Moderate RiskRecurrent VTEBileaflet aortic valve prosthesis and ONE of the following: Afib, prior Stroke/TIA, HTN, Diabetes, CHF, Age >75yrsAtrial Fibrillation & CHADS2 score of 3-4VTE within previous 3-12 monthsNon-severe thrombophilic conditions (e.g., heterozygous Factor V Leiden or Factor II gene mutation)Active Cancer (treated within 6-months or palliative)

High RiskAny Mitral Valve ProsthesisAortic Valve Prosthesis except bileafletRecent (< 6 months) stroke/TIA Atrial Fibrillation & CHADS2 Score 5-6Rheumatic Valvular Heart DiseaseRecent VTE (< 3 months)Severe thrombophilia (e.g. Prot. C def., Prot. S def., Antithrombin III def., Antiphospholipid Antibody Syndrome or multiple abnormalities

Moderate - High Risk

Patient with:Bileaflet aortic valve prosthesis without Atrial fibrillation & no other stroke risk factorsAtrial fibrillation & CHADS2 score 0-2 (and no prior stroke/TIA)Single VTE occurred >12 months ago and no other risk factors

Low-Risk

CHADS 2 CHF = 1point HTN = 1point Age >75 yrs = 1point Diabetes = 1point Stroke/TIA = 2 points
