Before You Begin…

Before you can complete this procedure, the following conditions must be met:

You must be a Facility Editor or a Network Patient Editor.

You must have scope over the facility to which you are admitting the patient.

Admitting Transient Patients

You must admit a patient as a transient when the admission is temporary—less than 30 days or no more than 13 treatments.

Admitting a patient as a transient allows the patient to remain admitted to his/her home facility in CROWNWeb (without being discharged), while temporarily visiting your facility.

Admitting a Transient Patient

The following interactive tutorial will guide you through admitting a transient patient in CROWNWeb.

1. Click Patients From the CROWNWeb Home Page, click Patients. The Search for Patients screen displays.

2. Click Admit Patient Click Admit Patient from the sub-menu. The Admit Patient screen displays.

3. Enter Patient Information Enter the admit information for the transient patient and then select Yes for the Transient Status. Click Next. The screen expands and transient information is required.

4. Enter Transient Information Enter the Transient Reason and any extra transient information for the patient, and then click Next. The Patient Admission Confirmation screen displays.

5. Patient Admission Confirmation CROWNWeb prompts you to confirm that you are admitting a transient patient. Click Accept. The Admit a Patient - Add Treatment screen displays.

6. Enter Treatment Information Enter treatment information for the patient, and then click Next. The Edit Patient Attributes screen displays.

7. Exact Match Message The Edit Patient Attributes screen displays the message "This patient was an exact match for the New Patient info you entered." Scroll to the bottom of the screen, and click Submit.

8. Transient Details SUBMITTED The View Patient Attributes screen displays the message “View Patient Details – SUBMITTED.”

Clinicals for Transient Patients

Transient patients are not required to have labs drawn until after they have had six treatments at your facility.

If a transient patient does not have any labs drawn during their visit, the facility must complete the minimum required fields in HD or PD clinical (those with a red asterisk (*) or have a checkbox, such as Sodium Education and the Vaccinations).

The minimum required fields do not require lab data—just a simple selection or data entry.

Discharging Transient Patients

The following interactive tutorial will guide you through discharging a transient patient in CROWNWeb.

1. Click Patients From the CROWNWeb Home Page, click Patients. The Search for Patients screen displays.

2. Enter Search Criteria Enter search criteria to find the patient, then click Search. The Patient Search Results screen displays a list of all patients who match the criteria entered.

3. Click CROWN UPI Click the CROWN UPI of the patient. The View Patient Attributes screen displays.

4. Click Admit/Discharge Summary Click Admit/Discharge Summary in the gray sub-menu. The Admit/Discharge Summary screen displays.

5. Click Admit Date Click the Admit Date for the date that the transient was admitted to your facility. The View Admit/Discharge Information screen displays.

6. Click Edit Admit/Discharge Click Edit Admit/Discharge in the gray sub-menu. The Edit Admit/Discharge Information screen displays.

7. Enter Patient Discharge Information The fields are now enabled for editing. Go to the Patient Discharge Information section and:

Enter the Discharge Date and select a Discharge Reason of Transfer.

The Transfer Discharge Subcategory field becomes active; select Dialysis Facility from the pull-down menu.

Click Submit.

8. Patient Discharged The Admit/Discharge Summary screen displays the message "Patient Admit Discharge Submitted." The Discharge Date and Discharge Reason are now populated on the admit/discharge summary.

Troubleshooting

Commonly experienced problems, recognized solutions, and other key information:

You must have scope over a facility before you can admit a transient to that facility.

Only a Facility Editor or Network Patient Editor can admit a transient patient to a facility.

If an out-of-scope near match is found during the admission process, your Network Patient Editor will help you admit the patient.

Summary

To admit a transient patient in CROWNWeb:

1. From the Home Page, click Patients.

2. Click Admit Patient.

3. Enter preliminary patient information. For Transient Status select Yes.

4. Enter the Transient Reason and patient’s transient information. Click Next.

5. Click Accept on the Patient Admission Confirmation screen.

6. Enter treatment information. Click Next.

7. Click Submit. To discharge a transient patient in CROWNWeb:

1. From the Home Page, click Patients.

2. Search for the patient.

3. Click on the patient’s CROWN UPI.

4. Click Admit/Discharge Summary.

5. Click on the date link in the Admit Date column.

6. Click on Edit Admit/Discharge.

7. Enter the Date of Discharge, Discharge Reason (Transfer), and Transfer Discharge Subcategory (Dialysis Facility).

8. Click Submit.
