Tax Information Required for Verification

If you do not have a copy of your 2015 federal income taxes, you may obtain a 2015 Tax Return Transcript from the IRS to submit to us instead.

For information on obtaining the following documents from the IRS: • Tax Return Transcripts • Verification of Nonfiling • Wage and Income Transcripts (instead of W-2s)Please refer to detailed information online: www.jccc.edu/admissions/financial-aid/obtain-tax.html.

Individuals Who Filed an Amended IRS Income Tax ReturnIf an individual filed an amended IRS income tax return for tax year 2015, provide both of the following: • A signed copy of the 2015 federal income taxes; and • A signed copy of the 2015 IRS Form 1040X, “Amended U.S. Individual Income Tax Return,” that was filed with the

IRS.You may be required to have the documents stamped by the IRS.

Individuals Granted a Filing Extension by the IRSIf an individual is required to file a 2015 IRS income tax return and has been granted a filing extension by the IRS, provide the following documents: 1. A copy of IRS Form 4868, “Application for Automatic Extension of Time to File U.S. Individual Income Tax Return,”

that was filed with the IRS for the tax year 2015; and 2. A copy of the IRS’s approval of an extension beyond the automatic six-month extension if the individual requested an

additional extension of the filing time for tax year 2015; and 3. A Verification of Nonfiling letter from the IRS dated on or after October 1, 2016; and 4. A copy of IRS Form W-2 for each source of employment income received for tax year 2015 and, if self-employed, a

signed statement certifying the amount of the individual’s Adjusted Gross Income (AGI) and the U.S. income tax to be paid for tax year 2015.

5. When your taxes are filed, you must then provide copies of your original IRS Tax Return Transcript.

Individuals Who Were Victims of IRS Identify TheftAn individual who was the victim of IRS tax-related identity theft must provide: • A Tax Return DataBase View (TRDBV) transcript obtained from the IRS, or any other IRS tax transcript(s) that includes

all of the income and tax information required to be verified; and • A statement signed and dated by the tax filer indicating that he or she was a victim of IRS tax-related identity theft and
