FRIDAY, JUNE 15 08:00-17:00 ADVANCED: Perioperative, Hands-on Point of Care Ultrasound Course (POCUS) Pre-Conference Workshop

This one day hands-on course will focus on advanced applications of POCUS in perioperative medicine. These will include advanced application of cardiac, and lung US, FAST, Trans-cranial Doppler, Optic nerve sheath diameter and Deep Venous thrombosis assessment. All instructors are experts in the field from across the country. The course is entirely hands-on and will require completion of online learning pre-course and post-course tests. Scans will be performed using a wide variety of equipment on healthy volunteers and simulators. Image interpretation will be practiced on real US images. First offered at the 2017 Annual Meeting, this workshop garnered top participant scores, and outstanding presenter and content reviews. We have brought it back in response to member requests. Don’t miss out! This workshop focuses on the fundamental skills of negotiation, and adds an in-depth examination of conflict management, using a combination of lecture and short interactive exercises. The workshop was co-developed for anesthesiologists, to address profession-specific scenarios, and is not intended for other health professionals. The negotiation component will cover topics such as how to plan and prepare for a negotiation, how to assess your options, and how to create value for a more optimal agreement. In the conflict management section, participants will practice techniques to enhance their active listening skills, particularly for managing difficult communications; learn how to approach potential conflict; develop competencies for effectively resolving disagreements; and discuss best practices. Case of Difficult Airway in a Morbidly Obese Patient For Ambulatory Shoulder Surgery Difficult airway and obstructive sleep apnea (OSA) may contribute to perioperative morbidity and mortality.[1,2] Although the incidence of the difficult intubation (1-6%) and failed intubation (0.1-0.3%) are very low, it can contribute to increased risk of airway trauma, rapid desaturation, laryngeal injuries, unexpected intensive care unit admission and death. The different abnormalities in the upper airway anatomies like a large tongue, overcrowding of the oropharyngeal structures, decreased upper airway diameter and greater neck circumference may contribute to the difficult airway in OSA patients during the perioperative period. Getting the Most Out of an Echo Report Echocardiograms are commonly ordered pre-operatively, yet the literature does not support their efficacy in improving outcome. Is this because we are falsely reassured by the written conclusion "normal EF"? Recent anaesthesia interest in diastolic dysfunction and in "heart failure with preserved ejection fraction" (HFPEF) means that practitioners must understand echocardiography reports at the highest level. Frontiers in Cardiovascular Thoracic Anesthesia: New Frontiers in Lung Transplantation There is a growing patient population with end-stage lung disease that would gain survival benefit from lung transplantation. Unfortunately, there continues to be such a large shortage in donor lungs that >1/3 patients on lung transplant recipient waitlists do not survive to getting a transplant. This issue is further compounded by a very low rate of donor lung utilization, where <25% of lungs donated are accepted for transplantation. Ex-Vivo Lung Perfusion (EVLP) provides a platform to assess, preserve, and repair donor lungs leading to an increase in donor lung utilization. Currently, all clinical EVLP devices use positive pressure ventilation that results in barotrauma and decreased potential for lung recruitment. We describe a novel negative pressure ventilation system for EVLP that was developed pre-clinically and is now in safety first in human clinical trials to improve outcomes during EVLP. Are We There Yet? Bridging the Gaps Towards ERAS Adoption ERAS proponents have recognized in the literature, lectures and informal conversation, that ERAS adoption has reached a plateau. AF is particularly significant in cardiac surgery, where a third of patients will suffer from it perioperatively. Transthoracic Echocardiography in Obstetric Anesthesia – Is it Time we Knew our Patients’ Hearts? The use of Transthoracic Echocardiography (TTE) in anesthesia has been growing and becoming a common first line diagnostic procedure in elective and emergency situations. In obstetric anesthesia, profound cardiovascular changes of pregnancy combined with evolving events of labor and delivery can cause rapid deterioration. TTE at the bedside can quickly establish cardiac pathology and allow for timely and appropriate treatment in the peri-partum period. In high risk parturients, TTE can allow for monitoring and optimization of cardiac function and volume status thus minimizing the risk of peri-partum complications. This session aims to address current uses for TTE in obstetric anesthesia practice through review of literature and case examples. 09:15-10:00 Anesthesia for Patients on LVAD Undergoing Non-Cardiac Surgery The indications for implantation of Left Ventricular Assist Devices (LVADs) for end-stage heart failure have evolved in the past few years, leading to a constant increase in the number of patients with implanted LVADs. These patients may need non–cardiac interventions and these procedures may present a particular challenge to the Anesthesiologist. Successful perioperative management of non-cardiac procedures in this specific group of patients mandates an understanding of the indications for implantation, the patients’ native cardio -circulatory system status, comorbidities, need for anti coagulation and proper positioning and monitoring, and how to prevent and treat potential complications. 10:30-11:15 Brain Stress Test: How it Works, What it Reveals, What it Means Vascular organization and physiology can be interrogated in ambulatory subjects by applying a vasoactive stimulus in the form of standardized change in end-tidal PCO2 and monitoring regional changes in perfusion using MR imaging. This results in functional images related to the hemodynamic significance of vascular lesions and the regional perfusion reserve. Such imaging are highly predictable of risk of stroke and cognitive decline. It may also have a role in identifying indications for surgery, and predicting perioperative risk of stroke and delirium.
