• The report is produced by our proprietary software for measuring and monitoring health

• Physician contact can be done face-to-face or on the phone where necessary

• “Red flag“ situations are referred to Eternity partner specialists

HR MIS – Corporate Health DashboardA number of statistics can be provided in report format that provide HR with valuable information on the company‘s overall health.

This helps with the scheduling of workshops that Eternity would carry out – for example, to discuss diet and diabetes, heart health, etc.), and others.

Step 1: Review insurance plan

• If health screening covered by insurance, match up program(s) to insurance 1-1

• If no insurance, company decides budget for screening and we customize program(s) accordingly

Step 2: Eternity Medicine Institute proposal review

• Eternity prepares proposal according to insurance-covered- or non-insurance covered-programs

• Proposal to include specifications for portal customization

Step 3: Portal development through to launch

• Upon acceptance of proposal, Eternity‘s development team begins portal customization (no cost to client)

• Two-to-three week lead time to launch
