IRTS & HOUSING REQUEST PROCESS & INFORMATION SHEET

Certain request for placement or funding is to be reviewed by the Supervisor Review Team, which meets every Tuesday and Thursday beginning at 9:00 am.

Requests that need to be reviewed:

Request for housing Subsidies Request for IRTS (Rule 36) Placement Long-Term stay at Cochran (over 45 days) Hotel/Motel stays over 1 Week

PROCESS1. The Social Worker must first complete a Housing Request Form (DAK617) or IRTS Request Form (DAK616) and attach the most

current LOCUS or APCCA Score Sheet.The Social Worker will take or fax a copy to the Adult Services Case Aide.(Fax #651 554-6043).

2. After completing the appropriate form, the Social Worker will schedule a time slot to discuss client’s initial or ongoing housing needs, housing subsidy needs or IRTS placement needs. The requests for both are scheduled through the Adult Services Case Aide. (651- 554 – 6381). The Social Worker may attend the meeting or participate by conference call. (651 554-6361.

3. Copies of the forms: are made (3) sets for the housing review committee and distributed with Housing Schedule Sheet. Included is the Housing Subsidy Encumbrances and the IRTS Access database are printed and given with the other forms.

4. The Supervisor Review Team will review the request and approve or disapprove. If the request is disapproved, the team will assist in finding alternatives. If the request is approved, the supervisor team will complete the form with approval amount and if another review will be scheduled. IRTS request are generally faxed over review date and Social Workers will be called so as to be prepared.

5. The housing Case Aide will send copies back to the Social Workers on all review and will be expecting a 504 on rent amounts once approved or placed in apartment. Payments cannot be made without a 504.

6. For reviews complete the review parts of the Housing/IRTS paperwork and send to the Housing Case Aide. The housing Case Aide will contact Social Worker when a review is scheduled and paperwork should be completed and sent to Case Aide the day before review date. Please let the case aide know if there is any change in the Subsidy or IRTS placements.

7. If an extension is needed on a Housing Subsidy/ IRTS call the Case Aide and schedule the review in advance of time period ending, complete paperwork and give to case aide.

8. The Case Aide will pay the subsidy at the 4-5 days before the end of each month.

Medical TransportationThe county no longer pays for medical mileage or parking, etc. Minnesota Non-Emergency Transportation or MNET is now making payment.

If you have a medical appointment or other need using your health care card, you must call MNET before the appointment to get prior approval for payment of mileage, parking and other related costs.

You do not need to fill out a claim form. MNET will figure the mileage and verify the appointment. Payment will be sent to you or your driver.

The payment rate for you or your drive is 20-cents per mile.
