Borrower/s: Date: Completed Loan Builder Calculations - This comes from the Initial Scenario Request submitted to FGMC. Max Base Loan Amount on LoanBuilder should match Max Base Loan amount on 1003. Lot / Land Acquisition - If Building on own land, a current payoff will be needed if there is an existing lien. If land is to be purchased, a Purchase Agreement for the land must be included. Purchase Agreement / Contract for Home and Improvements - Purchase Agreement should reflect LoanBuilderCalculations. Manufactured Construction Cost Breakdown - This will be provided by the Construction Resource Team and will be based on the LoanBuilder calculations Specific Pages from the Blueprints/Plans (listed below) with Dated Builder and Borrower Signatures on each page (if VA, refer to VA Handbook 10.10 Construction Exhibits for Preparation) a. Floor Plan/s b. Foundation Plan c. Elevation Pages showing the Front, Rear, and Sides of the Home Builder's Certification - FHA uses HUD-92541. For Manufactured homes mark items 2,3,4,5,8 The following documents are Required for Project Underwrite. Failure to include these documents will result in the file being suspended until they are received. Structural Engineering Cert of the Foundation Plans - This requires both the foundation plan sealed ansigned by a Structural Engineer, and a letter stating that the Foundation will meet all local and HUD requirementsDescription of Materials with Dated Borrower and Builder signatures - FHA/VA Only - (FHA requires signedHUD-92005, VA requires either HUD-92005 or VA 26-1852)Proposed Plot Plan completed by Builder - Not required to be to scale, and must show Home placement onthe lot, and must include the distances to well and septic (if applicable)Manufactured Home Construction Draw Disclosure

Manufactured Home Construction Requirements - (FHA/VA loans only)Disbursement Authorization and Appointment of Agent - Must be completed or Borrower approval will berequired for all disbursements of funds.Agreement Regarding Payment of Interest

FHA/VA Construction/Permanent Loan Disclosure - Only on FHA and VA loans. Must also be included inLender's HUD case binder.Builder Attestation Form Regarding State and Local Licensing

Completed "As-Complete" Appraisal - This should typically be ordered after the disclosure of the loan. a. Wholesale - FGMC will be responsible for the ordering of FHA and Conventional appraisals VA appraisals must be ordered by the submitting Broker b. Correspondent - Appraisal ordering will be the responsibility of the submitting Correspondent

Interest Rate Disclosure - To be completed once the loan has been locked. Must match lock terms
