Clinical OUTCOME

• TELEMETRY MONITORING for 24 to 36 hr. • Eparin Na+ iv for 24 to 36 h.• Oral anticoagulant therapy (ACT) started 24 h post- procedure.• ECHO evaluation pre-discharge.• Discharged with ACT (maintained for 1-3 Mo.)• Discharged with AADs (maintained for 2 Mo.) (20% with propafenone, 65% with flecainide and 15% Sotalol)
Post-ablation MANAGEMENT

• Discharge with 1 drug that, in the past seemed to be well tolerated although unable
to prevent AF.
• AAD and ACT discontinued after 3 mo in the absence of AF recurrence, unless other risk factors were present.
• Other medications prescribed when indicated.

• Serial ECHO and HOLTER monitoring performed at discharge and at 3, 6, 12 and 18 mo • Procedure was considered successful if NO RECURRENCES of AF lasting >30 s were present during post-discharge FU. • TEE performed at 6 mo after ablation to assess potential PV narrowing.

As it is intrinsically unlikely that SR is per se harmful to the patient’s life, we believe that the quest for safer and more effective techniques (RFCA) for curing AF will, and should, continue.
Only large and prospective or randomized clinical studies in comparison between RFCA of PV and alternative approach (rate CTR, AADs Rx for prevent AFib , Ablate and Pace etc) for Rhythm CTR and for Ventricular rate based strategies will give us the ANSEWERs our question on best treatment for AFib
