Online Application:

1. Go to Undergraduate Application Form and fill out all the information requested.

2. Review the information you have entered.

3. Click on the [Confirm] button to display the Confirmation page. A Confirmation number will be assigned to you. You may still review the information you have provided. If there are corrections, click on the [Back] button.

4. Click on the Print button in the Confirmation page to print the application form on a legal size (long) bond paper. Attach your passport photo.

5. Submit this Form together with the other requirements to the DLSU College Admissions Office.

Requirements & Procedures:

1. Proceed to the College Admissions Office and submit the completed printed application form with all the required documents.

1.1. Certified Correct Secondary Scholastic Records (pdf)(Students who have graduated high school/secondary level will have to submit a photocopy of their 4th year high school report card or high school Transcript of Records for foreign applicants.)

1.4. Photocopy of National Statistics Office (NSO) Birth Certificate

2. Pay the non-refundable College Admissions Processing Fee (includes payment for the College Admissions kit, testing and processing) at the Accounting Office.

For Filipino citizens and dual citizens: Php 600.00 For international students (non-Filipino Citizens): US$ 50.00

3. Present the Official Receipt to the College Admissions Office and secure the testing permit. Your entrance exam schedule will depend on the earliest testing date available upon submission of requirements. (Only applications with correct and complete information/requirements will be processed.)

FOR WALK-IN APPLICANTS:

1. Proceed to the Admissions Office for inquiries.

2. Pay the non-refundable College Admissions Processing Fee (includes payment for the College Admissions kit, testing and processing) at the Accounting Office. o For Filipino citizens and dual citizens: Php 600.00o For International students (non-Filipino Citizens): US$ 50.00

3. Present the Official Receipt to the Admissions Office and secure the College Admissions kit that includes the application requirements.

4. Follow steps 1 to 4 of the Online Application above.

5. Submit the completed Application Form with all the required documents (see Requirements & Procedures nos 1.1 to 1.4) at the Admissions Office before your entrance exam date to secure your exam permit. Your entrance exam schedule will depend on the earliest testing date available upon submission of the requirements. General Requirements Manila residents who are graduates of public high schools of the Division of City Schools (DCS), Manila and other high schools in and outside Manila recognized by the Department of Education (DepEd) may be admitted as freshmen into the University subject to the following: 1. presentation of appropriate documents satisfying the qualifying criteria 2. performance in the PLM Admission Test

Qualifying Requirements Bring the original and photocopy of the ff.: Fourth year High School Report Card (Form 138: Original or Certified True Copy) with a General Weighted Average (GWA) of at least 80% or better Barangay certification of Manila Residency Parent’s latest Voter’s ID/Voter’s Registration Record, SK Voter’s ID Parent’s Income Tax Return/Real Property tax receipt (2009), Company ID

Application Requirements Upon satisfaction of the qualifying criteria for application, please submit the following requirements: 1. Duly accomplished PLMAT Form No. 1 (Application Form); and 2. Two 1.5" x 1.5" identical photographs taken within the last six (6) months.

Note: Previous PLMAT applicants who did not qualify for admission need not apply again. Admission Privileges 1. Manila residents who are honor graduates of public high schools of the Division of City Schools-Manila (Valedictorians, Salutatorians, and 1st to 3rd Honorable Mentions) shall automatically qualify for admission upon presentation of a certification issued by a duly authorized school official indicating the honor received. Note: If the certification is not available yet upon application, they may take the PLMAT; however, if they qualify for honors, they shall be automatically admitted. 2. Graduates of other high schools recognized by the Department of Education (DepEd) who are either Valedictorians or Salutatorians may qualify for admission upon passing the PLMAT. 3. Children/direct descendants of PLM employees may qualify for admission upon passing the PLMAT. PLMAT Examinee’s Report The PLMAT Examinee’s Report is a system-generated output which indicates whether or not the applicant is qualified to enroll in PLM. Qualified applicants will be advised to see the Office of the University Registrar for enrollment purposes. This brochure, the PLMAT application form, and the PLMAT fee are all FREE. Important: Upon passing the PLMAT, graduates of public high-schools under the Division of City Schools-Manila and all schools in and outside Manila who are residents of Manila shall be admitted as full (non-paying) scholars. All other PLMAT passers shall be admitted as highly-subsidized (minimal-paying) scholars. Academic Programs The different colleges of the University offer academic programs based on the approved curricula for the particular field of study. Usually, the programs require the teaching of the courses of study by leaders in the business or industry and the conduct of actual field exposures. The participative involvement of practitioners in the formation of courses of study in the different colleges is reflected in the skills-oriented, community-directed and value-driven curricular programs of the University.

Newsflash for students currently in third year high school. We now have word on the next UPCAT application and exam schedule. Call it UPCAT 2011 (for it would be administered this year) or call it UPCAT 2012 – call it what you will, but we’re referring to the UPCAT Application for Freshman Admission for the First Semester of the 2012-2013 academic year.

The deadlines for submitting your UPCAT application form are the following:

June 17, 2011 (Friday) – if you’re from a MM school June 24, 2011 (Friday)- if you’re from a non-MM school

The UPCAT  for the first semester of the  2012-2013 freshmen intake year will be administered on the following dates:

August 6, 2011 (Saturday) August 7, 2011 (Sunday)

As usual, the UPCAT will be held in various testing centers nationwide. There will be two testing schedules each day: morning and afternoon. Every UPCAT taker will be assigned to only one UPCAT testing schedule and will be assigned only to one UPCAT testing date. In other words, it’s a one-shot deal so take your UPCAT in your testing date and testing schedule.

Important: You don’t have to be in the top 10 percent (or top-whatever-percent) of your class to take the UPCAT. The UPCAT is open to everyone (as long as specific non-grade eligibility requirements are satisfied, of course).
