g; p=0.018), 3rd (CC 292.60±10.36 versus C200 242.83±7.23 g;

p=0.003) and 4th (CC 310.80±11.00 versus C200 241.60±12.60 g; p<0.001)

week of treatment. Total body weight gain did not differ significantly between

groups treated with 100 or 200 mg/Kg of extract (CC 109.60±9.95 versus C100

13.22±5.70 versus C200 29.00±9.08 g; p<0.001) (Table 2).

Treatment of diabetic animals with 100 mg/kg of extract significantly

reduced total (DC 84.22±7.26 versus D100 62.28±2.93 mg/dl; p=0.053) and

HDL (DC 44.33±4.70 versus D100 19.86±3.33 mg/dl; p=0.002) cholesterol

levels (Table 1). Total (CC 80.11±4.01 versus C100 58.75±3.13 mg/dl; p=0.01)

and HDL (CC 37.60±3.75 versus C100 20.86±1.33 mg/dl; p=0.005) cholesterol

levels decreased in non-diabetic animals (Table 2).

The treatment with 200 mg/Kg of extract led to a significant decrease in

total cholesterol levels in both diabetic (DC 84.22±7.26 versus D200

52.40±5.83 mg/dl; p=0.008) and non-diabetic (CC 80.11±4.01, n=9 versus C200

64.83±2.84 mg/dl; p=0.037) rats, and to lower HDL cholesterol levels (DC

44.33±4.70 versus D200 21.60±4.24 mg/dl; p=0.011) in diabetic rats (Table 2).

Histopathological analysis of the pancreas revealed lack of significant

differences with respect to hemorrhage, hydropic degeneration, hyperemia and

presence of pancreatic cysts. Significant islet hyperplasia (X2=20.384, p=0.005)

(Figure 3) and pancreatic duct dilation (X2=14.232, p=0.047) were observed in

D200 and C100 groups (Table 3).

DISCUSSION

Plathymenia reticulata is a protein- and enzyme-rich plant growing in the

Brazilian cerrado.(21) The antimicrobial activity of the hydroalcoholic extract from

this plant against Gram-positive bacteria (Streptococcus mutans and

Staphylococcus sp.) has been demonstrated in vitro.(14) Crude hydroalcoholic

extracts (hot and cold) have been shown to have anti-inflammatory activity,

decreasing leukocyte migration in carrageenan-induced peritonitis in mice.

Crude hydroalcoholic extracts also induced anti-edematous activity against

carrageenan-induced paw edema, showed antiproliferative activity against lung,

ovary and melanoma tumor cells, and inhibited nociception in acetic acid-

induced pain tests, with a similar profile to indomethacin.(7)

Antifungal activity of the alcoholic extract (made from Brazilian cachaça)

has been recently reported.(22) Also, the hexane, dichloromethane, ethyl acetic

fractions of ethanolic Plathymenia reticulata extract were able to inhibit B.

jararacussu poison toxicity, possibly via a tannin-mediated effect.(23)

Anti-hyperglycemic effects of Plathymenia reticulata extract have not

been reported to date. However, this study introduces a prospective novel

treatment for diabetes mellitus, with potential pancreatic protection, and a new

disease prevention strategy, supported by the ability of the extract to promote

weight loss and islet hyperplasia in non-diabetic animals.

The mode of action of the plant in this study translated into significant

anti-hyperglycemic effects, leading to blood glucose level decrease in diabetic

and non-diabetic rats; this decrease was significant in non-diabetic animals,

albeit with no clinical manifestations (hypoglycemia). Pancreas-protective

effects (i.e., pancreatic islet hyperplasia) were also observed. Hypoglycemic

effects may reflect the ability of the plant extract to promote beta cell

neogenesis, as the presence of islet hyperplasia and pancreatic duct dilation

suggests, or restore beta cell dedifferentiation, a process through which

dedifferentiated cells revert to progenitor-like cells expressing Neurogenin3,

Oct4, Nanog, and L-Myc. Some diabetes medications in the market are able to

promote beta cell neogenesis or restore dedifferentiation.(24,25)

The hypoglycemic effect occurred early in animals treated with

100 mg/kg (on the second week) and late in those treated with 200 mg/kg, and

may have been due to hormesis - a dose-response relationship phenomenon

characterized by low-dose stimulation and high-dose inhibition. This concept

introduces a changing perception that the fundamental nature of the dose

response is neither linear nor threshold, but U-shaped, changing the concept

and conduct of toxicological and risk assessment.(26)

Beta cell dysfunction plays a key role in the onset and progression of

type 2 diabetes. Among acquired factors, lipotoxicity and glucotoxicity may be of

of particular relevance to cell damage. More recently, a predominant role of

inflammation in beta cell dysfunction in type 2 diabetes has been proposed.(27)

Hence, the plant studied may be indicated for both, type 1 and type 2 diabetes

mellitus.

The role of mitochondrial dysfunction on beta cell injury (i.e., beta cell

apoptosis resulting from production of reactive oxygen species (ROS) in

response to metabolic stress) has been reported in type 2 diabetes (28) and

animals with type 2 diabetes (29). Other recent reports based on autopsy

specimens suggested amyloid deposits in pancreatic islets contribute to

decreased beta cell mass in patients with type 2 diabetes,.(30) Animal

experiments in monkeys (nonhuman model primate) also showed correlations

with alpha cell hyperplasia, leading to insulin deficiency, hyperglucagonemia

and insulin resistance.(31)

Body weight loss in non-diabetic rats treated with 200 mg/Kg or 100

mg/Kg suggests additional effects on obesity control. Weight loss may have

reflected reduced food intake or impaired food absorption in response to plant

extract tannins.(23) Studies analyzing food and water intake in these animals are

being conducted to unveil the mechanisms underlying anti-obesity effects.

Similar hormesis effect can observed in body weight variation in non-

diabetic animals, which had lower weight gain when treated with the 100 mg/kg,

followed by treatment with 200 mg/kg.(26) Initial body weight differences between

animals lead to the use of body weight variation (initial weight minus final

weight) as a parameter in this study.

Positive effect on total cholesterol levels were observed; however, HDL

cholesterol levels decreased. HDL cholesterol carries cholesterol to the liver,

where it is picked up by SR-B1 receptors, contributing to vascular bed

protection against atherogenesis via mechanisms such as removal of oxidized

LDL, inhibition of adhesion molecule and monocyte fixation to the endothelium

and stimulation of nitric oxide release.(32)

Lower total cholesterol levels are beneficial for the population at large

and for diabetic individuals in particular. However, HDL cholesterol reduction

may increase cardiovascular risks, an undesirable effect for diabetics.

The experimental model of diabetes induced by high streptozotocin

doses is a useful tool for investigation of several aspects of type 1 diabetes.(33)

In this study, oral gavage of hydroalcoholic Plathymenia reticulata Benth extract

led to a significant drop in blood glucose levels and promoted islet hyperplasia

in treated animals. These findings may be the first step towards development a

novel oral drug for treatment of type 1 and possibly to type 2 diabetes.

CONCLUSION

Further studies are warranted to unveil the mechanisms underlying extract

effects, investigate extract toxicity and single out the compound responsible for

the effects described. This study may raise prospects of new therapies for type

1 or type 2 diabetes, based on hypoglycemic effects and beta cell preservation

or, more importantly, lead to the identification of a compound capable of

preventing the onset of type 2 diabetes in susceptible individuals via pancreatic

protection and weight loss. Beneficial effects described may reflect the ability of

the plant extract to promote beta cell neogenesis or restore beta cell

dedifferentiation.

Objective: To investigate the anti-hyperglycemic effects of Plathymenia

reticulata hydroalcoholic extract and related changes in body weight, lipid profile

and the pancreas. Methods: Diabetes was induced in 75 adult male Wistar rats

via oral gavage of 65 mg/Kg of streptozotocin. Rats were allocated to one of 8

groups, as follows: diabetic and control rats treated with water, diabetic and

control rats treated with 100 mg/Kg or 200 mg/Kg of plant extract, and diabetic

and control rats treated with glyburide. Treatment consisted of oral gavage for

30 days. Blood glucose levels and body weight were measured weekly. Animals

were sacrificed and lipid profile and pancreatic tissue samples analyzed.

Statistical analysis consisted of ANOVA, post-hoc Tukey-Kramer, paired

Student’s t and chi-square tests; the level of significance was set at 5%.

Results: Extract gavage at 100 mg/Kg led to a decrease in blood glucose levels

in diabetic rats in the 2nd, 3rd (198.71±65.27 versus 428.00±15.25) and 4th weeks

(253.29±47.37 versus 443.22±42.72), body weight loss (13.22±5.70 versus

109.60±9.95) and lower cholesterol levels (58.75±3.13 versus 80.11±4.01) in

control rats. Extract gavage at 200 mg/Kg led to a decrease in glucose levels on

the 4th week in diabetic rats, body weight loss in the 2nd, 3rd and 4th weeks in

control rats, and lower cholesterol levels in diabetic and control rats. Islet

hyperplasia (p=0.005) and pancreatic duct dilation (p=0.047) were observed in

diabetic and control rats. Conclusion: Plathymenia extract reduces blood

glucose levels in diabetic rats, and body weight in control rats, and promotes

pancreatic islet hyperplasia in diabetic and control rats.

RESUMO

Objetivo: Avaliar o efeito anti-hiperglicêmico do extrato hidroalcoólico de

Plathymenia, alterações no peso, lipídios e efeito sobre o pâncreas. Métodos:

O diabetes foi induzido pela administração de estreptozotocina 65 mg/Kg, em

75 ratos Wistar adultos machos, divididos em 8 grupos diferentes: ratos

diabéticos e controle + água, ratos diabéticos e controle + 100 mg/Kg ou

200 mg/kg de extrato, ratos diabéticos e controle + gliburida. O tratamento foi

realizado por gavagem (oral) por 30 dias. Glicose e peso semanalmente. Os

animais foram sacrificados e amostras de lipídios e pâncreas foram analisadas.

A análise estatística incluiu ANOVA, post-hoc Tukey-Kramer, teste t de Student

pareado, e teste do qui-quadrado, com nível de significância de 5%.

Resultados: O extrato 100 mg/Kg promove redução nos níveis de glicose

sanguínea em ratos diabéticos na 2a, 3a (198,71±65,27 x 428,00±15,25) e 4a

semanas (253,29±47,37 x 443,22±42,72), perda de peso (13,22±5,70 x

109,60±9,95) e diminuição do colesterol (58,75±3,13 x 80,11±4,01) em ratos

controle. Com extrato de 200 mg/Kg houve redução dos níveis de glicose na 4ª

semana, nos ratos diabéticos, de peso na 2ª, 3ª e 4ª semanas nos ratos

controle, e de colesterol nos animais diabéticos e controle. Houve hiperplasia

de ilhotas (p=0,005) e dilatação dos ductos pancreáticos (p=0,047) em ratos

diabéticos e controles. Conclusão: O extrato de Plathymenia reduz os níveis

de glicose em ratos diabéticos, peso em ratos controle, e promove hiperplasia

de ilhotas pancreáticas em diabéticos e controles.

INTRODUÇÃO

Diabetes mellitus, uma das doenças crônicas mais comuns no mundo, afeta

mais de 245 milhões de pessoas e representa a quarta ou quinta principal

causa de óbito em países em desenvolvimento. O número mundial de casos de

diabetes melittus chegou a 135 milhões em 1995, e 415 milhões em 2015,

devendo atingir 642 milhões em 2040, sendo dois terços dos afetados

residentes de países em desenvolvimento.(1) No Brasil, houve um aumento de

61,8% no número de casos diagnosticados ao longo de 10 anos.(2)

O aumento da sobrevida dos indivíduos diabéticos trouxe um maior risco

de complicações crônicas associadas ao tempo de exposição à hiperglicemia.

Tais complicações podem ser debilitantes para os indivíduos afetados,

comprometendo a longevidade e a qualidade de vida, além de onerar o sistema

de saúde.(3)

Sabe-se que diversos extratos vegetais são capazes de reduzir os níveis

sanguíneos de glicose em animais e a grande diversidade de classes de

compostos químicos sugere o envolvimento de diversos modos de ação.

Apesar do potencial valor terapêutico de algumas dessas substâncias em

alguns casos, em outros a hipoglicemia pode ser um efeito colateral da

toxicidade, principalmente da hepatotoxicidade.(4,5)

Plathymenia reticulata Benth, da família Leguminosae, é uma planta

típica do Cerrado rica em compostos fenólicos, como taninos e flavonoides. Os

taninos hidrolisáveis são capazes de inibir o desenvolvimento de insetos,

fungos e bactérias.(6) Os flavonoides são substâncias abundantes na natureza e

sua importância deriva de efeitos biológicos, como atividade antimicrobiana e

cardiovascular.(7)

A planta popularmente conhecida como vinhático, uma árvore

pertencente ao gênero Plathymenia Benth, foi estudada e duas espécies foram

descritas: P. reticulata Benth (vinhático do campo) e P. foliolosa Benth

(vinhático da floresta).(8) Existe ainda uma terceira espécie, P. modest (Speg.)

Burk., que cresce na Argentina, segundo Rizzini(9). Segundo Pius Correa et al.

(10), o sinônimo científico de P. reticulata Benth é "Chrysoxylon vinhático casar".

Entretanto, para Antezana(11) P. reticulata Benth seria P. foliolosa Benth.

As características químicas de P. reticulata foram associadas à atividade

anti-inflamatória;(6) além disso, dois diterpenos cassânicos foram descritos,(12)

assim como a atividade antimicrobiana. A fração ThePr5 foi definida como a

fração rica em taninos condensados (CTPr), e constitui uma boa fonte de

inibidores naturais da inflamação local induzida por veneno de serpente.(13) A

inibição de 84,7 % dos estafilococos pelo extrato hidroalcoólico de P. reticulata

na concentração de 0,625 mg/mL foi relatada .(14) Apesar do emprego desse

extrato vegetal no tratamento do diabetes na medicina popular, seu potencial

hipoglicemiante não foi comprovado cientificamente.

OBJETIVO

Avaliar o efeito anti-hiperglicêmico do extrato hidroalcoólico de Plathymenia e

alterações de peso corporal, perfil lipídico e pancreáticas relacionadas.

MÉTODOS

Este estudo experimental foi aprovado pelo Comitê de Ética em

Experimentação Animal (CEEA 002/2011). Foram utilizados 75 ratos machos

adultos jovens da linhagem Wistar, pesando entre 180 e 220 g e provenientes

do biotério da Universidade. Os animais foram mantidos em ambiente com

temperatura controlada (22° a 25°C) e receberam ração e água à vontade.

Após um período de adaptação de 10 dias, os animais foram alocados

aleatoriamente em diferentes grupos experimentais.

Indução do diabetes

Após jejum de 24 horas, os animais receberam 65 mg/Kg de uma solução

aquosa de estreptozotocina pela via intraperitoneal, que foi previamente

preparada com 10 mmol/L de tampão de citrato de sódio (pH 4,5).(15,16) O

monitoramento dos níveis glicêmicos teve início 7 dias após a indução do

diabetes, por meio de amostras sanguíneas coletadas da veia caudal com

Accu-check Performa. O diabetes foi definido como a perda de peso associada

à glicemia de jejum superior a 200 mg/dL.

Estudo da Planta

Coleção e identificação botânica:

As amostras empregadas no preparo de exsicatas destinadas à identificação

botânica e das entrecascas do caule secundário destinadas aos testes

farmacológicos foram obtidas de um espécime de Plathymenia reticulata Benth

(família Leguminosae, subfamília Mimosoideae), coletadas no mês de

setembro (mês de floração) de 2006, na seguinte localização geográfica: 15°

47' 16,65 " S e 48° 33' 26.96" (Google Earth, 2010), distrito de Edilândia,

Município de Cocalzinho - GO/Brasília, Distrito Federal. O espécime-

testemunha foi preparado de acordo com métodos padrão de herborização(17,18)

e armazenado no herbário do Departamento de Biologia da Faculdade de

Filosofia, Ciências e Letras de Ribeirão Preto da Universidade de São Paulo

(FFCLRP-USP), departamento de Biologia (SPFR), número de registro 13120

SPFR, coletor e número M. Groppo 2073.(b)

Preparo do extrato vegetal:

Os extratos foram preparados a partir de entrecascas previamente

selecionadas e fragmentadas, por meio de extração com solução alcoólica

(70ºC, temperatura ambiente). O extrato foi seco a vapor e resuspendido em

água destilada nas concentrações de 100 e 200 mg de vegetal/mL para fins de

testes farmacológicos.

Controle farmacognóstico do material vegetal e respectivo extrato:

Os testes foram realizados de acordo com a 4a edição da Farmacopeia

Brasileira(19) e com Simões et al.(20) O controle farmacognóstico consistiu dos

seguintes testes: teor de cinzas, acidez das cinzas, teor de umidade,

substâncias voláteis totais, teor de substâncias passíveis de extração por

etanol, teor de lipídeos e resinas, determinação da densidade e pH dos

extratos, teor de matéria seca do extrato e da planta medicinal e confirmação

da presença de grupos químicos (saponinas, fenois, taninos e flavonoides) por

meio de reações físico-químicas e colorimétricas, segundo descrições

constantes nas referências bibliográficas.

Tratamento dos animais

Os animais utilizados no estudo foram distribuídos em 8 grupos: animais

diabéticos e não-diabéticos tratados com extrato hidroalcoólico na dose de 100

mg/kg (D100 e C100) ou 200 mg/kg (D200 e C200), ou gliburida na dose de

600 mcg/Kg (DG e CG) e animais diabéticos e não-diabéticos não tratados (CD

e CC – administração oral de água). A administração de extrato vegetal ou

água (controles) foi mantida por 30 dias.

Dosagem de glicose sanguínea e aferição do peso corporal

As dosagens de glicose sanguínea foram realizadas semanalmente por meio

de amostras coletadas da veia caudal com Accu-check Performa, após jejum

mínimo de 12 horas. O peso corporal foi aferido e registrado antes do

procedimento descrito.

Sacrifício dos animais

No trigésimo e último dia de tratamento, todos os animais (63) foram

submetidos a jejum de 12 hora e então anestesiados com tiopental sódico

(50 mg/kg) pela via intraperitoneal. Os órgãos abdominais e torácicos foram

removidos em bloco e o sangue coletado por punção cardíaca ou da veia cava

inferior com seringa de 10 ml e agulha calibre 25 x 8. As amostras de sangue

foram transferidas para tubos secos e então centrifugadas a 4.500 rpm por 15

minutos. Após a centrifugação, o soro foi transferido para outro tubo para

dosagem de colesterol HDL, colesterol total e triglicérides por meio de kits

comerciais (LABTEST). Os testes foram realizados no setor de bioquímica do

Laboratório de Análises Clínicas da Universidade de Uberaba.

Anatomopatológico

As amostras de órgãos torácicos e abdominais foram preservadas em

formalina a 10% e enviadas para o Laboratório de Patologia Animal do Hospital

Veterinário de Uberaba, MG, Brasil, para fins de confecção das lâminas. As

amostras foram cortadas para obter fragmentos de pâncreas de

aproximadamente 1 cm3. Após o corte, cada fragmento foi fixado em formalina

a 10% enviado para o Laboratório de Histologia da Universidade de Uberaba.

Para confecção das lâminas, as amostras foram desidratadas em

concentrações crescentes de álcool (até 100%), imersas em solução de xileno

para branqueamento e então colocadas em blocos de parafina. Cortes de 6

micrômetros foram realizados com micrótomo. As amostras foram então

reidratadas, coradas com hematoxilina-eosina (HE), desidratadas, clareadas,

cobertas com resina e protegidas por lamínula para leitura.

Análise estatística

Os dados foram colocados na base de dados do SPSS 14.0. Os resultados

foram expressos como média±EPM (erro padrão da média). As análises entre

os grupos foram baseadas na análise de variância (ANOVA) e no teste (post-

hoc) de comparações múltiplas de Tukey-Kramer. As análises intragrupo foram

realizadas empregando-se o teste t de Student para dados pareados. Os

achados anatomopatológicos foram submetidos ao teste qui-quadrado ou ao

teste de Fischer, se necessário. O nível de significância adotado foi de = 5%.

RESULTADOS

Conforme mostra a Tabela 1, houve redução significante dos níveis glicêmicos

no grupo de animais diabéticos tratados com 100 mg/Kg de extrato

hidroalcoólico (D100) em relação ao grupo animais diabéticos não tratados

(DC) na 2a semana de tratamento (DC, 374,78±32,77 x D100,

246,22±45,33 mg/dl; p=0,039), efeito esse que se manteve na 3a (DC,

428,00±15,25 x D100 198,71±65,27 mg/dl; p=0,005) e na 4a (DC, 443,22±42,72

x D100 253,29±47,37 mg/dl; p=0,021) semanas de tratamento. O tratamento

com 200 mg/Kg de extrato hidroalcoólico (D200) também promoveu redução

significativa nos níveis glicêmicos nos animais diabéticos (DC, 443,22±42,72 x

D200, 201,00±66,97 mg/dl; p=0,013), porém somente após 4 semanas de

tratamento. A alteração média dos níveis sanguíneos de glicose durante o

experimento (ou seja, a diferença entre glicemia final e glicemia inicial) foi

significantemente menor no grupo D200 do que no grupo controle (-

243,50±92,66 mg/dl e 87,77±64,01; p=0,012) (Tabela 1).

A análise da variação intragrupo em animais diabéticos mostra que a

dose de 100 mg/Kg promoveu redução dos níveis glicêmicos em relação aos

níveis iniciais na primeira semana (t=3,254, p=0,010), enquanto a dose de 200

mg/Kg promoveu redução na segunda semana em relação à primeira (t=2,609,

p=0,048) e na quarta em relação à terceira (t=2,815; p=0,067) semana de

tratamento. Não houve redução nos animais tratados com gliburida ou água -

controles (Figura 1).

Os dados da Tabela 2 mostram que o tratamento de ratos não-

diabéticos com 100 mg/Kg de extrato hidroalcoólico (C100) promoveu queda

significante dos níveis glicêmicos nesses animais em relação aos controles na

1a (CC, 93,60±2,68 x C100, 81,20±1,94 mg/dl; p=0,006), 2a (CC, 102,60±2,31 x

C100, 80,80±4,84 mg/dl; p=0,014) e 4a (CC, 93,10±3,61 x C100 74,33±1,31

mg/dl; p=0,001) semanas de tratamento. O tratamento com 200 mg/Kg de

extrato hidroalcoólico (C200) levou a uma redução significante dos níveis

glicêmicos na 4a semana (CC, 93,10±3,61 x C200, 74,83±8,41 mg/dl; p=0,055)

(Tabela 2).

A análise da variação intragrupo em animais não-diabéticos mostra

redução dos níveis sanguíneos de glicose em relação aos valores iniciais na

primeira semana (t=2,594, p=0,029) e entre a quarta e a terceira semanas

(t=3,690, p=0,006) de tratamento com 100 mg/Kg de extrato (Figura 2).

O efeito sobre o ganho de peso dos animais também foi avaliado. O

tratamento com 100 mg/Kg (D100) ou 200 mg/Kg (D200) do extrato

hidroalcoólico não promoveu ganho significativo de peso corporal nos animais

diabéticos, exceto na 2a semana de administração de 200 mg/Kg extrato (DC,

213,78±11,42 x D200, 165,17±3,04; p=0,037). Também não houve variação

significativa de peso (peso final menos peso inicial) nos animais diabéticos

(Tabela 1). Em contrapartida, observou-se que o tratamento de animais não-

diabéticos com 200 mg/Kg de extrato hidroalcoólico (C200) promoveu redução

significativa do ganho natural de peso nesses animais ao longo da 2a (CC,

242,83±7,23 g; p=0,003) e 4ª (CC, 310,80±11,00 x C200 241,60±12,60 g;

p<0,001) semanas de tratamento. Além disso, não houve diferença significativa

de ganho total de peso nos grupos tratados com 100 ou 200 mg/Kg de extrato

(CC, 109,60±9,95 x C100, 13,22±5,70 x C200 29,00±9,08 g; p<0,001) (Tabela

2).

O tratamento de animais diabéticos com 100 mg/kg de extrato promoveu

redução significante do colesterol total (DC, 84,22±7,26 x D100,

62,28±2,93 mg/dl; p=0,053) e colesterol HDL (DC, 44,33±4,70 x D100,

19,86±3,33 mg/dl; p=0,002) (Tabela 1). Nos animais não-diabéticos, houve

queda do colesterol total (CC, 80,11±4,01 x C100, 58,75±3,13 mg/dl; p=0,01) e

do colesterol HDL (CC, 37,60±3.75 x C100, 20,86±1,33 mg/dl; p=0,005)

(Tabela 2).

O tratamento com 200 mg/Kg de extrato levou a uma redução

significativa dos níveis de colesterol total tanto em ratos diabéticos (DC,

84,22±7,26 x D200, 52,40±5,83 mg/dl; p=0,008) quanto em ratos não-

diabéticos (CC, 80,11±4,01, n=9 x C200 64,83±2,84 mg/dl; p=0,037) e também

à queda do colesterol HDL em ratos diabéticos (DC, 44,33±4,70 x D200

21,60±4,24 mg/dl; p=0,011) (Tabela 2).

A análise histopatológica revelou ausência de diferenças significantes

em relação à hemorragia, degeneração hidrópica, hiperemia e presença de

cistos pancreáticos. Observou-se hiperplasia significante das ilhotas

(X2=20,384, p=0,005) (Figura 3) e dilatação significante de ductos pancreáticos

(X2=14,232, p=0,047) nos grupos D200 e C100 (Tabela 3).

DISCUSSÃO

Plathymenia reticulata é uma planta do cerrado brasileiro com altos teores de

proteína e enzimas.(21) Estudos prévios mostraram que o extrato hidroalcoólico

dessa planta possui atividade antimicrobiana contra bactérias Gram-positivas

(Streptococcus mutans e Staphylococcus sp.) in vitro.(14) Os extratos brutos

(quente e frio) mostraram atividade anti-inflamatória, reduzindo a migração

leucocitária na peritonite induzida por carregenina em camundongos. Os

extratos brutos também induziram atividade antiedematogênica, reduzindo o

edema de patas induzido pela carregenina, além de mostrar atividade

antiproliferativa em células tumorais no pulmão, ovário e melanoma e inibição

da nocicepção em testes de algesia induzida por ácido acético, com perfil

similar ao da indometacina.(7)

Relatos recentes mostraram que o extrato hidroalcoólico de Plathymenia

reticulata (à base de cachaça) possui atividade antifúngica (22) e que as frações

hexano, diclorometano e etil-acética do extrato etanólico inibiem o efeito tóxico

do veneno de B. jararacussu, provavelmente por meio de taninos.(23)

Os efeitos anti-hiperglicêmicos do extrato de Plathymenia reticulata

ainda não foram relatados. Entretanto, esse estudo abre a possibilidade de um

novo tratamento para o diabetes mellitus, com possível proteção pancreática,

além de uma nova estratégia preventiva da doença, dada a capacidade do

extrato de induzir perda de peso e hiperplasia de ilhota em animais não-

diabéticos.

A ação farmacológica da planta estudada se traduziu em efeito anti-

hiperglicêmico importante, com queda dos níveis sanguíneos de glicose em

ratos diabéticos e não-diabéticos, sendo que nos animais não-diabéticos a

queda foi significante, embora sem manifestação clínica (hipoglicemia). Além

disso, foi observado efeito protetor sobre o pâncreas (hiperplasia de ilhotas

pancreáticas). O efeito hipoglicemiante pode refletir a capacidade do extrato

vegetal de promover a neogênese de células beta, conforme sugere a

ocorrência de hiperplasia de ilhotas e dilatação de ductos pancreáticos, ou

restaurar a diferenciação de células beta, um processo através do qual as

células dediferenciadas revertem para células progenitor-like que expressam

Neurogenin 3, Oct4, Nanog e L-Myc. Algumas drogas destinadas ao tratamento

do diabetes disponíveis no mercado são capazes de promover a neogênese de

células beta ou restaurar a diferenciação.(24,25)

O efeito hipoglicemiante foi precoce nos animais tratados com

100 mg/kg de peso corporal (na segunda semana) e tardio nos animais

tratados com 200 mg/kg de peso corporal, e pode ter refletido o efeito hormese:

um fenômeno dose-resposta caracterizado pela estimulação por baixas doses

e inibição por altas doses. Esse conceito indica uma nova percepção da

natureza fundamental da relação dose-resposta, que não é linear nem de

limiar, e sim em formato de U, alterando o conceito e a condução da avaliação

toxicológica e de risco.(26)

A disfunção das células beta tem papel fundamental no

desencadeamento e progressão do diabetes tipo 2. Dentre os fatores

adquiridos, a lipotoxicidade e a glicotoxicidade podem ter importância singular

na lesão celular. Mais recentemente, foi proposto que a inflamação tem papel

predominante na disfunção das células beta no diabetes tipo 2.(27) Portanto, a

planta estudada poderia ser indicada tanto para o diabetes mellitus tipo 1

quanto para o tipo 2.

O papel da disfunção mitocondrial na lesão de células beta (apoptose de

células beta em decorrência da produção de espécies reativas de oxigênio –

ROS - mediada por estresse metabólico) foi relatado no diabetes tipo 2 (28) e em

animais com diabetes tipo 2.(29). Outros relatos recentes indicam que a

presença de depósitos de amiloides nas ilhotas pancreáticas, observada em

material coletado em autópsia, contribui para a redução da massa de células

beta em pacientes com diabetes tipo 2.(30) Experimentos em macacos (modelo

primata não-humano) também mostraram uma correlação com a hiperplasia de

células alfa, levando à deficiência de insulina, hiperglucagonemia e resistência

à insulina.(31)

A perda de peso observada nos ratos não-diabéticos tratados com doses

de 200 mg/Kg ou 100 mg/Kg sugere efeito adicional no controle da obesidade.

A perda de peso pode ter refletido a redução do consumo alimentar ou a menor

absorção alimentar em resposta aos taninos presentes no extrato vegetal.(23)

Estudos analisando o consumo alimentar e hídrico nesses animais estão em

andamento para elucidar o mecanismo responsável pelo efeito antiobesidade

observado.

O mesmo efeito hormese pode ser observado na variação de peso nos

animais não-diabéticos, que apresentaram menor ganho de peso com a dose

de 100 mg/kg, seguida pela dose de 200 mg/kg.(26) A diferença de peso

corporal inicial entre os animais levou à adoção da variação de peso (peso

inicial menos peso final) como parâmetro nesse estudo.

Houve efeito positivo sobre os níveis de colesterol total, porém redução

do colesterol HDL. O colesterol HDL transporta o colesterol para o fígado, onde

o mesmo é captado pelos receptores SR-B1, contribuindo para a proteção do

leito vascular contra a aterogênese por meio de mecanismos como a remoção

de LDL oxidada, a inibição da fixação de moléculas de adesão e monócitos ao

endotélio e o estímulo da liberação de óxido nítrico.(32)

A redução do colesterol total é salutar para a população em geral e para

indivíduos diabéticos em particular. Entretanto, e redução do colesterol HDL

pode aumentar o risco cardiovascular, o que não é desejável em indivíduos

diabéticos.

O modelo experimental de diabetes induzida por altas doses de

estreptozotocina empregado é uma ferramenta útil para o estudo de diversos

aspectos relacionados ao diabetes tipo 1.(33) Neste estudo, o extrato

hidroalcoólico de Plathymenia reticulata Benth administrado pela via oral

promoveu redução importante da glicose sanguínea e levou à hiperplasia de

ilhotas no animais. Tais resultados podem representar o início do processo de

obtenção de uma nova droga de uso oral para tratamento do diabetes tipo 1 e

possivelmente tipo 2.

CONCLUSÃO

Outros estudos se fazem necessários para desvendar os mecanismos

relacionados aos efeitos do extrato, investigar sua toxicidade e identificar o

composto responsável pelos efeitos descritos. Este estudo abre a perspectiva

para novas terapias para o diabetes tipos 1 e 2, baseadas no efeito

hipoglicemiante e na preservação das células beta e, ainda mais importante,

para a identificação de um composto capaz de prevenir o desencadeamento do

diabetes tipo 2 em indivíduos susceptíveis por meio da proteção pancreática e

de perda de peso. Os efeitos benéficos descritos podem refletir a capacidade

do extrato vegetal de promover a neogênese ou restaurar a dediferenciação de

células beta.
