Montgomery College Office of Concurrent Enrollment Programs Application Form Program Requirements: College courses offered through MC’s Concurrent Enrollment Programs (OCEP) are open to high school junior and senior students who meet the College’s Early Placement requirements as follows: 1. Be at least 16 years of age. 2. Have an overall GPA of 3.0 (juniors) or 2.75 (seniors) 3. Meet the ACCUPLACER assessment levels and prerequisites for the requested course. o Qualifying ACT or SAT scores may be used in lieu of the ACCUPLACER: 550 on SAT Critical Reading test or 24 on ACT English test. (EN 101 assessment level) 550 on SAT Math test or 24 on ACT Math test. (MA 97/99 assessment level) 4. Obtain High School approval to participate. Applications will not be complete until all material has been received by MC’s Office of Concurrent Enrollment Programs. Application Checklist Online portion of the application. Copy of high school transcript (provided by the high school). Score report from ACCUPLACER, SAT or ACT tests. Signed copy of this form (page 2). Submit all materials to high school Dual Enrollment Program Assistant (DEPA). Students are individually responsible for all tuition, fees and textbook expenses. Non-attendance does not relieve a student from this financial responsibility. College and Program Expectations: All Montgomery College students, including students taking classes in the high school, are bound by the Montgomery College Academic Regulations and Student Code of Conduct. The curriculum, including content, objectives and materials of Montgomery College courses are determined by the department, in accordance with the college’s academic regulations and standards regardless of class location. Montgomery College courses offered through OCEP do not count toward high school requirements or appear on the high school transcript. Students are individually responsible for all tuition, fees and textbook expenses. Students with demonstrated need may apply and receive financial assistance based on available funds. Students who fail to make arrangements prior to the payment deadlines risk financial penalties and/or withdrawal from class. Non-attendance does not relieve a student from this financial responsibility. Educational Records: When a student turns 18 years old or enters a postsecondary institution (such as Montgomery College) at any age, all rights afforded to parents under FERPA transfer to the student. By participating in this program through OCEP, students understand that Montgomery College has the authority to release their educational records to Montgomery County Public Schools I have read and understood the program and College expectations and requirements. If admitted, I agree to abide by policies and procedures of MC (http://cms.montgomerycollege.edu/pnp). Additionally, I understand that I am responsible for all costs including collection costs to satisfy my debts to MC. Upon request by a designated MC official, I agree to provide evidence of the information on this application for verification.
